Jan 31, 2004 As reported in The Times of India and elsewhere, a British woman gave birth to twin boy and girl who are genetically her grandchildren. The grandmother agreed to become the surrogate mother because the twins' genetic mother had a congenital condition in which her uterus and a few other reproductive organs are missingbut her ovary was intact and could provide an ova for IVF treatment. Her condition is known as MRKHMayer-Rokitansky-Kuster Hauser syndrome (simply referred to as the "Rokitansky syndrome" in the article)and occurs in about one in 5,000 women. Absence of the Uterus (Vaginal Agenesis)
by Marjorie Greenfield, M.D.

reviewed by Laura Jana, M.D., F.A.A.P.
Many girls and women have never heard of vaginal agenesis, a condition in which a girl is born without a uterus. And yet it's fairly common, affecting 1 out of every 4,000 girls. The condition can be traced to the fetal stage of development, when the uterus and upper vagina do not form properly. Abnormalities of the kidneys or bones may occur as well.
Girls with vaginal agenesis (also referred to as mullerian agenesis, Rokitansky syndrome, and Mayer-Rokitansky-Kuster-Hauseror MRKHsyndrome) usually experience normal pubertyincluding breast development, the appearance of pubic hair, and a growth spurtbut they never get a period.
The external, visible genital area of girls with this condition looks totally normal; it's only upon closer examination that a healthcare practitioner will discover a very shallow vagina and, after further testing, the absence of a cervix and uterus. Occasionally there is a small, nonfunctional "remnant uterus." Ultrasound or magnetic resonance imaging (MRI) scan is usually needed to be certain about this diagnosis.
Sexual intercourse Before sexual intercourse is possible, most girls with vaginal agenesis must undergo a procedure to lengthen the vagina. This generally is best accomplished with the dilation method, where a young woman is taught how to put pressure on the inside of her vagina with a dilator, as instructed by a doctor or nurse. Most girls feel ready to take on this responsibility between the ages of 15 and 20. Gradually, after a few months of daily exercises, the vagina becomes deeper and wider, to make intercourse possible.

ABSTRACT MayerRokitanski-Küster-Hauser (mrkh) syndrome, is a rare disordercharacterized by the congenital absence of the uterus and vagina.
http://www.sin-italia.org/jnonline/Vol14n4/316.html
Table of Contents Case report JNEPHROL 2001; 14: 316-318 Renal abnormalities in Mayer-Rokitanski-Küster-Hauser syndrome Carlo Basile , Vittorio De Michele Nephrology and Radiology Units, Hospital of Martina Franca - Italy ABSTRACT: Mayer-Rokitanski-Küster-Hauser (MRKH) syndrome, is a rare disorder characterized by the congenital absence of the uterus and vagina. The prevalence has been reported as one in 4000 5000 female births. Patients with MRKH syndrome have a 46.XX karyotype and normal secondary sex characteristics. The external genitalia appear normal, but only a shallow vaginal pouch is present. Ovarian function is normal.
A 54-year-old woman came to our observation for the treatment of arterial hypertension. Her history involved primary amenorrhea and sterility. She had undergone abdominal and pelvic sonography as a routine screening and they had shown the absence of the uterus, left renal agenesis and a contralateral pelvic kidney. These findings were confirmed by urography and CT scan of the abdomen. Gynecologic examination showed a small vaginal pouch (2 cm). Thus, the diagnosis of MRKH syndrome with associated congenital anomalies of the upper urinary tract was made for the first time in this lady at the age of 54 years.
Associated congenital anomalies of the upper urinary tract are reported to occur in 30 40 % of all cases of MRKH syndrome. No specific figures are available on what percentage of women with missing kidney might also have MRKH syndrome. However, in 40 50 % of patients with renal agenesis, an associated genital anomaly has been found.

MAYER-ROKITANSKY-KUSTER-HAUSER SYNDROME
Clinical Information:

The patient is a 16 year old white female who had undergone normal development of secondary sex characteristics, yet remained amenorrheic. Her pelvic exam was notable for pain, and the cervix was not visualized on speculum exam. Hormonal tests (estradiol, FSH, LH, TSH, prolactin) were all within normal limits for an adolescent female in the follicular stage. This enabled the physician to rule out androgen insensitivity (testicular feminization), and suggest that a pituitary tumor was not the cause of amenorrhea. An ultrasound exam was attempted (study not available). Transabdominally, the uterus was not identified. An endovaginal US exam could not be performed, secondary to pain. She was referred to MR for evaluation of the pelvic organs. Radiographic Findings:
