Heart Murmurs in Pediatric Patients: When Do You Refer?

A patient information handout on heart murmurs in children, written by the authors of this article, is provided on page 565. Many normal children have heart murmurs, but most children do not have heart disease. An appropriate history and a properly conducted physical examination can identify children at increased risk for significant heart disease. Pathologic causes of systolic murmurs include atrial and ventricular septal defects, pulmonary or aortic outflow tract abnormalities, and patent ductus arteriosus. An atrial septal defect is often confused with a functional murmur, but the conditions can usually be differentiated based on specific physical findings. Characteristics of pathologic murmurs include a sound level of grade 3 or louder, a diastolic murmur or an increase in intensity when the patient is standing. Most children with any of these findings should be referred to a pediatric cardiologist. (Am Fam Physician 1999;60:558-65.) P rimary care physicians frequently encounter children with heart murmurs.

What is a heart murmur? A heart murmur is just a noise that the blood makes as it flows through the heart. It's like the noise water makes when it flows through a hose. Many healthy people have heart murmurs. About 80 percent of all children have a heart murmur at some time. What is a "normal" heart murmur? Most heart murmurs don't mean there is anything wrong. Your doctor may call these murmurs "innocent" or "functional." "Normal" is another word to describe these murmurs. A normal murmur is just a noise caused by blood flowing through a normal heart. A normal murmur can get louder when the blood flows faster through the heart. For example, when the body's temperature goes up, the blood flows faster. Doctors often hear heart murmurs when they check children who have a fever. Many normal murmurs become hard to hear as children grow older. Some murmurs just go away.

What is a Heart Murmur
A heart murmur is a swishing or a whistling sound that the doctor hears when he listens to your child's heart with a stethoscope. The heart beat normally makes two sounds: the first is Lub and the second is Dub , these two sounds follow each other ( Lub Dub ) and are not separated by any extra sounds. A heart murmur will be heard as a swishing or a whistling sound in addition to the normal Lub-Dub sound. The moving blood sounds like the running water in your garden hose. A heart murmur may indicate that there is a heart problem or may be due to normal blood flow. A heart murmur is not a diagnosis or disease, it is a sign to alert our attention to check if there is anything wrong.
Most of the time heart murmurs are normal and do not indicate that there is anything wrong with the heart. However, sometimes they may result from a hole in the heart or a narrowed valve. A hole in the hose allows water to squirt out producing a whistling sound.

The table below describes 10 characteristics that can be described regarding each heart murmur. Most problems arise when trying to describe the shape: judge variation of intensity of the murmur; it may build-up(crescendo), rise and then fall (crescendo-decrescendo), decrease from a loud onset (decrescendo), or remain relatively constant (plateau).

INNOCENT MURMURS.
SUPRACLAVICULAR SYSTOLIC MURMUR.
This murmur begins after the first heart sound and is best heard underneath the right clavicle. The murmur generally is grade II but can be grade III and usually is of short duration, occupying less than one half of systole. Murmur frequency is low and is diamond shaped with a somewhat noisy quality. The murmur radiates towards the neck particularly on the right. Respiration has no effect. The murmur intensity decreases significantly when the child sits with the shoulders hyperextended by bringing the arms behind the patient. This murmur is caused by turbulence in the major brachiocephalic arteries as these vessels arise from the aorta. Differential diagnosis is seldom a problem; the murmur of aortic valve stenosis may come to mind, but can easily be distinguished by its specific characteristics as well as the presence of an aortic ejection sound.

Jaybird
Jaybird as his closest friends called him was introduced to me as my stepfather, however long before his passing he had simply become my dad. I am visiting my mom this weekend and we were going through some old papers and I came across the two pieces I wrote for his funeral. He passed away twenty-four years ago on June 8, 1987

What is an "innocent" or "functional" heart murmur?
Heart murmurs usually don't mean there is anything wrong with your child's heart. Your doctor may call these murmurs "innocent" or "functional." An innocent murmur is just a noise caused by blood flowing through a normal heart. These noises are commonly heard in children because their hearts are very close to their chest walls. An innocent murmur can get louder or softer depending on your child's heart rate, such as when they're excited or scared. Doctors often hear heart murmurs when they check children who have a fever. You react with fear and surprise and are quick to ask, "What does this mean and is my child in danger?" This can be a very frightening situation for new parents because they believe a heart murmur is a sign of a very serious condition. However, about 90 percent of normal children in the first five years of life are noted to have murmurs. A heart murmur is simply an extra sound that the heart makes. A normal beating heart makes two distinct sounds. The first sound, or the "lub", occurs when the valves between the upper (atria) and lower (ventricles) chambers close. These valves close after the ventricles fill with blood and begin to contract. The second sound, or the "dub", occurs when the valves between the ventricles and major arteries close. These valves close after the ventricles have finished pumping. A murmur is a sound made by turbulent blood flow and is heard between the other, normal heart sounds. Murmurs can be innocent, normal sounds or pathologic, abnormal sounds. I usually tell parents to think of murmurs as the sound of water flowing through pipes after the faucet is turned on. The sound may be a normal flow, a leaky faucet or a blocked faucet.

Heart murmurs: Which are innocent?
Many children have a heart murmur, especially during febrile illness. Most of these are "innocent", or "functional" murmurs, but some are caused by organic heart defects that are asymptomatic. When a heart murmur is detected during routine physical exam, a careful review of the child's appearance and history often provides clues about the murmur's significance. The classical innocent murmur is the vibratory systolic murmur referred to as Still's murmur. It is soft and of low frequency with a somewhat musical quality, like plucking a string or rubber band. This murmur is of short duration and occurs between the two heart sounds (mid-systole) and is often referred to as "physiologic", or "not significant." Other innocent murmurs are soft, quiet and "blowing" in character. Innocent murmurs often come and go. Also they may disappear if the child sits up or lies down. Once a murmur is discovered, (provided it's not a classical Still's murmur), observation of the child is important, along with noting vital signs. Is the color of mucous membranes and nailbeds pink or dusky (cyanotic)? Is exercise intolerance present? By history, have there been syncopal episodes or palpitations? All these findings suggest an organic murmur.

It does not necessarily mean there is an abnormality, just that it is detected. (Similar to hearing water going through a pipe – you hear it – and just because you hear it does not mean it is abnormal). The term "murmur" should not be confused with the term "abnormal murmur" just as the word "temperature" should not be confused with the term "fever". However, people mistakenly use both terms interchangeably. Murmurs are not always significant. – they may be normal or innocent . Innocent murmurs are usually found incidentally during a physical exam. They require no further evaluation. Pathologic murmurs are due to abnormal blood flow, usually as a result of a cardiac abnormality. These ought to be referred to a pediatric cardiologist (even prior to testing to avoid duplication and the improper performance of tests). How does it occur?
