Osteoarthritis causes progressive breakdown of cartilage until the bones, which were once separated by cartilage, rub against each other. This results in damage to the tissue and underlying bone, causing the painful joint symptoms of osteoarthritis. Osteoarthritis is the most common form of arthritis and is a major cause of pain and disability in older adults. It most often affects the

    About osteoarthritis
    What is it?
Osteoarthritis, or degenerative joint disease (DJD), is a form of arthritis characterized by the loss of joint smoothness and range of motion without major joint inflammation.
    Incidence and risk factors
Quick Quiz - Do you know the facts? Osteoarthritis involves low bone density. True or false? True False Osteoarthritis or degenerative joint disease (DJD) is the most common type of arthritis, affecting over 20 million people in the United States. DJD probably affects almost every person over age 60 to some degree, but symptoms are often mild. Some individuals have an inborn tendency to degenerative joint disease because they have changes in the structure of the important protein-building blocks of the articular cartilage which covers the surface of their joints. These seemingly small, but significant abnormalities predispose the joint to wear and degeneration. In other cases, joint injuries may contribute to the development of DJD.
    Facts and myths

By Dr David Delvin. Osteoarthritis is something that virtually all of us will eventually suffer from.
Indeed, by the age of 60 the vast majority of the population have some degree of osteoarthritis, somewhere in their bodies. Many people - especially younger folk - find that almost impossible to believe! Yes it's true.
You see, the problem is that as the years go by, the joints in your body gradually get damaged by wear and tear. That's osteoarthritis.
THAT WORD 'ARTHRITIS'
Now there's an awful lot of confusion about that word 'arthritis'. Many older people hear it applied to them - and think that it means that they're going to be crippled. That isn't true at all
So what does 'arthritis' mean?
Strictly speaking, it just means 'inflammation of a joint'. A simpler definition is 'damage to a joint (or joints)'.
WHAT ARE JOINTS?
A joint is the place where two bones meet - like, for instance the shoulder joint or the hip joint.
If you've ever looked at the inside of a joint of meat, and seen the place where two shiny, smooth surfaces of bone come together (often as a 'ball-and-socket') then you'll know what I mean. You may have noticed how well one bone moves on the other - simply because the joint surfaces are so nice and regular.

DENNIS Y. WEN, M.D.
A patient information handout on hyaluronic acid injections for knee osteoarthritis, written by the author of this article, is provided on page 572 S ymptomatic osteoarthritis of the knee joint is a common presenting problem in primary care and can be a frustrating problem for physicians and patients. Treatment options and their efficacies are often limited. Intra-articular injections of hyaluronic acid, known as viscosupplementation, are a recently available option. Osteoarthritis is characterized by a loss of articular cartilage, which has a highly limited capacity to heal itself. Along with these cartilage changes, a reduction in the elastic and viscous properties of the synovial fluid occurs. The molecular weight and concentration of the naturally occurring hyaluronic acid decreases. Theoretically, this loss of elastoviscosity decreases the lubrication and protection of the joint tissues and is one postulated mechanism of pain production in osteoarthritis. Pharmacologic treatment generally consists of analgesics and/or nonsteroidal anti-inflammatory drugs (NSAIDs). Physical therapy can be used, with exercises to maintain range of motion and strength. Intra-articular corticosteroid injections are often used for transient symptom relief. When conservative measures fail, surgical treatments limited to arthroscopic debridement, osteotomies to redistribute load and total joint replacements have been the only options until recently.

C urrent estimations suggest that 40 million Americans of all ages are affected by osteoarthritis and that 70 to 90 percent of Americans older than 75 years have at least one involved joint. Estimates of the prevalence of osteoarthritis based on radiographic evidence range from 30 to 90 percent. Men and women are equally affected, but symptoms occur earlier and appear to be more severe in women. Common synonyms for osteoarthritis include osteoarthrosis and degenerative joint disease. Osteoarthritis is not an inevitable consequence of aging. It is an acquired degenerative process that can be managed effectively by family physicians. Etiology The exact etiology of osteoarthritis is unknown. Multiple factors (e.g., heredity, trauma, and obesity) interact to cause this disorder. Current treatment for OA is relatively limited. Because there are currently no pharmacological agents capable of retarding or preventing disease, treatment is predominantly focused on relief of pain, and maintenance of quality of life and functional independence.
Non-steroidal Anti-inflammatory Agents
Analgesic Agents ) However, many patients eventually require NSAIDs or more potent analgesics to control pain.
Cyclooxygenase-2 (COX-2) inhibitors are a class of nonsteriodal anti-inflammatory agents (NSAIDs) that recently received Food and Drug Administration (FDA) approval. These specific COX-2 inhibitors appear to be as effective as current non-selective NSAIDs in treating the pain and inflammation of arthritis. Their theoretical advantage, however, is that they will cause significantly less toxicity than conventional NSAIDs, particularly in the GI tract. This theoretical enhanced safety profile will be a significant advantage in the management of all patients but especially those at high risk for peptic ulcers. Nonsteroidal anti-inflammatory drugs (NSAIDs) exert their anti-inflammatory effect primarily by inhibiting an enzyme called cyclooxygenase (COX), also known as prostaglandin (PG) synthase. COX catalyzes the conversion of the substrate molecule, arachidonic acid, to prostanoids.
Prostanoids consist of prostaglandins E, D and F

With the newly launched Osteoarthritis Initiative that may change. Electrical Stimulation Helps Knee OA A study of the use of home-based neuromuscular electrical stimulation (NMES) for the purpose of improving quadricep strength in older adults with knee osteoarthritis has produced promising results. The report in the July 2003 Journal of Rheumatology points to NMES as a alternative when medications, exercise, and surgery are not options. Atkins and Arthritis The Atkins Diet, the Zone, and the South Beach Diet are all low-carb diets which have recently generated alot of publicity, not all of which has been favorable.
