    PATIENT HISTORY: This 3 month old white female was referred to Children's Hospital with pancytopenia (pretransfusion hemoglobin = 5 g/dL), failure to thrive, gastroesophageal reflux, and increased irritability. Medications included folic acid, cimetidine, and metoclopramide. The child was the 2600g (5 lb. 11 oz.) product of an uneventful full-term pregnancy, born to a 31-year old gravida 3, para 2 woman. Labor, delivery, and early postnatal course were uncomplicated. Physical examination at 3 months revealed a pale infant with height 53.5 cm ( <5th percentile), weight 4.1 kg ( < 5th percentile), and occipital-frontal circumference 41 cm (75th percentile). The child was afebrile and normotensive; however, she was tachycardic (heart rate, 172 bpm) and tachypneic (respirations, 60/min). No dysmorphic features were noted. The remainder of the physical and neurologic examinations were within normal limits. The patient's sibling had an uneventful postnatal period and is developing normally.

    White blood cells protect us from infection. Several kinds of white cells exist, and each has a special function. The two most most common types are
    Neutrophils , which surround and destroy bacteria in the body, and
    lymphocytes , which are the key part of the body’s immune system and defend against viruses.
    Neutropenia occurs when the number of neutrophils circulating in the blood is too low.
    Neutrophils are very important in defending the body against bacterial infections. Patients with too few neutrophils are more susceptible to infections. For the missing item, see the original print version of this publication. Munshi and Montgomery used information obtained through a MEDLINE search for their review. Management and evaluation of neutropenia should depend on the severity of the neutropenia and on the presence of symptoms suggesting infection. Patients with severe neutropenia with fever or signs of infection and those with evidence of malignant disease should be hospitalized. The cause of neutropenia should be determined, potential sources of infection evaluated and antibiotic therapy started. The diagnostic evaluation includes history of prior infection, a review of prior blood counts and exploration for evidence of infection or rheumatologic conditions. Patients with mild neutropenia and no evidence of infection may be monitored with blood cell counts for six to eight weeks. Neutropenia in children is likely to be benign and often resolves without intervention. Laboratory tests include microscopic examination of a peripheral blood smear, appropriate cultures and specific chemical, bacteriologic and viral studies, and serologic testing for collagen vascular disease. Bone marrow aspiration is needed when the cause of neutropenia is not clear, when the neutropenia does not resolve or if there is evidence of multiple cytopenias.

    Understanding - and Overcoming - Neutropenia Probably one of the most frustrating-and potentially dangerous-side effects from chemotherapy is the lowering of your blood counts. Chemotherapy works by targeting fast-growing cells in your body-a primary characteristic of cancer cells. However, there are other normal fast-growing cells that exist naturally in your body, including those in the bone marrow that are responsible for producing most of the body's new blood cells. Unfortunately, these can also be affected by chemotherapy. When the production of red blood cells is adversely affected by chemotherapy, and the total red blood cell count in your body drops, fatigue and anemia can result. When white cells are similarly affected, there is a much greater risk of infection. A condition called neutropenia occurs when you have a dangerously low total of white blood cells called neutrophils in your body.
    Colony-Stimulating Factors
    One of the primary treatments for building up blood counts, including neutrophils, is by giving injections of colony-stimulating factors or CSFs. Colony-stimulating factors (sometimes called hematopoietic growth factors) are hormone-like substances which are naturally produced by the body. They stimulate the bone marrow to produce more blood cells. In recent years, researchers have discovered ways to manufacture CSFs synthetically as medications for patients with low blood cell counts.
