Your guest today is author Sam Vaknin, who has just published his new book, Malignant Self Love , about narcissistic personality disorder. He's joining us today from Macedonia, right at the edge of our conflict in the Balkans. At what point does narcissism go from something everyday like lingering a bit too long in the mirror to narcissistic personality disorder? Vaknin: Narcissism is a healthy thing. It has gradations. Sometimes, the reflection replaces the self, that's when we have a personality disorder (PD). But narcissists have no or very little personality they have only or mostly their reflection. That's why they need others, dependent on them for their very self-definition.

The narcissist is an actor in a monodrama, yet forced to remain behind the scenes. The scenes take center stage, instead. The narcissist does not cater at all to his own needs. Contrary to his reputation, the narcissist does not "love" himself in any true sense of this loaded word. He feeds off other people who hurl back at him an image that he projects to them. This is their sole function in his world: to reflect, to admire, to applaud, to detest - in a word, to assure him that he exists. Otherwise, they have no right to tax his time, energy, or emotions - so he feels. According to the legend of Narcissus, this Greek boy fell in love with his own reflection in a pond. Presumably, this amply sums up the nature of his namesakes: narcissists. The mythological Narcissus rejected the advances of the nymph Echo and was punished by Nemesis, consigned to pine away as he fell in love with his own reflection - exactly as Echo had pined away for him. How apt. Narcissists are punished by echoes and reflections of their problematic personalities up to this very day. How do I know so much about narcissism? I am a narcissist and, of course, I've done a lot of research on the subject.

Narcissistic disturbance can be found in everyone. An individual’s existence would be in jeopardy if some narcissistic traits were not present. These traits protect the ego from severe damage inflicted by others thus keeping our self-esteem in tact. Narcissistic Personality Disorder (NPD) occurs when narcissistic traits become exaggerated in an individual’s personality.
Psychodynamic theories for the development of NPD revolve around issues of unmet basic childhood needs. Narcissistic defenses are innately present during the first eighteen months of an infant's life. The infant is able to experience being the center of his mother's world and a oneness with the mother develops. This makes the child feel powerful and omnipotent. The child's world has no limits. One theory suggests that near the end of the eighteen-month period a psychological transformation causes the disintegration of the child’s oneness with the mother to occur. During the separation period if the mother was not responsive and sensitive to the child's needs during the first eighteen months then NPD will develop.
Other theories suggest that the development of the ego is governed by the limits and consequences the parents provide between two and ten and the amount of abuse or trauma the child receives during the first seven years of life. This abuse does not necessarily have to come from trauma induced by parents, but encompasses authority figures, peers, and others.

Narcissistic Personality Disorder Groups
The Gale Encylopedia of Psychology states that Havelock Ellis was the first psychologist to address narcissism in a published work in 1898. Freud wrote his paper "On Narcissism" in 1914. Wilhemin Reich defined the "phallic-narcissist" which is the fore runner of the diagnosis of NPD today. NPD appeared as a valid diagnosis for a mental disorder in the Diagnostic and Statistical Manual of the American Psychiatric Association (DSM-III) in 1980. Theodore Milton described five characteristics for diagnosis in the DSM-III: 1)inflated self image, 2)explorititive, 3)cognitive expansiveness, 4)insouciant temperment, and 5)deficient social conscious.
NPD developmental phases are very close to those described for borderline personality disorder. Perhaps this is because the original research on which the DSM-III adopted much of the criteria for narcissist personality disorder came from Kerberg’s work which was based on a population primarily diagnosed with borderline personality disorder.
