  • Can be caused by a wide variety of pathogenic processes, including, but not limited to, vitamin D deficiency (reduced exposure to sunlight, poor nutrition, malabsorption syndromes)
  • Defective metabolism of parent vitamin D to active metabolites (drug-induced, i.e., anticonvulsants - phenytoin (Dilantin), chronic renal failure), hypophosphatemia (renal tubular acidosis, hypophosphatemic syndrome), miscellaneous (long-term hemodialysis, malnutrition, vitamin D-dependent rickets)

    Diseased hip Definition: Osteomalacia involves softening of the bones caused by a deficiency of vitamin D or problems with the metabolism of this vitamin. Causes And Risk: There are numerous causes of osteomalacia. In children, the condition is called rickets and is usually caused by a deficiency of vitamin D . Conditions that may lead to osteomalacia include inadequate dietary intake of vitamin D, inadequate exposure to sunlight (ultraviolet radiation), which produces vitamin D in the body and malabsorption of vitamin D by the intestines. Other conditions that can cause osteomalacia include hereditary or acquired disorders of vitamin D metabolism kidney failure and acidosis ; phosphate depletion associated with low dietary intake of phosphates; kidney disease or cancer (rare), and side effects of medications used to treat seizures Risk factors are related to the causes. In the elderly, there is an increased risk amongst those who tend to remain indoors and who avoid milk because of

Department of Endocrinology, West Middlesex University Hospital, London, UK. Most patients with vitamin D deficiency and osteomalacia have elevated parathyroid levels with normal or low serum calciums .This hyperparathyroidism is described as secondary hyperparathyroidism and resolves with correction of vitamin deficiency. We describe three cases where there was coexistant osteomalacia vitamin D deficiency, elevated parathyroid hormone and hypercalcaemia. Case 1 A 40 year old Asian female presented with a six month history of bone pain. She had a waddling gait. Investigations revealed calcium 2.83mmol/l and alkaline phosphatase 779 IU/l, PTH 668 ng/l (12-72), 25-hydroxy Vitamin D was undetectable. Radiographs showed generalised osteopaenia and hyperparathyroid bone disease. She was commenced of Ergocalciferol and her calcium rose to 3.04 mmol/l. The PTH level remained elevated after 3 months. She had a parathyroidectomy. At histology there was evivdence of an underlying parathyroid carcinoma. Case 2 A 41 year old Asian lady was complaining of pain in her right hand and forearm. She had a calcium of 2.76 mmol/l. PTH was 19.1 IU/l (1.05-6.84) and an undetectable 25-hydroxy vitamin D. She has been commenced on Ergocalciferol, and is now pregnant.

    How do osteoporosis and osteomalacia differ? Osteomalacia is a defect in mineralization of osteoid (lack of incorporation of calcium and/or phosphate into the osteoid protein bone matrix). Osteoporosis is a reduction in the quantity of normally mineralized bone. Radiographically, osteomalacia and osteoporosis may be indistinguishable, as both may show diffuse osteopenia. In osteomalacia, however, looser zones or pseudofractures may be seen, although these findings are rare. Looser zones are small fractures that develop perpendicular to the long axis of the bone and are often bilaterally symmetric. These radiographic lucencies are caused by the lack of mineralization. Osteomalacia may be mild and asymptomatic or severe, predisposing people to bone deformities, pain, and pseudofractures. Symptoms include vague bone pain, generalized myopathy (particularly in the proximal muscles), and bone tenderness of the appendicular skeleton. Osteoporosis, on the other hand, predominantly affects the axial skeleton and is asymptomatic until fractures occur, leading to acute and chronic pain. It is imperative to consider osteomalacia in any individual with low bone density, because the treatment differs from that for osteoporosis.
