INTRODUCTION Background: The amniotic fluid that bathes the fetus is necessary for its proper growth and development. It cushions the fetus from physical trauma, permits fetal lung growth, and provides a barrier against infection. Normal amniotic fluid levels vary; however, the average volume increases with gestational age, peaking at 800-1000 mL, which coincides with 36-37 weeks’ gestation. An abnormally high level of amniotic fluid, polyhydramnios, alerts the clinician to possible fetal anomalies. Inadequate levels of amniotic fluid, oligohydramnios, results in poor development of the lung tissue and can lead to fetal death.
In pregnancies affected by polyhydramnios, approximately 20% of the neonates are born with a congenital anomaly of some type; therefore, the delivery of these newborns in a tertiary care setting is preferred. This article presents the causes, outcomes, and treatments of polyhydramnios and oligohydramnios, as well as their effects on the developing fetus and neonate.
Pathophysiology: Rupture of the membranes is the most common cause of oligohydramnios. However, because the amniotic fluid is primarily fetal urine in the latter half of the pregnancy, the absence of fetal urine production or a blockage in fetus’ the urinary tract also can result in oligohydramnios. Fetal swallowing, which occurs physiologically, reduces the amount of fluid, and an absence of swallowing or a blockage of the fetus’ gastrointestinal tract can lead to polyhydramnios.

What is amniotic fluid? Amniotic fluid is the liquid that surrounds your baby as he grows in the amniotic sac in your uterus during pregnancy. It cushions your baby against trauma (if you fall down, for example), keeps the umbilical cord from being compressed (which can reduce your baby's oxygen supply), and protects both your baby and your uterus from infection. Your baby will produce this fluid in his lungs and swallow it to help his breathing and digestive systems develop properly. Finally, amniotic fluid helps your baby develop muscle and bone by providing a zero gravity environment for him to move around in. Where does it come from?

I am at 19 weeks as of yesterday, and last Friday I had an ultrasound and was told that I had low amniotic fluid and that there were no sign of kidneys, or fluid in the belly or bladder. I was not happy to hear this. I have an appointment on 6/17 with a specialist, but they don't hold out much hope that there was a mistake, or that I will miraciously have a healthy baby. I decided not to abort, and to let nature take its course as long as my baby has a heartbeat. The scariest part is knowing that I have to go through labor and that it will not be a live birth. I already have a set of twins that are 4, but this is very hard for me. If anyone can shed some light or is going through the same thing, i would love to hear from you. my email is kfrisk23@hotmail.com. Polyhydramnios is a high level and oligohydramnios is low level of amniotic fluid. Description Amniotic fluid is the liquid that surrounds the developing fetus during pregnancy Causes and symptoms Polyhydramnios, also referred to as hydramnios, can have any one of a number of causes related either to an underlying maternal or fetal condition. Maternal diabetes, which is associated with a macrosomic (enlarged) fetus, is a common cause. The medication lithium, used to treat depression, can also increase amniotic fluid levels. Twin gestations are prone to polyhydramnios. Infections passed from mother to fetus such as rubella , cytomegalovirus, and toxoplasmosis , can also result in damage to the fetus and elevated amniotic fluid levels. Fetal abnormalities, including many that are life-threatening or lead to a significant impairment in the quality of life, are found in up to a quarter of all patients. For this reason, the initial finding of excess amniotic fluid should be followed by thorough diagnostic studies to determine the cause and the prognosis. Because fetal swallowing is a major factor in amniotic fluid removal, fetal abnormalities that prevent fluid uptake should be investigated. These include gastrointestinal obstructions such as

Measurement of the volume of amniotic fluid is extremely difficult and subjective assessment is still used by many. Measurement of the depth of pools of amniotic fluid gives a semiobjective method of assessment either in the form of amniotic fluid index or just as maximum pool depth. The commonest causes of oligohydramnios are premature rupture of membranes, intrauterine growth retardation renal tract abnormalities and major syndrome complexes. Fetal ultrasound usually demonstrates the renal tracts with some accuracy although oligohydramnios does prevent high quality demonstration of the kidneys. It will usually exclude obstructive renal disorder. Renal agenesis can be diagnosed but the level of certainty of the diagnosis is hampered by the lack of amniotic fluid.

This is an example of a deformation sequence in which an anomaly (here the lack of normal kidneys) leads to lack of fetal urine output with resultant oligohydramnios that causes deformation through constraint.

    There are four categories of amniotic fluid:
  • Oligohydramnios . Too little amniotic fluid.
  • Pockets seen greater than 1 cm in diameter (normal).
  • Adequate fluid, seen everywhere between the foetus and uterine wall (normal).
  • Polyhydramnios . Too much amniotic fluid. Measurement is commonly taken by using an ultrasound to determine the Amniotic Fluid Index (AFI). Oligohydramnios When a woman is said to have too little amniotic fluid she has oligohydramnios. This is defined as having less than 200 ml of amniotic fluid at term or an AFI of less than 5 cm. This means that during an ultrasound the largest pocket of fluid found did not measure 1 cm or greater at its largest diameter. It is clinically very hard to prove prior to delivery. After the birth, examining the placenta for the presence of amnion nodosum on the placenta is highly correlated with oligohydramnios. Depending on when the woman is diagnosed with oligohydramnios, there are different complications to look for, although the majority of women diagnosed will not have problems.
