most common form of childhood kidney cancer Risk factors Symptoms Diagnosis In addition, blood and urine tests may be performed. If more investigation is needed, your child will undergo an ultrasound and perhaps a CT scan. Additional X-rays, ultrasound or magnetic resonance imaging (MRI) and bone scans may also be used. Diagnosing cancer All of these tests can help determine if cancer is present, what type of cancer it is, and, in some cases, if it has spread. Treatment Before cancer is treated, it has to be staged. Staging involves finding out where the cancer is located, the extent of the cancer and whether it has spread. Early stage cancers (Stage 1 and 2 that have not spread) are the easiest to treat. Treatment usually includes surgery to remove the tumor and all or part of the kidney, in addition to other nearby tissues. (The body can function normally with only one kidney.) If the cancer is confined to one kidney, it is called Stage 1. Cancers that have spread near the kidney are called Stage 2, while cancer that has spread regionally to local lymph nodes is called Stage 3. Stage 4 indicates cancer has spread to other organs.

Wilms' tumor
Overview Symptoms Treatment Prevention Definition:
Wilms' tumor is a cancerous tumor of the kidney that occurs in children.
Alternative Names: Nephroblastoma; Kidney tumor
Causes, incidence, and risk factors: Wilms' tumor is one of the most common tumors of the abdomen in children and the most common type of kidney tumor. The exact cause of tumor formation in most children is unknown. It is associated with certain birth defects including urinary tract abnormalities, absence of the iris (aniridia), and hemihypertrophy (enlargement of one side of the body). It is more common among some siblings and twins, which suggests a possible genetic cause. The tumor may become quite large, but usually remains encapsulated (self-enclosed). It may spread to other body tissues, especially the lungs.
The frequency of disease occurrence is estimated to be about 1 out of 200,000 to 250,000 children. The peak time of occurrence is at 3 years old, and Wilms' tumor is rare after the age of 8 years.

  • Wilms tumours are either sporadic or familial (1-2%); it may be associated with hemihypertrophia or genitourinary malformations (10%) and part of a recognized syndrome (2%).
  • the syndromes predisposing to Wilms tumours are:
    WAGR
    (Wilms tumour, aniridia, genitourinary abnormalities and mental retardation),
    DDS (Denys-Drash syndrome: mesangial sclerosis, male pseudohermaphrodism and Wilms tumours),
    BWS
    (Beckwith-Weideman: exomphalos, macroglossia, gigantism) and
    SGBS
    (Simpson Golabi Behemel syndrome: overgrowth, mental impairment, craniofacial anomalies). Epidemiology the most common paediatric cancer of the kidney, affecting 10/10 children; 50% of cases occurs before the age of 3 yrs and 90% before 6 yrs. Clinics the localization is primarily the kidney, the incidence of bilateral involvement is 5- 10% blastematous tissue with some differentiated glomerular structures associated with mesenchymal tissue and tubules. Courtesy Pierre Bedossa Pathology Wilms tumours show a mimicry of nephrogenesis as the tumour comprises undifferentiated blastemal cells, differentiated epithelial cells and stromal cells; ectopic components, particularly skeletal muscle, are observed in 5-10% of tumours; the presence of identical deletions of WT1 in all components of some sporadic Wilms tumours suggests that the stromal components are neoplastic, raising the possibility that undifferentiated blastema cells are precursors of the stromal and heterologous elements.
