Alternate Names : Gluten Intolerance, Gluten-Sensitive Enteropathy, Nontropical Sprue, Sprue Definition Celiac disease is an inherited, autoimmune disease. The lining of the small intestine is damaged in response to ingestion of gluten and other proteins found in wheat, barley, rye, possibly oats, and their derivatives. The intestines contain projections (called villi) that normally absorb nutrients. In undiagnosed or untreated celiac disease, these villi become flattened and the ability to absorb nutrients properly is altered. As a result, several other organ systems may also be affected. The disease can first develop at any point in life from infancy to late adulthood.

Celiac sprue is an inflammatory condition caused by intolerance to gluten, a substance found in wheat and other grains. The inability to digest and process this substances may lead to inflammation of the intestines, vitamin deficiencies due to lack of absorption of nutrients, and bowel abnormalities. Gluten may be found in many foods, especially processed foods and baked goods. Breads, cakes, desserts that use thickeners, alcoholic beverages (except wine), cereals and pastas may all contain gluten. Previous Image Next Image Jump to another image

Progressive multifocal leukoencephalopathy with 10-year survival in a patient with nontropical sprue. Report of a case with unusual light and electron microscopic features
JJ Kepes, SM Chou and LW Price Jr
A 46-year-old man with nontropical sprue had anemia and hypoproteinemia for several years, until his condition was diagnosed and treated with dietary measures. Within a year after the diagnosis, progressive multifocal leukoencephalopathy developed, and the patient had a slightly fluctuating chronic downhill course until he died 10 years later. It is postulated that this patient's immune deficiency was related to his malabsorption syndrome and hypoglobulinemia, and the course became unusually protracted (longest

    Definition
Celiac disease is an autoimmune disease affecting the digestive tract. In susceptible people, eating food with gluten damages little protrusions in the small intestine (called villi) that absorb nutrients. The condition affects absorption of all nutrients, not just those in foods with gluten, so untreated patients often become malnourished. Gluten is found in wheat, rye, barley, and oats.
    Causes
Doctors do not fully understand what causes celiac disease, but eating gluten seems to be involved. Most likely, patients whose immune systems are genetically predisposed develop the disease after exposure to gluten. There is some evidence that in such people, earlier exposure in infancy causes more severe disease than later exposure.
    Risk Factors
A risk factor is something that increases your chance of getting a disease or condition.

Alternative Names Sprue; Nontropical sprue; Gluten intolerance; Gluten-sensitive enteropathy Causes And Risk The exact cause of celiac disease is unknown. Once thought rare, recent research suggests that an estimated 1 of every 133 Americans has celiac disease. However, only a small fraction of people living with it have been diagnosed in the United States at this time. Those with a family member with celiac disease are at greater risk for developing the disease. The disorder is most common in Caucasians and those of European ancestry. Females are affected more commonly than males.

Treatment: A life-long gluten-free diet is required. This allows the intestinal villi to heal. Foods, beverages, and medications which contain wheat, barley, rye, and possibly oats are eliminated completely. You must read food and medication labels carefully to look for "hidden" sources of these grains and their derivatives. Since wheat and barley grains are found abundantly in the American diet, the treatment is challenging but achievable with education and planning. You should NOT begin the gluten-free diet before a diagnosis is made. Doing so will alter future testing for the disease. Vitamin and mineral supplements may be prescribed to correct nutritional deficiencies. Occasionally, corticosteroids (such as prednisone) may also be prescribed for short-term use or if you have refractory sprue. Following a well-balanced, gluten-free diet is generally the only treatment required for achieving wellness and eliminating symptoms.
