(Heterotopic Bone Formation) What is Myositis Ossificans? If you have a bad muscle strain or contusion (dead leg!) and it is neglected then you could be unlucky enough to get Myositis Ossificans. It is usually as a result of impact which causes damage to the sheath that surrounds a bone (periostium) as well as to the muscle. Bone will grow within the muscle (called calcification) which is painful. The bone will grow 2 to 4 weeks after the injury and be mature bone within 3 to 6 months. What causes myositis ossificans?
  • Not applying R.I.C.E. (rest, ice, compression and elevation) immediately after the injury. Having intensive physiotherapy or massage too soon after the injury. Use someone who is properly qualified and insured. Returning too soon to training after exercise.
Symptoms include:
  • Restricted range of movement.

Muscling in on myositis
This rare but unpleasant disease is explained by Ernest Choy, Barbara Mason and Nicolo Pipitone. Myositis is a relatively uncommon condition, and only about five new cases occur a year for every million people. Women are two to three times more likely to be affected than men. It can affect children as well as adults.
Symptoms
Muscles: The commonest symptom in patients with myositis is weakness in the upper arms, hips, and thighs. Because of the weakness in the upper arm, many patients find it to difficulty to comb and shampoo their hair, get dressed and reach for objects above their heads, while weakness in the thigh muscles makes climbing stairs and rising from a chair difficult. Often muscle weakness starts slowly, although in children with dermatomyositis, symptoms may develop more rapidly. Occasionally, muscles of the gullet (the canal through which food and drink pass from the mouth to the stomach) are also affected, causing difficulty in swallowing. When the muscles of the throat are involved, the voice can become hoarse. In severe cases, muscles of the chest and heart could also be affected and patients can become short of breath. Although less common than weakness, more than half of the patients also experience muscle pain.

Leo P Rando Sarah AL Cording and Harvey H Newnham MJA Clinical record A 43-year-old man presented with myalgia and upper respiratory tract symptoms. He had noted weight gain of 7 kg, lethargy and cold intolerance over the previous 12 months. He had no chest pain, rash or fever. Examination revealed that he was hypertensive (160/90 mmHg) and obese (body mass index, 32 kg/m ). He had marked corneal arcus, but no tendon xanthomas or xanthelasma, and no goitre. There was no muscle tenderness and power was normal. Reflexes were delayed. The results of some of the laboratory tests performed are shown below. In addition, normal findings were recorded for full blood count, erythrocyte sedimentation rate, and C-reactive protein, electrolyte, glucose, and calcium levels. Serological tests for hepatitis, and test results for protein electrophoresis, antinuclear factor, immunoglobulin and prostate-specific antigen levels, were all normal. There was no myoglobinuria, haematuria or proteinuria. Chest x-ray findings were normal, electrocardiography showed old Q waves, and an abdominal ultrasound showed hepatic steatosis. Apart from -glutamyltransferase, normal results were obtained for all other liver function tests.

Click on Images for Enlarged View Clinical History: A 76 year old woman presents with a several week history of a rapidly growing, painful, firm, fixed mass on the lateral aspect of the upper arm. Patient reports no known prior trauma. Findings: Plain radiographs illustrate a non-calcified, soft-tissue mass adjacent to the proximal humerus. MR demonstrates a mass located within the triceps muscle. T1 weighted axial MR images (a) demonstrates a mass with homogeneous signal intensity isointense with muscle, and T2 weighted axial images (b) of the mass demonstrate heterogeneous hyperintense signal. After gadolinium administration (c), the mass diffusely enhances. Edema is present in the surrounding muscle belly. Repeat plain films two weeks later demonstrate a circumscribed radiopaque lesion with a central lucent zone. Diagnosis: Myositis Ossificans. Discussion: Myositis Ossificans is a benign, ossifying soft-tissue lesion that primarily occurs within skeletal muscle. It is commonly associated with soft tissue trauma, although in almost one-third of the cases there is no history of injury. It is usually seen in the second and third decades, but has been documented in all age groups. Classification is based on imaging and histologic appearance, and is divided into early, intermediate, or late phase. It is important to be able to recognize myositis ossificans radiographically. Its plain radiograph and MR appearance can be difficult to distinguish from a soft tissue sarcoma or an inflammatory process, such as an abscess. This is especially true in the early stages. The enhanced rim can mimic an abscess or necrotic tumor, and adjacent soft tissue edema can suggest infection.

Myositis Myositis is a group of rare diseases that causes inflammation of the muscles and persistent muscle weaknesses. In these diseases, inflammatory cells surround, invade and destroy normal muscle fibers as if they were defective or foreign to the body. In rare cases, inflammation of the heart, lungs, intestines and skin can occur. Types of myositis include polymyositis, in which many muscles become inflamed, and dermatomyositis, in which the muscles and skin become inflamed. Symptoms All forms of myositis involve chronic muscle inflammation. This almost always results in muscle weakness, frequent falling, swollen feet and legs and muscular and joint pain. Early signs of the condition include:
  • Difficulty getting up from a chair Difficulty climbing stairs Difficulty lifting the arms Fatigue after standing or walking a long time Trouble swallowing or breathing
The muscle and skin inflammation associated with dermatomyositis can cause a distinctive, patchy rash on the eyelids, cheeks, nose, back or upper chest, elbows, knees and knuckles. Sometimes hardened bumps develop under the skin. The rash is often followed by muscle weakness. Causes and Risk Factors The cause of myositis is unknown. These diseases can affect people of any age. Most children who get the disease are between five and 15 years old, and most adults are 30 to 60 years old. Women are more likely to get the myositis than men.

Myositis is a rare and debilitating disease, that is an inflammation of the muscles. As it runs its course, muscle cells and fibers in different parts of the body are damaged and destroyed. The result is increasing fatigue, loss of muscle strength and the patient may have difficulty performing routine tasks, eventually basic physical activities are nearly impossible. Myositis usually involves a breakdown of muscle tissues, but it can also injure the heart muscle, the lungs, skin areas and the colon. In some cases, the muscles affected may be in only one arm or leg, or a specific muscle group. Patients that have chronic muscle pain and tenderness and fatigue, can take blood tests used to measure the amount of muscle tissue released. The tests include a creatine kinase (CK) or creatine phosphokinase (CPK). People with elevated CK or CPK levels will be evaluated for myositis or rhabdomyolysis. Other related muscle disorders or "inflammatory myopathies" are called dermatomyositis, polymyositis, and inclusion body myositis.
