Preventing and Controlling Oral and Pharyngeal Cancer Recommendations from a National Strategic Planning Conference
Summary In August 1996, CDC convened a national conference to develop strategies for preventing and controlling oral and pharyngeal cancer in the United States. The conference, which was cosponsored by the National Institute of Dental Research of the National Institutes of Health and the American Dental Association, included 125 experts in oral and pharyngeal cancer prevention, treatment, and research; both the private and public sectors were represented. Participants at the conference developed recommendations concerning advocacy, collaboration, and coalition building; public health policy; public education; professional education and practice; and data collection, evaluation, and research. CDC will use these recommended strategies to develop programs to reduce the burden of oral and pharyngeal cancer in the United States. Through the Oral Cancer Roundtable, a group of conference and meeting participants, CDC will communicate to interested agencies, organizations, and state health departments ways in which they can implement elements of the national plan. The Roundtable will help CDC track the efforts and progress of these groups. INTRODUCTION During the past decade, federal health agencies have focused on reducing the incidence of oral and pharyngeal cancer and increasing the 5-year survival rate from these cancers in the United States. Beginning with a consortium of health agencies in 1992 (and including a strategic planning conference in 1996 and a follow-up meeting in 1997), CDC has been involved in concerted efforts to establish a national plan for preventing and controlling these cancers. This report presents recommended strategies for action from the 1996 conference and a list of priority recommendations from the 1997 meeting. These recommendations will enable CDC to develop a coordinated national plan to reduce morbidity and mortality from oral and pharyngeal cancer in the United States.

Overview Oral Cancer Campaign In June 2003, the ADA launched a campaign urging dentists to examine patients for signs of early oral cancer. Approximately 30,000 new cases of oral cancer are diagnosed each year in the United States. According to the American Cancer Society, oral cancer occurs almost as frequently as leukemia and claims almost as many lives as melanoma cancer. Routine, careful examination of patients is appropriate and necessary. This can easily be achieved during a regular dental visit. The stage at which an oral cancer is diagnosed is critical to the course of the disease. When detected at its earliest stage, oral cancer is more easily treated and cured. When detected late, the overall five-year survival rate is about 50%. The campaign was featured at the ADA's Annual Session in San Francisco in October 2003, and ran through January 2004. The ADA worked with state and local dental societies to promote education and awareness among its members. The effort featured a series of activities targeted to dentists, dental schools, patients and consumers. CDx Laboratories, providers of OralCDx, a computer-assisted, painless brush biopsy oral cancer detection test supported the campaign through an unrestricted educational grant.

Academic Dental Centers, Hospitals, and Dental Societies Offer Free Testing on April 15, 2004 (University at Buffalo, April 17th.) On April 15, NY, NJ, and PA residents can find out if they have any oral lumps or sores by attending a free oral cancer screening sponsored by the Oral Cancer Consortium. When the disease is detected early, the survival rate is 80 percent compared with 17 percent when found in the later stages. Those who participate in the free screening will receive a comprehensive oral cancer examination administered by a dental professional. If in the examination, a suspicious looking red or white spot is found, the dental professional may administer a new, painless brush biopsy test to determine if there are any potentially pre-cancerous or cancerous cells present or suggest another appropriate treatment option. According to the Centers for Disease Control and Prevention, early detection is key to increasing the survival rate for oral cancer and plays a significant role in patients' success in fighting the disease. However, only 15 percent of the population reports ever having an oral cancer examination and a mere seven percent reports receiving annual screenings. "Early detection plays an important role in the success of treating many cancers," said Dr. Debra Cinotti, former Chair of the Oral Cancer Consortium and Associate Dean for Clinical Affairs at Stonybrook School of Dental Medicine. "It isn't any different with oral cancer. Our goal is to increase awareness of oral cancer and encourage people not only to become more aware of the abnormal spots in their mouths, but also to discuss oral cancer with their dentists. Since testing is now painless, it is simpler than ever to know for sure."

This, however, translates to 30,000 new cases every year in the United States. The single greatest risk factor is tobacco. While cases of oral cancers are seen in patients who do not use tobacco, these constitute a very small percentage of all oral cancers. All forms of tobacco have been implicated as causative agents including cigarette, cigar, and pipe tobacco as well as chewing tobacco. In India and Sri Lanka, where chewing tobacco is used with betel nuts and reverse smoking (placing the lit end in the mouth) is practiced, there is a striking incidence of oral cancer- these cases account for as many as 50% of all cancers! Heavy alcohol usage is an additional causative factor. Lip cancer, while included in statistics for oral cancer, is more similar to skin cancers. Sun exposure is the primary cause of these, while pipe smoking is also a factor. Cancers of the mouth present in various forms. Any persistent white patch must be regarded as being suspicious. Additionally, velvety red patches- particularly those with white speckles- should be areas of concern. Finally, any non-healing ulcer (erosion) merits evaluation. More often than not, these areas are painless.

Oral cancer, the sixth most common cancer, accounts for about 3.6 percent of all cancers diagnosed, with roughly 40,000 new cases of oral cancer reported annually in the United States. The vast majority of oral cancers occur in people older than 45 years, with men being twice as likely as women to develop the disease. The most frequent oral cancer sites are the tongue, the floor of the mouth, soft palate tissues in back of the tongue, lips, and gums. If not diagnosed and treated in its early stages, oral cancer can spread, leading to chronic pain, loss of function, irreparable facial and oral disfigurement following surgery, and even death. Your general dentist can perform a thorough screening for oral cancer. What causes oral cancer?
Scientists aren't sure of the exact cause of oral cancer. However, the carcinogens in tobacco products, alcohol and certain foods, as well as excessive exposure to the sun have been found to increase the risk of developing oral cancer. Risk factors for oral cancer may also be genetically inherited. What are the warning signs to watch out for?
