  • Overview . Osteoarthritis (OA) is the most common joint disease. OA is a condition of synovial joints characterized by focal cartilage loss and an accompanying reparative bone response. Typical radiographic features are joint space narrowing and the presence of osteophytes and sclerosis. OA is strongly related to age. It is rare under 45 years of age without trauma, but at least half of those over age 65 years have radiographic evidence of OA.
  • Types
  • Primary OA . Primary OA is a wear-and-tear phenomenon. OA generally spares the shoulders and MCP joints. Secondary OA . May involve joints that are generally not involved with primary OA, including MCP joints, shoulder, or isolated large joints. It may be related to chondrocalcinosis or another secondary cause (below).
  • Traumatic arthritis secondary to slipped capital femoral epiphysis, congenital hip dislocation, destruction secondary to septic joint, hemophilia, or other injury.

    What is the OAI?
    The OAI Osteoarthritis Initiative is a nationwide research study, sponsored by the National Institutes of Health knee osteoarthritis , one of the most common causes of disability in adults. OAI is a four-year study and will recruit 5,000 men and women aged 45 and above at high risk for developing symptomatic knee osteoarthritis. Osteoarthritis causes more health problems and medical expenses than any other form of arthritis. Symptoms of osteoarthritis can range from stiffness and mild pain to severe joint pain and even disability. As the U.S. population swells with graying baby boomers, vast numbers of people will suffer from osteoarthritis. Today, 35 million people, 13 percent of the U.S. population, are 65 and older, and more than half of them have radiological evidence of osteoarthritis in at least one joint. By 2030, 20 percent of Americans, about 70 million people, will have passed their 65th birthday and will be at risk for OA. Help promote health Joining the OAI health study now could improve the lives of your grandchildren and millions of Americans later.

    The study, published in the current issue of Arthritis and Rheumatism , found that osteoarthritis causes changes in the levels of two markers of collagen production and metabolism. Collagen is a component of cartilage. "What this study clearly shows is that there are biological markers for cartilage synthesis and degradation, and that they are present in osteoarthritis," says Dr. John Klippel, medical director of the Arthritis Foundation. Klippel was not involved in the current research. Osteoarthritis is a disorder caused by the destruction of the cartilage that cushions the joints. As cartilage wears away, bones begin to rub together. The condition can cause pain, swelling, stiffness, and loss of motion. More than 20 million Americans suffer from osteoarthritis, according to the National Institute of Arthritis and Musculoskeletal and Skin Diseases. Half of all Americans have osteoarthritis in at least one joint by the age of 65. There is no treatment to halt the progression of osteoarthritis, only to manage its symptoms.

    Osteoarthritis (OA) , also known as degenerative joint disease (DJD), is a process of progressive deterioration of articular cartilage and formation of new bone (osteophytes) at the joint surface. Primary osteoarthritis is age related and associated with repetitive and/or high mechanical stress on a normal joint. Secondary osteoarthritis is due to an underlying cause such as trauma, inflammatory, metabolic, developmental, or connective tissue diseases. Distribution:
    Primary osteoarthritis of the hands typically targets the PIP, DIP, scaphotrapeziotrapezoid and the 1st carpometacarpal joints. The knee, hip, spinal, and 1st metatarsophalangeal joints are other areas which are commonly involved. Additional joints can be affected in cases of secondary osteoarthritis.
    Radiographic Findings: Affected joint spaces are narrowed with reactive subchondral sclerosis (eburnation). Other classic radiographic findings include osteophytes and subchondral cysts. Heberden's nodes at the DIP joints and Bouchard's nodes at the PIP joints of the hands are areas of osteophyte formation and soft tissue swelling associated with osteoarthritis. Intraarticular loose bodies can also sometimes be seen. Erosive osteoarthritis is an inflammatory arthritis primarily seen in peri- and post-menopausal women; the distribution pattern in the hands is the same as in primary osteoarthritis. However, this process is associated with osteoporotic changes and erosions at the center of the involved articular surfaces which create a "gull-wing" deformity.

    • Osteoarthritis is a common, chronic condition which requires an individualised management plan involving multiple kinds of treatment. Exercise programs and the Arthritis Self-Management Program reduce pain and disability associated with osteoarthritis. Paracetamol is the most appropriate first-line analgesic. Non-steroidal anti-inflammatory drugs may be used as second-line analgesia on an as-needed basis (including continuous use), but must be used with caution. Cyclo-oxygenase-2-specific inhibitors are used if there are risk factors for upper-gastrointestinal complications, but only after considering cardiovascular risk. Glucosamine sulfate is a safe and effective over-the-counter treatment.

    Definition: Osteoarthritis is a chronic disease causing deterioration of the joint cartilage (the softer parts of bones, which cushion their connections to each other) and the formation of new bone (bone spurs) at the margins of the joints.

    Causes, incidence, and risk factors: For most people, the cause of osteoarthritis is unknown, but metabolic, genetic, chemical, and mechanical factors play a role in its development. It is associated with the aging process and is the most common form of arthritis It may first appear without symptoms between 30 and 40 years of age and is present in almost everyone by the age of 70. Symptoms generally appear in middle age. Before the age of 55 it occurs equally in both sexes. However, after 55 the incidence is higher in women.
