Definition
A form of child abuse in which a parent induces real or apparent symptoms of a disease in a child. Causes and Risks The parent is very helpful in the hospital setting and is appreciated by the nursing staff for the care she gives her child. This behavior makes the child accessible so that the symptoms can be made to persist. Acute changes in physical exam, vital signs, etc. are never witnessed by hospital staff, but almost always occur in presence of the mother. Munchausen syndrome is not an intentional type of abuse. It occurs because of psychological problems in the adult, and is generally an attention-seeking behavior. However, the syndrome can be life-threatening for the child involved because this unusual behavior can escalate. Prevention Recognition of Munchausen syndrome in the child-parent relationship can prevent continued abuse and unnecessary, expensive and possibly dangerous medical testing.

In relating the details of Munchausen Syndrome by Proxy (MSBP), the initial reaction is usually shock, followed quickly by fascination. The reason for the latter is that the medical community has yet to make up their minds about what exactly MSBP is. The debate: psychiatric disorder v. child abuse. Essentially the arguments for both create a divide between the brain and behavior, though not relating the two. Munchausen Syndrome "is a condition manifest by persons feigning or inducing illness in themselves for no other apparent gain than adopting the sick role and thus exposing themselves to painful and sometimes damaging and disfiguring medical procedures ." The name of the syndrome originates from an eighteenth-century Baron, Hiernymous Karl Friedrich von Munchausen, who was a military mercenary widely known for regaling fantastical stories of exploits

Le SMP serait à l'origine de 8 à 20% des morts subites inexpliquées du nourrisson.
Découvert en 1977 par Sir Roy Meadow (St James University Hospital). Il est impossible de quantifier son importance réelle mais aux USA 1000 cas annuels sont répertoriés grace à l'autopise systématique des morts subites du nourrisson..Sir Ria Meadow estime avoir du faire face à 400 cas.
En France, il n'existe pas de consensus sur la réalité du SMP et bon nombre de pédiatres et de psychiatres ne peuvent imaginer des mères infligeant de telles souffrances à leur enfant et refusent d'admettre avoir été ainsi mystéfiés.
Il reste toutefois trés difficile de faire la différence entre un enfant qui s'est étouffé tout seul sous sa couette de celui qui est mort parce qu'on a appuyé un oreiller sur son visage.
La vidéo surveillance reste le moyen diagnostic le plus rapide mais il est contraire au droit français qui demande de prévenir les mères le l'existence du matériel de suveillance. Des réticences semblent retarder sont utilisation.
Il faut toutefois mettre en exergue que le but n'est pas de mettre en accusation une mère manifestement malade mais de sauver des enfants et il justifie totalement et sans arrière pensée l'utilisation de la vidéo-surveillance.

Q. Can you tell me more about Munchausen Syndrome by Proxy?
A. Munchausen Syndrome by Proxy (MSBP) is defined as the fabrication of an illness in a child by the child's parent (usually, but not always, the mother). There is controversy as to whether MSBP should be regarded as an "illness" or as a deliberate and serious form of child abuse [of course, these are not necessarily incompatible formulations]. In one study by Bools et al (Child Abuse Negl Sept. 1994), it was found that about 1/2 of the mothers involved in MSBP had either smothered or poisoned their child as part of the fabrication. 17 of the 47 mothers had a personality disorder of some kind. Some (see Lesaca, W V Med J Nov-Dec 1995) have hypothesized that the partent involved in MSBP is seeking a supportive relationship with a physician or other health care provider. In any case, the child suffers from the parent's actions and is subjected to great risks, owing to extensive medical procedures ordered by the unsuspecting physician.

Munchausen syndrome by proxy (MSBP) is a mental disorder in which a person causes or reports symptoms in another person under his or her care, usually a child. It is related to Munchausen syndrome, a rare mental disorder in which a person causes or reports symptoms in himself or herself for some reward, such as attention from health professionals. When a child is the victim of MSBP, the offending caregiver is most commonly the child's mother. Sometimes, the offending caregiver may actually do something to the child to cause the symptoms. In other cases, the caregiver may falsely report symptoms that do not actually occur. Munchausen syndrome by proxy involving children is a form of child abuse. This syndrome can have serious effects on a child. The child may have to go through needless and painful medical tests. He or she may become ill, be injured, or die as a result of the actions of the offending caregiver. Some life-threatening events and even cases of unexpected death in babies may be a result of MSBP.

Munchausen Syndrome by Proxy: The Ultimate Betrayal* By Dawn Doran Wilsey Munchausen Syndrome is defined as “a condition characterized by habitual presentation for hospital treatment of an apparent acute illness, the patient giving a plausible and dramatic history, all of which is false.” The DSM-IV-TR uses the term “factitious disorder” to define the “physical or psychological symptoms that are intentionally produced or feigned in order to assume the sick role.” The ultimate goal on the part of the patient is to seek attention. In 1977, an English pediatrician named Roy Meadow recognized the same pattern of behavior in which parents, usually mothers, would repeatedly present their children at hospitals complaining that their children were suffering from various illnesses. After running numerous tests and observing these patients over time, doctors discovered that the illnesses were non-existent or created by the parents. The offenders were using their children to achieve the desired result of attention for themselves. Thus, the term Munchausen Syndrome by Proxy was coined. It is defined as “occurring when a parent or guardian falsifies a child’s medical history or alters a child’s laboratory test or actually causes an illness or injury in a child in order to gain medical attention for the child which may result in innumerable harmful hospital procedures.” Since 1977, there have been over 300 articles published concerning the recognition, diagnosis, and treatment of this syndrome.

The Munchausen Syndrome by Proxy In 1951, Richard Asher used for the first time the term of “Munchausen syndrome” to describe a psychological disorder in which the patient fabricates or induces an illness to play the sick role. The choice of “Munchausen” was made after a German man, the Baron Von Munchausen who used to tell fake and exaggerated stories to everybody who would listen to him. About twenty years later, in 1977, Roy Meadow talked of another form of this disease : “Munchausen Syndrome By Proxy” (MSBP). This psychological disorder can be defined as following : “The intentional production or feigning signs and/or symptoms in another person who is under the individual’s care for the purpose of assuming the sick role by proxy”. The aim’s perpetrator is to be looked by others as a very devoted and brave person since he/she undergoes all these sufferings, along with “the need to be in a perverse relationship with a doctor or a hospital staff”. In more than 98 % of the cases, the perpetrator is a female and in about 95% of the cases, it’s the mother. The physician or the hospital staff become unknowing accomplices.
