Material and Methods
Study Population The study population consisted of consecutive patients with SCD and those with chronic anemia (other than SCD) who were either admitted to King Fahd Hospital of the University or attending the outpatient department. Control subjects were volunteers from hospital employees, blood donors and medical students. They all had their heights and weights recorded. Informed consent was obtained from all subjects who participated in the study after approval from the research committee of the College of Medicine and Medical Science, King Faisal University. Hematological Investigations All subjects had a complete blood count (CBC), hemoglobin electrophoresis (including quantitation of HbF and HbA2), determination of serum iron, serum ferritin and total iron binding capacity.

Mitral Valve Prolapse WHAT IS MITRAL VALVE? The mitral valve is one of the four valves of the heart. It is located in the left side of the heart, and it separates the left atrium (or upper chamber) from the left ventricle (or lower pumping chamber). Because the mitral valve lies between the left atrium and left ventricle, it is also known as the left atrioventricular valve. Valves open and close, like doors. The closure of the valves in the heart is what produces the characteristic "lub-dub" heart sounds. They help direct the flow of blood through the heart, normally allowing flow in one direction. The mitral valve leaflets open equally when blood is flowing from the left atrium and filling the left ventricle. The mitral valve leaflets then close, with both leaflets firm and shut, when the left ventricle contracts and pumps the blood out to the body. WHAT IS MITRAL VALVE PROLAPSE? Mitral valve prolapse (MVP) is a condition in which there is a slight variation in shape or structure of the mitral valve, causing one or both of the mitral valve leaflets to "billow" or "bulge" backward into the upper left atrium during the contraction of the lower left ventricle. Remember, when the left ventricle contracts and ejects blood into the aorta and to the body, the mitral valve leaflets should remain firm and shut. Instead, the mitral valve leaflets(s) billow backward causing prolapse Sometimes when the mitral valve leaflet prolapses, it may produce a "click". As mentioned earlier, valves help direct the flow of blood in one direction through the heart. Often, when the mitral valve leaflet billows, or prolapses, it allows for a small backward leak of blood. This may produce a soft murmur. The "click-murmur", if present, can sometimes be heard with a stethoscope. Over the years, MVP has also been called the Click-Murmur Syndrome, the Floppy-Valve Syndrome, and Barlow's Syndrome, after the name of its discoverer.

Then, in the picture below, notice how in Mitral Valve Prolapse, the ends of the valve do not quite come together, allowing a leakage of blood back into the chamber of the heart. DEFINITION OF MVP:
A fairly common and often benign disorder in which a slight deformity of the mitral valve (situated in the left side of the heart) can produce a degree of leakage (mitral insufficiency). Mitral valve prolapse causes a characteristic heart murmur that may be heard through a stethoscope.
Often no symptoms are present and the condition may be discovered on a routine examination.
On the other hand, there are many who experience the following symptoms :
Chest pain (sharp, dull or pressing).
Fatigue, shortness of breath.
Dizziness.
Anxiety.
Lightheadedness when getting up from a chair or bed.
Palpitations.

What is mitral valve prolapse?
Mitral valve prolapse (also known as "click murmur syndrome" and "Barlow's syndrome") is the most common heart valve abnormality, affecting five to ten percent of the world population. A normal mitral valve consists of two thin leaflets, located between the left atrium and the left ventricle of the heart. Mitral valve leaflets, shaped like parachutes, are attached to the inner wall of the left ventricle by a series of strings called "chordae." When the ventricles contract, the mitral valve leaflets close snugly, preventing backflow of blood from the left ventricle into the left atrium. When the ventricles relax, the valves open to allow oxygenated blood from the lungs to fill the left ventricle. In patients with mitral valve prolapse, the mitral apparatus (valve leaflets and chordae) becomes affected by a process called myxomatous degeneration. In myxomatous degeneration, the structural protein, collagen, forms abnormally and causes thickening, enlargement and redundancy of the leaflets and chordae. When the ventricles contract, the redundant leaflets prolapse (flop backwards) into the left atrium, sometimes allowing leakage of blood through the valve opening (mitral regurgitation). When severe, mitral regurgitation can lead to heart failure and abnormal heart rhythms. Most patients are totally unaware of the prolapsing of the mitral valve. Others may experience a number of symptoms discussed below.

Overview Mitral valve prolapse (MVP) is one of the most common conditions affecting the valves of the heart. It occurs when the twin flaps (leaflets) of tissue that make up your heart's mitral valve billow up and don't close correctly. The mitral valve separates the two chambers on the left side of your heart: the left atrium and the left ventricle. When the mitral valve malfunctions and doesn't close properly, blood may leak from the ventricle back into the atrium. Another name for mitral valve prolapse is click-murmur syndrome. When a doctor listens to your heart using a stethoscope, he or she may hear a clicking sound as the valve's leaflets billow out, as well as a murmur resulting from blood flowing back into the atrium. What is mitral valve prolapse?
Mitral valve prolapse (MVP) occurs when the leaflets of the mitral valve bulge backwards into the left atrium during contraction of the left ventricle. It is caused by an underlying structural abnormality of the mitral valve. Normally, when the heart beats, blood is first pumped through the mitral valve from the left atrium into the left ventricle. The valve acts like a gate, allowing blood to flow in only one direction, from the upper to lower chamber (left atrium to left ventricle). When the left ventricle contracts to pump blood to the body, the mitral valve closes to prevent blood leaking back into the left atrium, and so the blood is expelled from the heart to the rest of the body. If you have mitral valve prolapse, the valve flaps bulge backward into the left atrium. Usually, the effect on your heart is minor because the seal between the two heart chambers is tight enough to prevent leakage. In this case, you won't have any symptoms or need treatment. However, if the valves don't seal tightly, blood could leak back into the left atrium, a condition called

Mitral Valve Prolapse
Mitral valve prolapse (MVP) is the most common condition of the heart valves. Some studies show it affects 6% of all women. Incidence
MVP is extremely interesting since it is so commonly diagnosed in young women and yet, the incidence decreases markedly in elder women. This drop off in incidence is not seen in the male population (refer to graph below). The occurrence of MVP in men is consistent in both young and old populations. Previous to the Framingham study, the incidence of MVP was thought to be much higher in women, however, with this new data, it is now believed that MVP affects equal numbers of men and women. This intrigues researchers searching for the cause(s) of mitral valve disease and attempting to explain why the condition manifests itself so differently in men and women. The cause of MVP is usually unknown but occassionally, MVP is associated with other heart conditions (e.g. atrial septal defect, coronary artery disease, diseases of the heart muscle) but most likely, these diseases would have been present with or without MVP. The outcome from MVP differs for men and women too. Recent studies have indicated that, for men, the incidence of mitral prolapse requiring surgical intervention is higher than for women and increases with age.
