Excerpted from CSA Handbook, Ist Edition Lifeline, Winter 1995, Vol XIII, No 1, pp 1-2 Treatment for nontropical sprue includes correcting the nutritional malfunction of the small bowel and the deficiency states. Removal of gluten from the diet results in the histological appearance of the intestinal mucosa returning to the normal state. Those conditions represent the need for the clinical gluten-free diet for nontropical sprue (celiac sprue) patients. Gluten-Free Diet. A specific diet that omits the glutamine-bound fraction of protein (glutenin and gliadin) is the clinical treatment of choice for nontropical sprue. In this diet wheat, rye, barley and oats are excluded, since they all contain large amounts of this protein fraction. Buckwheat and millet are also excluded because of lack of reliable information about their glutamine content. The gluten-free diet means that all bakery products and packaged foods must be scrutinized before they are used. Labels must be read carefully because, besides being a basic ingredient, gliadin-containing grains may be added as a derivative when the food is processed or prepared. For example, hydrolyzed vegetable protein may be made from soy or corn, or it may be made from mixtures of wheat, corn and soy. Thus, the need to know the content of hydrolyzed vegetable protein (HVP) before ingestion by celiac patients.

A: Colorado Celiac Disease (also known as celiac sprue, nontropical sprue and gluten-sensitive enteropathy) is a genetically inherited autoimmune disorder. People with CD cannot tolerate a protein - found in wheat, rye, barley and oats - called gluten. When gluten is ingested by someone with CD, the person's antibodies attack the villi of the small intestine (tiny 'fingers' on the lining of the small intestine) and destroy them. Without these villi, the absorption of nutrients is severely compromised and, regardless of the mass quantities of food consumed, causes malnourishment. This in turn causes many other physical problems such as anemia, osteoporosis, certain gastrointestinal cancers, seizures, joint pain, miscarriages, malformed fetuses, infertility, memory loss, mood swings and (if it remains undiagnosed in children) stunted growth or failure to thrive in infants. CD affects as many as one in 133 people.

    Background: Sprue (ie, celiac disease [CD]) is a permanent intolerance to certain storage proteins found in some cereals (eg, wheat, rye, barley) that occurs in genetically susceptible individuals and is triggered by an immune-mediated mechanism. The classic clinical picture first was described by Samuel Gee in 1888. In the typical presentation of CD in the young child, a combination of a potbelly and thin buttocks are observed, with proximal arm and thigh muscle wasting as a result of nutrient malabsorption. About 1950, understanding of the disease improved when the Dutch pediatrician W.K. Dicke cleverly observed an association between wheat consumption and incidence of CD. During World War II, wheat was unavailable; thus, grain products were very scarce in Holland. People with sprue found a surprising improvement in their symptoms, which ended when wheat and other grains became available again. Since then, research has uncovered the pathophysiology of this disease, which has enabled medical understanding and treatment. In 1968, adult nontropical sprue and childhood CD finally were discovered to be the same affliction. Sprue can present at any stage in life; a sprue diagnosis is not unusual in people older than 60 years. Pathophysiology: Sprue primarily affects the mucosal layer, which is where an inflammatory state, caused by a cascade of immune events, is activated in predisposed individuals by the exposure to gliadins. The condition causes a deepening and hyperplasia of the crypts and a concomitant flattening of the villi (ie, fingerlike projections of the mucosa with the primary function of increasing its absorptive surface). These changes are more evident proximally and fade more distally. Recently, it has been well documented that the described typical changes may occur with a patchy distribution. When gluten is excluded from the diet, the diseased mucosa returns to normal. The process of normalization is a highly variable one and may occur as soon as 3-4 months or as late as 1-2 years.

    Definition:
    Celiac disease is an inherited, autoimmune disease. The lining of the small intestine is damaged in response to ingestion of gluten and other proteins found in wheat, barley, rye, possibly oats, and their derivatives. The intestines contain projections (called villi) that normally absorb nutrients. In undiagnosed or untreated celiac disease, these villi become flattened and the ability to absorb nutrients properly is altered. As a result, several other organ systems may also be affected. The disease can first develop at any point in life from infancy to late adulthood.
