This disease involves softening and weakening of bones in children due to deficiency of Vitamin D. This softening occurs from a loss of the mineral calcium from the skeleton as a result of which bones become flexible and gradually are molded by forces, such as bearing weight, that are exerted on them. This condition could lead to deformities of the body structure.
Rickets is most likely to occur during periods of rapid growth, when the body demands high levels of calcium and phosphate. Rickets may be seen in young children 6 to 24 months old and is uncommon in newborns.

Introduction
Osteomalacia is the failure of bone and/or the epiphyseal cartilage to adequately mineralize. In children, this is called rickets while, in adults, it is called osteomalacia.
Causes
The underlying causes include:
Dietary deficiency
A deficiency of Vitamin D, calcium, or phosphate in the diet is a cause of osteomalacia or rickets.
Gastrointestinal disorders
Osteomalacia, or rickets, can be traced to gastric disease, hepatobiliary disease, enteric disorders, malabsorption syndromes, chronic diarrhea, post-surgical malabsorption, or any disorder which inhibits the absorption of adequate amounts of calcium in the gut.
Vitamin D "resistance"
Historically, this group of metabolic disorders, which result in rickets or osteomalacia, was so named because of their apparent resistance to treatment with Vitamin D. More importantly, these patients are now known to have an abnormality in the kidney and/or biochemical pathway(s) that results in systemic manifestations. There are four pathogenic mechanisms which account for the bone problems seen in these disorders:
Phosphate wasting
Hypophosphatemic rickets, or phosphate diabetes (still sometimes called Vitamin D resistant rickets), is a sex-linked dominant disorder of the renal tubule resulting in inability to resorb phosphate. The calcium, PTH, and Vitamin D levels are normal, but serum phosphate is low while urine phosphate is elevated. Treatment consists of phosphate replacement.

J Korean Med Sci 1994 Aug;9(4):289-98 Abstract quote Oncogenic osteomalacia is an unusual and rare clinicopathologic syndrome characterized by mesenchymal tumors that apparently produce osteomalacia and biochemical abnormalities consisting of hypophosphatemia, normocalcemia, and increased levels of alkaline phosphatase. We collected from the Mayo Clinic files and from our consultation files the records for 17 cases of osteomalacia associated with bone lesions. There were five cases of fibrous dysplasia, three of hemangiopericytoma, and two of phosphaturic mesenchymal tumor. There was one case each of osteosarcoma, chondroblastoma, chondromyxoid fibroma, malignant fibrous histiocytoma, giant cell tumor, metaphyseal fibrous defect, and hemangioma.

Causes, incidence, and risk factors: There are numerous causes of osteomalacia. In children, the condition is called rickets and is usually caused by a deficiency of vitamin D . Conditions that may lead to osteomalacia include inadequate dietary intake of vitamin D, inadequate exposure to sunlight (ultraviolet radiation), which produces vitamin D in the body and malabsorption of vitamin D by the intestines. ABSTRACT Background Mutations in fibroblast growth factor 23 (FGF-23) cause autosomal dominant hypophosphatemic rickets. Clinical and laboratory findings in this disorder are similar to those in oncogenic osteomalacia, in which tumors abundantly express FGF-23 messenger RNA, and to those in X-linked hypophosphatemia, which is caused by inactivating mutations in a phosphate-regulating endopeptidase called PHEX. Recombinant FGF-23 induces phosphaturia and hypophosphatemia in vivo, suggesting that it has a role in phosphate regulation. To determine whether FGF-23 circulates in healthy persons and whether it is elevated in those with oncogenic osteomalacia or X-linked hypophosphatemia, an immunometric assay was developed to measure it. Tumor-induced osteomalacia (also known as oncogenic osteomalacia) is a rare disorder characterized by phosphaturia, hypophosphatemia, and osteomalacia mimicking the clinical phenotype of either X-linked or autosomal dominant hereditary hypophosphatemic rickets. Tumor-induced osteomalacia develops because of tumors that are predominantly of benign mesenchymal origin but that may occasionally be malignant, as was recently reported.
