What is multiple myeloma?
Multiple myeloma is a cancer of the body's plasma cells. Plasma cells are normally present in the bone marrow (making up less than 5% of the bone marrow) and are responsible for the production of antibodies when the body is dealing with infection. Cancerous plasma cells (myeloma cells) build up in the bone marrow, interfering with normal immune response and bone marrow production of normal blood cells. These myeloma cells invade and damage the bone, causing tumors (myelomas) to form. Myeloma cells then travel to other sites in the body, causing further tumors (hence the name "multiple myeloma"). Common features or symptoms of multiple myeloma are bone pain (especially with movement), anemia, fatigue, hypercalcemia, and increased incidence of infections (such as bladder or kidney infections, skin infections, pneumonia, or sinusitis).

Peer Review Status: Externally Peer Reviewed by the AMA This case is a 51 year old male with known multiple myeloma who returned for his third course of chemotherapy. During the admissions history and physical the patient reported a one week history of left posterior pleuritic chest pain that felt better when he rubbed the area with his hand. He had dyspnea on exertion but not at rest. He denied cough, fever, or hemoptysis. He also reported an episode of left lower extremity pain that began 3 weeks ago. The pain has decreased but the swelling has persisted. His arterial blood gases on admission were PH 7.45 Pco2 33 Po2 64 He had a low grade temperature 37.9 The chest x-ray showed a pleural based density in the left costophrenic angle and a linear density in the posterior aspect of the left lower lobe. The perfusion scan showed an area of decreased perfusion in the posterior aspect of the left lower lobe that matched the area of infiltrate in the left lower lobe on the chest radiograph. The ventilation scan showed decreased ventilation in the posterior aspect of the left lower lobe.

Back to main page When the Mills study of multiple myeloma among Seventh-Day Adventists claimed in 1990 that smokers had over three times the risk of non-smokers, the national media eagerly trumpeted it as yet another new danger of smoking. This was despite the fact that previous studies actually found a slightly reduced overall risk among smokers, and that Mills' 21 cases represented not even 3% of the multiple myeloma cases studied to that date. It's a classic "little tiny study with a great big headline." Just as important is the basic epidemiological principle that cases and controls should be as ALIKE as possible except in the condition or protocol being studies. Seventh-Day Adventists are not representative of the general population. Their drinking, smoking, dietary and exercise habits are well outside the parameters of the population at large. In effect, using them as a "control group" is not only scientifically unsound; it borders on the absurd.. What is more, even Mills et al. later admitted that

Questions and Answers What is multiple myeloma? How does it occur? What are the symptoms? How is it diagnosed? ... How can I take care of myself? What is multiple myeloma? Multiple myeloma is a cancer of plasma cells. Plasma cells are white blood cells made in the bone marrow, which is the spongy tissue inside bones. Normal plasma cells make antibodies to fight infection. In multiple myeloma, abnormal plasma cells grow rapidly and uncontrollably to form tumors in bone. They make abnormal antibodies, which are not useful, and the tumors interfere with the growth and function of normal cells made by the bone marrow. This results in anemia and lowered resistance to infection. Fewer blood platelets are produced, so your blood clots more slowly than normal. The uncontrolled production of plasma cells in the bone marrow also causes pain in the bones. Bone tissue may be destroyed, and the bones may break more easily. Calcium from destroyed bone tissue can accumulate in the blood and damage the kidneys. Collapsed bones in the spine may compress nerves, causing numbness or paralysis. If it is not treated, multiple myeloma can spread throughout your body and be fatal.

Overview
In 2001, about 14,400 people in the United States will be diagnosed with multiple myeloma. The number of new cases diagnosed each year has increased over the past decades, for reasons not well understood. Risk Factors In most cases, patients with multiple myeloma have no known risk factors. Symptoms The symptoms of multiple myeloma most commonly include one or more of the following: bone pain and skeletal fractures; infections; weight loss; hypercalcemia; loss of kidney function; hyperviscosity.
