What Is Obesity?
Being obese and being overweight are not exactly the same thing. An obese person has a large amount of extra body fat, not just a few extra pounds. People who are obese are very overweight and at risk for serious health problems. To determine if someone is obese, doctors and other health care professionals often use a measurement called body mass index (BMI). First, a doctor measures a person's height and weight. Then the doctor uses these numbers to calculate another number, the BMI. Once the doctor has calculated a child's or teen's BMI, he or she will plot this number on a specific chart to see how it compares to other people of the same age and gender. Weight Control and Diet December 2001 WHAT IS OBESITY? Stable weight depends on an even balance between energy intake from food and energy expenditure. Energy expenditure occurs during the day in three ways:
  • As energy expended during rest ( basal metabolism ). This accounts for about two-thirds of expended energy, which is generally used to maintain body functions, such as maintaining body temperature and muscle contractions in the heart and intestine.
  • As energy used to metabolize food ( thermogenesis ), accounting for about 10% of expended energy.
  • As energy expended during physical activity.
When a person's caloric intake exceeds his or her energy expenditure, the body stores the extra calories in the fat cells present in adipose tissue. These adipose cells function as energy reservoirs, and they enlarge or contract depending on how people use this energy. If people do not balance energy input and output by adopting healthy eating habits and regular exercise, then fat builds up, and they may become overweight.
Measurement of Obesity Obesity is determined by measurement of body fat, not merely body weight. People might be over the weight limit for normal standards, but if they are very muscular with low body fat, they are not obese. Others might be normal or underweight, but still have excessive body fat. Different measurements and factors are used to determine whether or not a person is overweight to the degree that it threatens health:

Over one-half of all Americans (about 97 million) are overweight or obese. If you are overweight or obese, carrying this extra weight puts you at risk for developing many diseases, especially heart disease, stroke, diabetes, and cancer. Losing this weight helps to prevent and control these diseases. Obesity is a condition of excess body fat. It is the most common form of malnutrition in the Western world. It is important to note that no "diet or weight loss formula" works independently of an eating plan based on the Dietary Guidelines given by the mainstream nutrition and health institutes. So if you are obese throw away all those "special formulas", supplements, and pills, stop eating more calories than you need, become more physically active and adopt a healthy eating plan. Severe overweight increases the risk for high blood cholesterol, high blood pressure, and diabetes and, hence, for diseases for which these conditions are risk factors (diabetes, coronary heart disease, high blood pressure, neurologic disorders, cancers, and kidney diseases). Obesity thus contributes to premature mortality. Of all obesity-related diseases, noninsulin dependent diabetes is most clearly and strongly associated with obesity. Obesity is associated with too many adipose cells (hyperplastic obesity), adipose cells that are too large (hypertrophic obesity), or both. While changes in the size of adipose cells may occur at any age, the number of adult cells are fixed and determined by weight gain during certain periods of childhood development; fat cell numbers are established by late adolescence and, once established, does not decline.

Between 5-25 percent of children and teenagers in the United States are obese (Dietz, 1983). As with adults, the prevalence of obesity in the young varies by ethnic group. It is estimated that 5-7 percent of White and Black children are obese, while 12 percent of Hispanic boys and 19 percent of Hispanic girls are obese (Office of Maternal and Child Health, 1989). Defining Obesity in Children and Adolescents
Obesity is defined as an excessive accumulation of body fat. Obesity is present when total body weight is more than 25 percent fat in boys and more than 32 percent fat in girls (Lohman, 1987). Although childhood obesity is often defined as a weight-for-height in excess of 120 percent of the ideal, skinfold measures are more accurate determinants of fatness (Dietz, 1983; Lohman, 1987). A trained technician may obtain skinfold measures relatively easily in either a school or clinical setting. The triceps alone, triceps and subscapular, triceps and calf, and calf alone have been used with children and adolescents. When the triceps and calf are used, a sum of skinfolds of 10-25mm is considered optimal for boys, and 16-30mm is optimal for girls (Lohman, 1987).
