We are pleased today to have one of our experts, Dr. Larry Kagen, who is a Professor of Medicine at the Weill Medical College of Cornell University and an attending physician at the NewYork-Presbyterian Hospital and the Hospital for Special Surgery, to talk about muscle disease. Dr. Kagen, what are the various types of muscle diseases that patients can present with? Lawrence J. Kagen, MD: Beyond the usual sprains and strains of everyday life and the difficulties that people have with their muscles in connection with overexertion and lack of training, in the summer time, the kinds of real illnesses that we see in the hospital are largely related to inflammatory muscle diseases occurring in connective tissue disorders. These are diseases like dermatomyositis, polymyositis, inclusion body myositis, or myositis in association with certain of the other connective tissue disorders, such as rheumatoid arthritis or scleroderma. A little goldmine I teach massage therapy here in Australia. Often, students will ask about books they should buy in order to become better therapists, and this is one that I always happily recommend. The techniques are well explained and relatively easy to apply. What's more they are safe, as well as beautifully effective. Anyone interested in improving their results with bodywork should have a look at this (and most of Chaitow's work, as far as I am concerned!). EXCELLENT!

Carnosine - Muscle ageing, muscle disorders
When man ages from 20 to 70 years of age, the lean body mass (muscles) decline by 20 %, and muscular strength and endurance decline likewise. The carnosine concentration and the antioxidant effect of carnosine decreases by half as age increases. This marked reduction in muscle carnosine concentration may be a cause of the age-related decline in muscle mass, strength and endurance.
Carnosine has a role in the following neuromuscular disorders, too:
  • ALS (amyotrophic lateral sclerosis) myastenia gravis (MG), polymyositis medicine-related muscle diseases (e.g., statins) late-onset-mitochondrial myopathy.
The role of carnosine in neuromuscular diseases has been investigated scientifically, and the results suggest supplementation with carnosine. While carnosine does not cure these grave disorders, it halts the oxidative stress and may increase the contractility of the muscles and add some strength and endurance.

Muscle Disorders
Muscle disorders (myopathies) encompass a wide range of problems in which muscle does not work normally for one reason or another. Symptoms of muscle disease can be remarkably varied, ranging from profound weakness and respir atory difficulties to mild syndromes in which individuals experience symptoms only when doing strenuous exercise. Muscle fatigue, poor relaxation of muscles, or exercise-induced weakness can also occur in some of these syndromes. In some conditions, other systems can also be affected such as the heart, lungs and intestine. Patients may seek evaluation and treatment for a possible muscle disease if they develop weakness or other muscle symptoms or if a laboratory value (called the creatine kinase (CK) level) is found to be abnormal on routine blood testing. Once a diagnosis is reached treatments vary considerably depending exactly on what the disorder is. For example, in certain metabolic disorders, dietary modification can be very helpful. In other disorders, certain medications can make a profound difference in terms of muscle strength. In some muscles diseases, no specific form of therapy is currently available, although further research may produce new future treatments. We are also currently conducting clinical research which may impact the long term care and treatment of patients with muscle disorders At Beth Israel Deaconess Medical Center, the Division of Neuromuscular Diseases is ready to assist you in helping to diagnosis and appropriately treat any and all muscle diseases, including: dermatomyositis, polymyositis, inclusion body myositis, muscular dystrophy, congenital myopathies, drug-induced muscle diseases, and mitochondrial muscle diseases. He can't run and within months, it's likely Johan wonÕt be able to walk at all - he has Duchenne muscular dystrophy, a progressive and debilitating muscular disorder. According to The Merck Manual of Medical Information, the term muscular dystrophy refers to a group of inherited muscle disorders that lead to muscle weakness of varying severity. It affects people of all ages; some forms may be apparent at birth or during infancy and childhood, others do not appear until middle age or later.
In most cases, the genetic abnormality for muscular dystrophy is inherited, so the condition tends to run in families. Some types of muscular dystrophy, including the most severe, Duchenne, are X-linked, which means that the abnormality is carried on the X chromosome that the mother contributes. A girl receives two X chromosomes, one from each parent, whereas a boy receives a Y chromosome from the father and an X from the mother. As a result, it is almost always boys who develop symptoms; girls inherit a normal X chromosome that may cancel out the abnormal one. These girls are called carriers of the muscular dystrophy gene, but have no muscle weakness themselves. Girls who carry the abnormal gene have a 50 per cent chance of passing the condition to their sons.

Common problems are aches and pain in the neck and shoulder region and disorders in the hands or the wrists. The European project PROCID aimed at providing a scientific base for prevention measures against such problems. A key element in medical and physiological studies of such issues is the analysis of electromyograms (EMG), i.e., the recordings of the electrical signals that are associated with all muscle activity. Ideally, the medical researcher would like to see the activation pattern of each individual motor unit (muscle fibre group). However, the measured electrical signal is the superposition of the signals from a large number of such of motor units, the 'signature' of which is not known a priori and may even change over time. STRABISMUS
Strabismus is a visual defect in which the eyes are misaligned. One eye may look straight ahead, while the other turns in or out, up or down. The misalignment may be present all of the time or it can come and go. And, the misaligned eye can even change. The exact reason for the misalignment of the eyes leading to strabismus is not fully understood.
Strabismus is a term that describes many eye muscle disorders.
Strabismus in Children
With normal vision, both eyes aim at the same spot. The brain then puts the two pictures into a single, three-dimensional image. This blending or fusing of the two pictures gives us depth perception. There are several types of strabismus.
  • Esotropia is an eye misalignment where the eye turns inward towards the nose. It can affect one or both eyes and can alternate between the two eyes.
