Disorders of Muscle Tissue Muscle disorders are caused by disruption of normal innervation, degeneration and replacement of muscle cells, injury, lack of use or disease. Atrophy Muscular atrophy refers to a decrease in size of muscles. Individual muscle fibers decrease in size, and there is a progressive loss of myofibrils. Disuse atrophy is a muscular atrophy that results from lack of muscle use. Bedridden people, people with limbs in casts, or those who are inactive for other reason experience disuse atrophy in the muscles that are not used. Disuse atrophy is temporary if a muscle is exercised after it is taken out of a cast. However, extreme disuse of a muscle results in muscular atrophy in which there is a permanent loss of skeletal muscle fibers and the replacement of those fibers by connective tissue. Immobility that occurs in bedridden elderly people can lead to permanent and severe muscular atrophy. Denervation atrophy results when nerves that supply skeletal muscles are severed. When motor neurons innervating skeletal muscle fibers are severed, the result is flaccid paralysis. If the muscle is reinnervated, muscle function is restored, and atrophy is stopped. However, if skeletal muscle is permanently denervated, it atrophies and exhibits permanent flaccid paralysis. Eventually muscle fibers are replace by connective tissue, and the condition cannot be reversed. Transcutaneous stimulators are used to supply electrical stimuli to muscles that have had their nerves temporarily damaged or to muscles that are put in casts for a prolonged period to time. The electrical stimuli keep the muscles functioning and prevent permanent atrophy of the muscles while the nerves resupply the muscles or until the cast is removed.

No subject in dentistry is shrouded in more controversy than TMJ therapy. There are many "experts", but little agreement as to diagnosis and treatment. The TMJ is the joint upon which the jaw hinges when you open and close your mouth. You have one on each side of your head, and it is located about one inch in front of your ear. Muscle Disorders By far the most common TMJ problems are caused by muscle spasm, which in turn are caused by a lack of harmony in the muscles which move the joint on either side. Some muscle disorders cause only pain, while others can cause physical damage to the parts of the joint. One common cause of muscle disorders is a misalignment of the bite. Internal Derangements Internal derangements are when internal parts of the joint are misaligned or malformed. These problems can be a result of muscle disorders, trauma, or arthritis-related disorders. Tumors Thankfully rare, some TMJ problems can be caused by tumors in or around the joint. These are often difficult to diagnose, as different types of tumors show up differently in different types of radiographs. Facial Pain Not all facial pains are caused by TMJ type problems. Some other types of pain can be caused by neurological disorders and vascular disorders.

BS397 LECTURES 20-24 Neuromuscular Disorders The following topics are covered to give a feel for how the application of new methods has advanced our knowledge and understanding of muscle diseases, and to correlate pathological information with clinical data at the individual patient level. HISTORY Our understanding of neuromuscular disorders usually begins with clinical descriptions and attempts to classify the disease and then on to a description and understanding of its pathology (often helped by animal and experimental studies involving physiology, biochemistry and pharmacology). New breakthroughs in molecular genetics have now allowed us to accurately classify and describe the inheritance, aetiology and pathological mechanisms involved in many of these disorders and are bringing us to the verge of possible new treatments. Clinical descriptions of neuromuscular disorders in sporadic and familial groups exhibiting proximal or distal wasting and weakness and variable progression. Some attempts at relating pathology of nerve or muscle to the disease.
