If we weren't so interested in ourselves, life would be so uninteresting that none of us would be able to endure it." (Schopenhauer) Paranoia occurs in two forms: (1) the "bad me" paranoid; and (2) the "poor me" paranoid. Paranoia affects .5 to 2.5% of the population. The "bad me" type tends to be more rageful and sadistic than the other type. Paranoia in all its forms tends to be organized around aggression, from sadomasochistic violence to lingering hostile mood. Paranoia is an insidious disease which develops slowly as a secondary personality characteristic, fuses into a more or less dysfunctional coping style, and may or may not become the dominant pattern. Psychologists suspect that the cause of paranoia is found in the mothering experience , in particular, the breast-feeding experience. Successfully breast-fed infants develop the capacity to feel supported and a tolerance for frustration. Unsuccessfully breast-fed infants (those who viewed the experience as "bad" in some way) develop a distinct inability to experience self-satisfaction, tolerance, and positive relationships. Internalization of the bad experience leads to the initiation of provocative and confirmatory interactions with others, mostly through splitting (seeing things as black-white, good-bad, weak-strong) and projection (accusing others of having the disowned aspects of your self). A full-blown "bad me" paranoid perceives threats in everything other people do, often exploding in manic, counterphobic episodes. A full-blown "poor me" type views the world as basically unfair and persecutory, countering their anticipation of discomfort with either antisocial behavior or grandiosity.

Needs to associate with high achievers to support image of himself. Requires excessive attention. Has a sense of being entitled to special treatment. Exploits other people and is insensitive and lacking in empathy. Jealous of others. Arrogant and disrespectful. Narcissism is a defined form of depression, and , as such, has more severe consequences ad is more likely to result in hospitalization. Another group of patients for whom hospitalization is indicated, provided long-term residential treatment is available, are those who have poor motivation for outpatient treatment, fragile object relationships, chronic destructive acting out, and chaotic life-styles. An inpatient program can offer an intensive milieu which includes individual psychotherapy, family involvement, and a specialized residential environment. The structure is physically and emotionally secure enough to sustain the patient with severe ego weakness throughout the course of expressive, conflict-solving psychotherapy. The possibility of a cure is limited and the goal should be to mitigate the worst aspects of the condition. Long term residential treatment in severe cases where motivation is clearly inadequate for outpatient treatment. This situation can improve the possibility of dealing with severe ego weakness through a course of conflict resolving therapy.

Definition Narcissistic personality disorder is a condition characterized by an inflated sense of self-importance and an extreme preoccupation with one's self. The cause of this disorder is unknown. Personality disorders are long-lasting patterns of behavior that cause problems with relationships and work. Narcissistic personality disorder usually begins by early adulthood and is marked by disregard for the feelings of others, grandiosity, obsessive self-interest, and the pursuit of primarily selfish goals.

People with narcissistic personality exhibit a persistent pattern of grandiosity, need for admiration and a lack of empathy. They have an exaggerated sense of self-importance, are absorbed by fantasies of unlimited success, and seek constant attention. Learn how to deal with Narcissistic Personality Disorder or NPD by learning the causes. There are two principal schools of thought regarding this issue.
Narcissistic Personality Disorder (NPD) is a severely disabling psychiatric condition, characterized primarily by intense self-love, delusions of grandeur, an inability to feel empathy for another human being and chronic pathological lying. Sufferers have trouble maintaining relationships, are hyper-reactive to criticism, often feel mistreated and are very susceptible to developing addictive behaviors. The question that this article will address is: what causes this disorder? NPD was first recognized in 1980 by the Diagnostic and Statistical Manual of Mental Disorders (American Psychiatric Association), and over the ensuing twenty years has proven very difficult to study. There are several reasons for this, but the principal reason is that narcissists tend not to seek help for their problems. The other major reason is that even when they do seek help (e.g. in the form of therapy) health professionals find that they are a very difficult population to deal with. bodyOffer(22227) The bottom line is that many people with NPD do not believe that they could possibly HAVE a problem in the first place.     The hospitalization of patients with severe Narcissistic Personality occurs frequently. For some, such as those who are quite impulsive or self-destructive, or who have poor reality-testing, this is the result of Axis I symptoms which are overlaid upon the personality disorder. Hospitalizations should be brief, and the treatment specific to the particular symptom involved. Another group of patients for whom hospitalization is indicated, provided long-term residential treatment is available, are those who have poor motivation for outpatient treatment, fragile object relationships, chronic destructive acting out, and chaotic life-styles. An inpatient program can offer an intensive milieu which includes individual psychotherapy, family involvement, and a specialized residential environment. The structure is physically and emotionally secure enough to sustain the patient with severe ego weakness throughout the course of expressive, conflict-solving psychotherapy. Small staff-patient groups within the wards, as well as large community meetings, at which feelings are shared and patients' comments taken seriously by staff, and constructive work assignments, recreational activities, and opportunities to sublimate painfully conflictual impulses make the hospital a "holding" environment rather than merely a containing one. The ultimate goals are of effecting a better integrated internal world, more cohesive and modulated self-object representation, and a self-concept less vulnerable to narcissistic injury.

How to Avoid the Wrath of the Narcissist
  • Never disagree with the narcissist or contradict him
  • Never offer him any intimacy
  • Look awed by whatever attribute matters to him (for instance: by his professional achievements or by his good looks, or by his success with women and so on)
  • Never remind him of life out there and if you do, connect it somehow to his sense of grandiosity
  • Do not make any comment, which might directly or indirectly impinge on his self-image, omnipotence, judgment, omniscience, skills, capabilities, professional record, or even omnipresence. Bad sentences start with: "I think you overlooked ... made a mistake here ... you don't know ... do you know ... you were not here yesterday so ... you cannot ... you should ... (perceived as rude imposition, narcissists react very badly to restrictions placed on their freedom) ... I (never mention the fact that you are a separate, independent entity, narcissists regard others as extensions of their selves, their internalization processes were screwed up and they did not differentiate properly) ..." NPD is one of a "family" of personality disorders (formerly known as "Cluster B").
Other members: Borderline PD, Antisocial PD and Histrionic PD.
NPD is often diagnosed with other mental health disorders ("co-morbidity") - or with substance abuse, or impulsive and reckless behaviours ("dual diagnosis").
NPD is new (1980) mental health category in the Diagnostic and Statistics Manual (DSM).
There is only scant research regarding narcissism. But what there is has not demonstrated any ethnic, social, cultural, economic, genetic, or professional predilection to NPD.
It is estimated that 0.7-1% of the general population suffer from NPD. Pathological narcissism was first described in detail by Freud. Other major contributors are: Klein, Horney, Kohut, Kernberg, Millon, Roningstam, Gunderson, Hare. The onset of narcissism is in infancy, childhood and early adolescence. It is commonly attributed to childhood abuse and trauma inflicted by parents, authority figures, or even peers.
