Scottish Parliament Public Petitions Committee Tuesday 2 October 2001 Myalgic Encephalomyelitis (PE398) The Convener: Helen McDade: I have with me Linda McLean, the mother of a severely affected 15-year-old girl who has been ill for three years, and Alan Stroud, a 17-year-old sufferer of ME, who has been ill for three years. I hope that the committee will appreciate that Alan had to make a lot of effort to be here this morning. I am the mother of an 11-year-old girl who has been ill with ME for five and a half years. She has attended school for about 25 per cent of that period. When she became ill at five and a half, she wanted to be a ballet dancer and join the rainbow guides and the brownies. She wanted to go swimming and take up gymnastics and horse riding. She is now 11 and has not done any of those activities. Her childhood ended when she was six. Many children are like her and Linda McLean represents another such child. We are here today because ME sufferers and their families are desperate. Members of the committee are one of our last shots. It is estimated that 15,000 Scots in Scotland have ME. That is more than the number of people with multiple sclerosis. Of that number, about 2,000 are children and of those children, 25 per cent are severely affected, which means that they are housebound or bedridden. Unable to stand noise, they may live in a darkened, soundproof room because of hypersensitivity and they may not receive support from anyone except, hopefully, their families. Many people live with such suffering.

To be classified as ME or CFS, the following criteria must be fulfilled and be present for at least 6 months: A: Severe, debilitating and disabling fatigue, usually made worse by physical exercise - delayed onset of fatigue is characteristic of this illness B: Plus 4 or more of the following main symptoms:

  • Impaired Cognitive Functions
    • - inability to concentrate
    • - calculation difficulties
    • - memory disturbance
    • - spatial disorientation
    • - frequently saying the wrong word
    • - being in a 'fog'
  • Chronic sore throat - often with recurrent flu-like illness
  • Tender and swollen lymph nodes - especially neck and underarms
  • Muscle pain
  • Multi-joint pain
  • New headaches - often severe
  • Non-refreshing sleep
  • Post-Exertional Malaise
    Other symptoms may include:
    • Nervous system problems
      • Sleep disturbance - insomnia, hypersomnia - often with vivid dreams or nightmares
      • Severe muscular weakness without muscle wasting
      • Paralysis or Paresis - weakness or short lived paralysis
      • Numb or tingling feelings
      • Twitching muscles
      • Muscle spasms
      • Changes in visual acuity and photophobia-intolerance to light
      • Changes to hearing - sensitivity to noise
      • Alteration of taste and smell
      • Tinnitus - ringing in ears
      • Disequilibrium - severe dizziness
      • Feeling 'spaced out' or 'cloudy'
      • Blackouts
      • Depression
      • Anxiety
      • Emotional lability - mood swings, may be related to fatigue

    Chronic Fatigue Syndrome
    This condition is also referred to as ME (Myalgic Encephalomyelitis), Post Viral Syndrome, Fibromyalgia, or Royal Free Disease and a variety of opinions have been put forward suggesting possible co-factors that may be responsible for its development. These may include:
    • A stressful way of life that does not give a chance for relaxation and recovery to occur.
    • Dependence on stimulants in order to keep the pace. These may include coffee, recreational or prescription drugs, cigarettes or alcohol.
    • Reliance on 'junk foods' rather than eating a good quality, varied diet.
    • Recurrent bouts of thrush.
    • A history of recurrent infectious illness with repeated treatment by antibiotic drugs. This is suspected to be the case because the latter are regarded by alternative therapists to have a compromising effect on the efficient functioning of our immune systems.
    • Contracting a viral illness that is not given a chance to clear up, but keeps relapsing.
    • It has also been suggested that a sensitivity to vaccinations may also create a vulnerability to developing chronic fatigue syndrome, especially if other co-factors are present.

This was followed by an excellent monograph in 1969 by Luis Leon-Sotomayor published by Pageant Press International Corp., New York and entitled, "Epidemic Diencephalomyelits; A Possible Cause of Neuropsychiatric, Cardiovascular and Endocrine Disorders." Since then many colleagues from the U.K. and other countries have demonstrated the pathological systemic consequences which may occur after a viral illness and have long term effects. All this work has been pursued by many colleagues in the U.K. and abroad since then, amongst whom are 72 who contributed to the book of more recent origin and published by the Nightingale Research Foundation, entitled, The Clinical and Scientific Basis of Myalgic Encephalomyelitis.

Myalgic encephalomyelitis, or chronic fatigue syndrome (CFS), is not easily describable. As yet, there is no finite definition of the condition. There is also no specific test to diagnose it, so doctors rely on recognising the symptom pattern typical of M.E. and ruling out other conditions. Early diagnosis improves chances of recovery. M.E. may also be diagnosed as post-viral fatigue syndrome (PVFS). What causes M.E.? The exact cause of M.E is not known. The condition affects the function of the immune and nervous systems. Viral infections such as glandular fever and viral meningitis are known to trigger M.E. One in ten people with glandular fever develop M.E. symptoms. The symptoms of M.E.?
