Neuroleptic Malignant Syndrome Medline NLM definition : A potentially fatal syndrome associated primarily with the use of neuroleptic agents (see ANTIPSYCHOTIC AGENTS) which are in turn associated with dopaminergic receptor blockade (see RECEPTORS, DOPAMINE) in the BASAL GANGLIA and HYPOTHALAMUS, and sympathetic dysregulation. Clinical features include diffuse MUSCLE RIGIDITY; TREMOR; high FEVER; diaphoresis; labile blood pressure; cognitive dysfunction; and autonomic disturbances. Serum CPK level elevation and a leukocytosis may also be present. Neuroleptic Malignant Syndrome Background - incidence ~ 1% of patients on neuroleptics - also occurs in Parkinson’s disease patients with sudden decrease in dosage of dopaminergic agents - most commonly within 3-9 days of initiation of therapy but can occur at any time during treatment or with dose increase - thought to be an idiosyncratic drug reaction., NOT thought to be related to dose or duration of treatment - can occur with typical and atypical/newer antipsychotics, but greater risk with high potency dopamine antagonists, such as haloperidol Pathophysiology Two major theories: 1. Dopaminergic depletion or blockade in basal ganglia (tone) and hypothalamus (thermoregulation) 2. Primary skeletal muscle defect similar to malignant hyperthermia. Clinical features NO DEFINITE DIAGNOSTIC CRITERIA MAJOR = - the absence of these findings puts the diagnosis into question MINOR = tachycardia, abnormal BP (high or labile), tachypnea, altered LOC, diaphoresis, leukocytosis (10-40 with left shift) - classic triad = hyperthermia

Updated (2003) by Ronald Gurrera, M.D., Associate Chief of Psychiatry and Assistant Professor of Psychiatry, Harvard Medical School at the Brockton VAMC and Harvard South Shore Psychiatry Residency Training Program Click here for Strategies for patients with a past history of NMS Incidence and mortality. Neuroleptic Malignant Syndrome (NMS) occurs in approximately 0.1% of hospitalized patients treated with antipsychotic medications. When it was first recognized more than 40 years ago, the mortality rate was 10% or greater. Improved clinician education, leading to more prompt and effective clinical intervention, is credited with a dramatic reduction in the lethality of the disorder over time. Nonetheless, patients with acute NMS can be gravely ill, and fatalities still occur. Diagnosis. The syndrome is characterized by a tetrad of clinical signs: hyperthermia, increased muscle tone, labile autonomic hyperactivity, and altered mental status. Diagnostic criteria vary from one academic institution to another, but those provided by the appendix in DSM-IV are reasonable. The diagnosis of NMS is generally one of exclusion, as patients at risk for NMS often have one or more potentially treatable medical processes (e.g., infection) capable of producing clinical findings that can be confused with NMS. Clozapine-Associated Neuroleptic Malignant Syndrome

Clozapine has been associated with atypical presentation of neuroleptic malignant syndrome . We observed a patient who had atypical neuroleptic malignant syndrome related to an increased clozapine dose. A 35-year-old man with bipolar disorder and schizophrenia presented to a community hospital with a 3-day history of diarrhea, fever and hallucinations. Medications included valproate sodium and oral clozapine, 100 mg/d. The latter had been started 3 weeks before admission and the dose had been increased to 150 mg/d 2 days before presentation. They are dedicated to reducing the morbidity and mortality of MH by improving medical care related to the syndrome, and improving the scientific understanding and research related to MH and other kinds of heat-related syndromes. MHAUS publishes a newsletter, The Communicator , five times a year that is included in the membership fee. They also have available a medical packet and a patient packet that includes a booklet "Understanding Malignant Hyperthermia" , a brochure "Traveling with Malignant Hyperthermia" , plus many more. Call or write for order form. MHAUS has stickers with the hotline number to be placed on or near the phone, and encourages the use of Medic Alert bracelets or necklace.

"The symptoms of the serotonin syndrome are: euphoria, drowsiness, sustained rapid eye movement, overreaction of the reflexes, rapid muscle contraction and relaxation in the ankle causing abnormal movements of the foot, clumsiness, restlessness, feeling drunk and dizzy, muscle contraction and relaxation in the jaw, sweating, intoxication, muscle twitching, rigidity, high body temperature, mental status changes were frequent (including confusion and hypomania - a "happy drunk" state), shivering, diarrhea, loss of consciousness and death.
