Coronary artery disease occurs from atherosclerosis, when arteries become narrow or hardened due to cholesterol plaque build-up. Further narrowing may occur from thrombi (blood clots) that form on the surfaces of plaques. Myocardial infarction occurs when a coronary artery is so severely blocked that there is a significant reduction or break in the blood supply, causing damage or death to a portion of the myocardium (heart muscle). Depending on the extent of the heart muscle damage, the patient may experience significant disability or die as a result of myocardial infarction. In addition to atherosclerosis, myocardial infarction may result from a temporary contraction or spasm of a coronary artery. When this occurs, the artery narrows and the blood flow from the artery is significantly reduced or stopped. Though the cause of coronary artery spasm is still unknown, the condition can occur in both normal blood vessels and those partially blocked by plaques.

    Delayed diagnosis of acute myocardial infarction can have a devastating effect on patient care and physician credibility. According to the Physician Insurers Association of America (PIAA), since 1985, over $123 million has been paid on behalf of physicians nationwide on claims related to myocardial infarction. NORCAL has responded to this trend by developing this course for physicians. Our goal is to increase the timely diagnosis of myocardial infarction, decrease the number of myocardial infarction claims and, ultimately, save patient lives. Program needs were assessed through a review of the following sources:
  • The Physician Insurers Association of America (PIAA) Acute Myocardial Infarction Study, which revealed the following:
    • 40 to 45 percent of MIs go unrecognized and undiagnosed.
    • Delays in ordering tests, misinterpreting tests and failure to refer patients for appropriate care lead to missed diagnoses of MI.
    • Claims related to acute myocardial infarction are one of the most expensive for physicians and professional liability companies, with an average indemnity of $191,000 per claim.

    This booklet discusses heart attack, acute myocardial infarction (AMI, MI) and coronary occlusion which all refer to damage of the heart muscle. Most heart attacks are caused by a blood clot blocking an artery that has been severely narrowed by atherosclerosis. If medical attention is reached within the first few hours of an MI, clot-dissolving drugs (called thrombolytics) can be given or angio-plasty can be performed to restore blood flow to the heart and limit permanent heart damage. Drugs dissolve only the blood clot, not the deposits in the artery. Angioplasty pushes the deposits into the artery wall and sometimes a spring-like stent is used to hold them in place. Neither procedure actually removes the deposits. But, the sooner these treatments are given, the more heart muscle is saved from permanent damage. A heart attack may affect the entire thickness of heart muscle, or only the inner surface. The rest of the heart will go on working despite the injury, but will have to work harder. Soon after the injury to the muscle, healing begins. A scar is formed over the affected area during the next 6-8 weeks. This scar tissue can disrupt the electrical circuits of the heart and result in sudden heart attack from arrhythmia, or irregular heart beat. The first 6-8 weeks after your heart attack is a critical period in your follow-up care.

The major pathophysiological consequences of acute myocardial infarction are diminished systolic function due to loss of functioning myocardium. With extensive infarction stroke volume and cardiac output are reduced; this may result in cardiogenic shock. Another consequence of acute infarction is diastolic dysfunction resulting in a decrease in ventricular compliance with elevation in left ventricular diastolic and pulmonary venous pressure.     What is a myocardial infarction? Myocardial infarction (MI) means that part of the heart muscle suddenly loses it's blood supply. Without prompt treatment, this can lead to damage to the affected part of the heart. It is usually caused by a blood clot in a heart (coronary) artery which forms over a patch of atheroma (fatty deposit). Another leaflet called Myocardial Infarction discusses MI in more detail. What can I do to help after having a myocardial infarction? Following an MI, there are things that you can do to reduce the risk of further heart problems. Everyone is different and individual circumstances will vary. You should discuss with a doctor or nurse what is best for you. This leaflet aims to complement any advice that you may receive. Smoking
    If you smoke, giving up smoking is the single most effective way to reduce your risk of having a further MI. The chemicals in cigarette smoke affect the arteries. If you stop smoking, your risk of a further MI is roughly halved compared to the risk if you continue to smoke. Angina pains are also more likely to develop in smokers.
