    Cardiovascular diseases including acute myocardial infarction are the most common cause of death in Japan and their medical cost represents as much as 20% of total medical expense. Therefore, it is nationally beneficial to find risk factors for the prevention of myocardial infarction. To date, some environmental risk factors such as smoking or high-fat diet have been revealed, but very little is known of their genetic ones in Japanese population. Identification of genes related to myocardial infarction will surely enable us to identify high-risk group before onset. Through medical advice to improve their life style, as well as the drug development against molecules related to the disease, large beneficial effect in the prevention of myocardial infarction will be expected.
    Our ongoing project is to identify association between myocardial infarction and SNP. The following ones will be to investigate quantitative and/or functional changes in the gene product in which the SNP should play major role. To realize ' personalized medicine' is our goal through these studies.
    We are at present searching for genes related to myocardial infarction using genotype data of approximately 1,000 patients and 1,000 general individuals. So far, we have identified lymphotoxin-alpha gene (LTA) as one of the genetic risk factors for myocardial infarction. Previous studies (by others) reported lymphotoxin-alpha is related to the onset of vascular inflammation, and we have shown carrier of the variation in LTA has 1.8-fold risk of this disease, by affecting the degree of inflammation.

    Before the era of tests for the troponins, the diagnosis was based on three components; clinical signs and symptoms, ECG-changes and level of cardiac enzymes. As the evidence accumulated that a small release of troponins (and CK-MB) is important as a prognostic marker for the individual patients, this created the need for new diagnostic criteria.

    Despite improved clinical care, heightened public awareness, and widespread use of health innovations, coronary heart disease (CHD) remains the leading cause of death in the United States ( ), and the decline in rates from CHD that began during the 1960s slowed during the 1990s ( ). This report provides national and state-specific death rates for CHD and for acute myocardial infarction (AMI). During 2001, approximately 1.1 million persons are expected to have a CHD event ( ). Prevention remains the key strategy for reducing CHD mortality. National and state mortality statistics are based on information from death certificates filed in state vital statistics offices and are compiled by CDC's National Center for Health Statistics ( ). Demographics (e.g., age and race/ethnicity) listed on death certificates are reported by funeral directors or provided by family members of the decedent.
