NARCOLEPSY Complaint of sleepiness or sudden muscle weakness Recurrent daytime naps or lapses into sleep that occur almost daily Overview Male = female Prevalence: about 1 in 4000 in North America and Europe (Hublin et al 1994) Onset: 20 to 40 years old, rarely before age 5 or after age 60. Polysomnography: short sleep latency, short REM sleep latency. Clinical symptoms Excessive sleepiness: Most common initial symptom Sleepiness is similar to those caused by sleep deprivation: most apparent in boring, sedentary situations and is partially relieved by movement. No amount of nighttime or daytime sleep produces full alertness. Cataplexy: Episodic weakness without altered consciousness lasting seconds to minutes. Precipitated by excitement or emotion: may occur several times daily or less than once per month. Laughter is the most common precipitant. Anger, embarrassment, excitement, and other forms of emotion also can induce it. Severe attacks produce complete paralysis of striated muscles, but respiratory muscles are spared.

What is narcolepsy?
What you've seen in the movies is partly true narcoleptics do suddenly fall asleep, but most are treated with medication so that this doesn't happen. Symptoms of Narcolepsy
  • Excessive daytime sleepiness - the overwhelming urge to fall asleep during the day, even after having plenty of sleep the night before
  • Cataplexy - a sudden, usually brief attack of general muscle weakness related to a strong emotional response (fear, anger, laughter)
  • Hypnagogic sleep paralysis - brief episodes of paralysis that occur when falling asleep
  • Hypnopompic sleep paralysis - brief episodes of paralysis that occur when waking up
  • Hypnagogic hallucinations - vivid, usually visual or auditory hallucinations that occur at the onset of sleep (sometime between falling asleep and the actual sleep state)

What is narcolepsy?
Did you know? About one in 2,000 people suffers from narcolepsy. The majority of narcoleptics first experience symptoms between the ages of 15 and 30. For those who do not understand narcolepsy, it may seem funny to think of someone unable to control himself or herself from suddenly falling asleep. Narcolepsy is a very serious condition, though, and one that has several serious repercussions. For the person with narcolepsy, waking hours can be danger zones depending on the task at hand. Most activities, including driving, working, or even cooking, might be dangerous if the person doing them falls asleep unexpectedly. Narcolepsy is a chronic neurological disorder that is the second leading cause of excessive daytime sleepiness after obstructive sleep apnea. People with narcolepsy experience daytime sleepiness and intermittent, uncontrollable episodes of falling asleep during the day.

Here's a little quiz. One or more statements may be correct. People with narcolepsy often: a) fall asleep during the day at inappropriate times b) have episodes of muscle weakness brought on by emotions such as laughing c) have nightmares just when they start to fall asleep. d) have feelings of paralysis during sleep Most people associate narcolepsy with "sleep attacks" or "falling asleep at inappropriate times." But that is just one aspect of narcolepsy. There can be associated nightmares at the start of sleep and episodes of Sleep paralysis . Even more common are sudden episodes of muscle weakness while awake, called 'cataplexy', which may herald a sleep attack. The answers are all correct. Conversely, just because someone falls asleep at inappropriate times (answer a) doesn't mean he or she has narcolepsy. The most common causes of excessive daytime sleepines, in fact, do not include narcolepsy. Far more common are simple sleep deprivation and obstructive sleep apnea

WHAT IS NARCOLEPSY?
The word narcolepsy comes from two Greek words that can be roughly translated as seized by numbness. The two primary symptoms in narcolepsy reflect this phrase:
  • Excessive daytime sleepiness, with frequent daily sleep attacks or a need to take several naps during the day.
    Temporary and sudden muscle weakness (called cataplexy ), usually brought on by strong emotions.
Some, but not all patients experience other symptoms:
  • Microsleep episodes, in which the patient behaves automatically but without conscious awareness.
    A sense of paralysis that occurs between wakefulness and sleep (called atonia
    Dream-like states between waking and sleeping (called hypnagogic hallucinations
    Periodic leg movements during sleep ( periodic limb movement disorder
REM (rapid eye movement) sleep is abnormal in narcolepsy. In fact, narcolepsy is sometimes defined as the loss of boundaries between wakefulness, non-REM sleep, and REM sleep. [For a full definition of REM and nonREM sleep, see box Healthy Sleep.]

Narcolepsy is a chronic sleep disorder with no known cause. The main characteristic of narcolepsy is excessive and overwhelming daytime sleepiness, even after adequate nighttime sleep. A person with narcolepsy is likely to become drowsy or to fall asleep, often at inappropriate times and places. Daytime sleep attacks may occur with or without warning and may be irresistible. These attacks can occur repeatedly in a single day. Drowsiness may persist for prolonged periods of time. In addition, nighttime sleep may be fragmented with frequent wakenings. 4 pages. Narcolepsy is a serious medical disorder and a key to understanding other sleep disorders Narcolepsy is a disabling illness affecting more than 1 in 2,000 Americans. Most individuals with the disorder are not diagnosed and are thus not treated. The disease is principally characterized by a permanent and overwhelming feeling of sleepiness and fatigue. Other symptoms involve abnormalities of dreaming sleep, such as dream-like hallucinations and finding oneself physically weak or paralyzed for a few seconds (see Symptoms). The Stanford University Sleep Clinic was the first medical clinic ever established to specialize in sleep disorders. It was founded in the early 1970s by Dr. William Dement to diagnose and treat narcoleptic patients. The Stanford Center for Narcolepsy was established in the 1980s and is now directed by Drs. Emmanuel Mignot and Seiji Nishino. The Center for Narcolepsy is part of the Department of Psychiatry and Behavioral Sciences and has published more than 100 articles on narcolepsy. It is the world leader in narcolepsy research. Several hundred patients with the disorder are currently treated at the Center or participate in various research protocols. Other research protocols are conducted in animals models of narcolespy. The Stanford Center for narcolepsy was the first to report that narcolepsy-cataplexy is caused by hypocretin (orexin) abnormalities in both animal models and humans (see Research). We are always looking for volunteers in our narcolepsy research studies.

ABSTRACT Sleep disorders in mammals, such as insomnia and narcolepsy, are treated by administering an effective amount of ethyl 4-acetoxybutanoate, or a closely related homolog thereof. The compounds may also be administered to mammals to cause muscle relaxation and as animal anaesthetics. Moreover the compounds appear to inhibit the release of dopamine in the brain, and are hence useful also in treatment of conditions which are associated with abnormalities of dopamine release and dopamine sensitivities, such as Parkinson's disease, schizophrenia and tardive dyskenesia.
