But recently, several discoveries culled from animal research indicate that molecular brain malfunctions may participate in the development of the ailment. The new insights are focusing the search for targeted human treatments for narcolepsy as well as other types of sleeping problems. Dramatically drowsy during calculus class? Maybe it's the monotone teacher, an overdose of David Letterman or, perhaps, narcolepsy. This brain disorder, which afflicts an estimated 200,000 Americans, is marked by an uncontrollable, overwhelming desire to sleep during the day. The attacks can occur at any time, even in the middle of a conversation about yesterday's homework.
Narcoleptics also may experience vivid, scary dreams and a temporary inability to move before nodding off or awakening. Bouts of sudden muscle weakness, termed cataplexy, also may erupt. A laughing fit or other experience tied to a strong emotion will set off the weakening, causing problems ranging from a slackened jaw to a complete body collapse where a narcoleptic appears knocked out.
Narcolepsy's myriad traits have made it extremely difficult to study. But several recent findings are rousing the field. Much of the new work links special chemicals to the disorder. The studies are leading to:

What is narcolepsy?
Narcolepsy is a chronic (long-lasting) neurological (affecting the brain or nerves) disorder that involves your body's central nervous system. The central nervous system is the "highway" of nerves that carries messages from your brain to other parts of your body. For people with narcolepsy, the messages about when to sleep and when to be awake sometimes hit roadblocks or detours and arrive in the wrong place at the wrong time. This is why someone who has narcolepsy, not managed by medications, may fall asleep while eating dinner or engaged in social activities - or at times when he or she wants to be awake. Recent discoveries indicate that people with narcolepsy lack a chemical in the brain called hypocretin, which normally stimulates arousal and helps regulate sleep. They also discovered that there is a reduction in the number of Hcrt cells or neurons that secrete hypocretin. This may be due to a degenerative process or an immune response. How this occurs is unknown. About one in 2,000 people suffers from narcolepsy. It affects both men and women of any age, but its symptoms are usually noticed after puberty begins. For the majority of persons with narcolepsy, their first symptoms appear between the ages of 15 and 30.

Symptoms of narcolepsy
The main characteristic of narcolepsy is overwhelming excessive daytime sleepiness (EDS), even after adequate nighttime sleep. A person with narcolepsy is likely to become drowsy or to fall asleep, often at inappropriate times and places. Daytime sleep attacks may occur with or without warning and may be irresistible. These attacks can occur repeatedly in a single day. Drowsiness may persist for prolonged periods of time. In addition, nighttime sleep may be fragmented with frequent wakenings. Three other classic symptoms of narcolepsy, which may not occur in all patients, are:
  • Cataplexy : sudden episodes of loss of muscle function, ranging from slight weakness (such as limpness at the neck or knees, sagging facial muscles, or inability to speak clearly) to complete body collapse. Attacks may be triggered by sudden emotional reactions such as laughter, anger, or fear and may last from a few seconds to several minutes. The person remains conscious throughout the episode. Sleep paralysis : temporary inability to talk or move when falling asleep or waking up. It may last a few seconds to minutes.

What is narcolepsy?
Narcolepsy is a chronic , or long-lasting, sleep disorder with no known cause. It affects the body's central nervous system, which is made up of nerves that carry messages from the brain to other parts of the body. When a person has narcolepsy, messages about when to sleep and when to be awake can get mixed up. This can cause a person to fall asleep when they do not want to, and often without any warning like feeling drowsy. The desire to sleep can be overwhelming and hard to resist, and can happen to a person several times during the day. Night sleep may also be poor, broken up by waking up often during the night. If not controlled with medication, narcolepsy can cause serious problems in a person's personal, social, and work life. It can also limit a person's activities, such as driving a car, work, and exercising. Studies indicate that narcolepsy may run in families.

Imagine listening to a funny joke, then in the middle of laughing, your legs suddenly buckle and you are unable to move for the next several minutes. Maybe you are in class, but you are incredibly drowsy and finally just can't stay awake even though you had plenty of sleep the night before. These are examples of what people with narcolepsy experience. People with narcolepsy have trouble staying awake. Narcoleptic often feel drowsy and fall asleep, even in the middle of the day, sometime in the middle of activities that produce strong emotions. It is thought that narcolepsy occurs when rapid eye movement (REM) sleep occurs while people are awake. Although the cause of narcolepsy is unknown, it does occasionally run in families. If one person in a family has the disorder, another relative may be susceptible to developing it if certain environmental triggers occur.
Symptoms
  • Excessive daytime sleepiness: people may fall asleep when they are doing a boring task or when they are working alone. These sleep attacks may become more frequent and occur at inappropriate and dangerous times such as when they are driving a car.
  • Cataplexy: sudden loss of muscle tone. Loss of muscle tone in the legs may cause a person to fall down. In people with normal sleep/wake functioning, loss of muscle tone is associated exclusively with REM sleep.

At Texas Regional Sleep Disorder Center, our approach to snoring and sleep apnea is comprehensive. Snoring is caused by an obstruction of air while one Is sleeping as it passes through the nose and to the back of the throat, or pharynx. Many times the snoring is not serious enough to cause any physical illness. Other times the snoring is a symptom of a very severe obstruction of breathing and the body has to work to breath to maintain normal bodily functions. If one has sleep apnea, breathing can stop for up to five minutes and during this time, when the breathing has stopped, the pulse increases and the blood pressure increases so as to give emergency needs of oxygen to the body. If this process Is allowed to progress then the individual having this is more prone to permanent high blood pressure, cardiovascular disease, leading to stroke and sudden death. Sleep apnea can affect people of all ages. Children get sleep apnea from enlarged tonsils and adenoids. This can happen in a newborn. This is usually manifested in children by fatigue, irritability, and of course, the audible snoring and breathing sensations. Breathing obstruction can be either in the nose or in the oral pharynx. In the nose, it is caused by cartilaginous or bony obstruction of the nasal septum which divides the nose into right and left compartments. It also can be caused by chronic sinusitis when one assumes the recumbent position, the infectious material rolls out of the paranasal sinuses and stops us the nose making one breathe through their mouth. After a period of time, this gasping for air through the mouth causes elongation of the soft palate and uvula, and later the tongue will press against the soft palate and uvula which has been elongated by this sleep apnea process, and breathing will stop.
