Narcissistic Personality Disorder (NPD)

What is Narcissism? A pattern of traits and behaviours which signify infatuation and obsession with one's self to the exclusion of all others and the egotistic and ruthless pursuit of one's gratification, dominance and ambition. Most narcissists (50-75%, according to the DSM IV-TR) are men. The Narcissistic Personality Disorder (NPD) is one of a "family" of personality disorders (known as "Cluster B"). Other members of Cluster B are Borderline PD, Antisocial PD and Histrionic PD. NPD is often diagnosed with other mental health disorders ("co-morbidity") - or with substance abuse and impulsive and reckless behaviors ("dual diagnosis"). NPD is new (1980) mental health category in the Diagnostic and Statistics Manual (DSM). There is only scant research regarding narcissism. But what there is has not demonstrated any ethnic, social, cultural, economic, genetic, or professional predilection to NPD. It is estimated that 0.7-1% of the general population suffer from NPD. Pathological narcissism was first described in detail by Freud. Other major contributors are: Klein, Horney, Kohut, Kernberg, Millon, Roningstam, Gunderson, Hare The onset of narcissism is in infancy, childhood and early adolescence

    The diagnostic criteria for narcissistic personality disorder is characterized as a pervasive pattern of grandiosity, need for admiration, and lack of empathy. These individuals display an exaggerated sense of self-importance, which is manifest in a pretentious, boastful overestimation of abilities Brief reactive psychoses may occur, which are experienced as a loss of the sense of continuity of oneself as worthwhile and lovable in the face of a current disappointment, criticism Depression is common, as is chronic intense envy. Defensive self-delusion or lying to oneself Differential Diagnosis Individuals with narcissistic personality disorder lack the impulsivity, self-destructiveness Epidemiology Prevalence estimates for narcissistic personality disorder are 1% in narcissistic personality disorder In normal psychological development, all very young children have an exaggerated sense of their own importance as well as an idealized view of parental figures as protective, powerful, and immortal Treatment Long-term psychoanalytic psychotherapy and psychoanalysis have been attempted with these patients Narcissistic Personality Disorder in Medical Practice Narcissistic patients try to sustain an image of perfection and personal invincibility for themselves Physical illness may shatter this illusion, and a patient may lose the feeling of safety inherent in a cohesive sense of self.     What is Narcissistic Personality Disorder? Narcissistic Personality Disorder is a persistent pattern of grandiosity, hypersensitivity to the evaluation of others, and lack of empathy that begins early in adulthood and is present in a variety of contexts. People with this disorder have at least five of the following symptoms: reacting to criticism with feelings of rage, shame or humiliation; taking advantage of others to achieve his or her own ends; a grandiose sense of self-importance; a belief that his or her problems are unique and can be understood only by other special people; preoccupation with fantasies of unlimited success, power, brilliance, beauty or ideal love; unreasonable expectations of especially favorable treatment; a need for constant attention and admiration; inability to recognize and experience how others feel; or preoccupation with feelings of envy.     • Learn ways to tolerate emotional distress. Distress usually rises to a peak quickly when you're hurt or upset, but if you tolerate the distress and do absolutely nothing about it, then the pain will gradually diminish. Practice making no other response to emotional distress than just to tolerate it.
    • Learn to fail. In other words, practice every day doing the things you find most difficult. Often the things we view as stupid are the things we find most difficult, so practice those things too. Learning that you can fail and survive is important.
    • Play games with people that are better than you, so you can learn how to lose without severe distress.
    • Identify the ways you put other people down. Ask other people to help you do this since it's often easier for other people to see your behavior clearly than for you to see your own. Once you know exactly how you put other people down, stop doing it.

      In response to your question about the kind of typical interaction between parent and child, let me give you an example; 4 year old son said I'm not feeling very well, father responded by saying "Don't worry, I'm feeling fine." In case it's not clear, the father didn't listen to what the child said, instead was focused on himself, and assumed the 4 year child was also concerned about how the father was feeling. It is typical of this father to not listen to the remarks and comments made by his children. Also when he does talk to them he usually refers to himself in the context of the conversation. I realize this may be a little vague. I will try to describe other incidents of narcissistic behavior as we continue to communicate if that's all right? This person is my husband, and we are in the middle of a very nasty divorce. To complicate matters, we are still living in the same house. We have three children, ages 6, 4, and 4 1/2 months. My husband has been seeing a therapist for over a year, and to my knowledge he has not been diagnosed with Narcissistic Personality disorder. It was actually my attorney, who was trained as a psychological social worker before she went into law, who suggested that my husband may have NPD. I am concerned because if a therapist doesn't diagnose him as such, it will not be considered as relevant to custody decisions. I know that the judge in our case takes the diagnosis of NPD very seriously but obviously he has to be diagnosed first. How can I help the children when he will almost certainly have custody of them for some percentage of the time?
