The cardiac valve surgery program at BWH is one of the largest in the United States, treating more than 800 patients per year. The Brigham Cardiac Valve Center is a multidisciplinary group of cardiologists, radiologists, anesthesiologists, and surgeons proficient in diagnosing and treating all cardiac valve syndromes and disorders, including mitral valve prolapse and aortic regurgitation. This involves sophisticated cardiac valve symptoms recognition, diagnostic techniques such as three-dimensional echocardiography, MRI and cardiac catheterization. Cardiac surgeons perform all types of aortic, tricuspid and mitral valve surgery, including the Ross pulmonary autograft switch operation in the young adult, repair of bicuspid and tricuspid aortic valves, homograft valve replacement for endocarditis, and elective aortic valve replacement in all stented and un-stented tissue and mechanical valve prostheses. BWH is a world leader in valve repair for ischemic, rheumatic and myxomatous conditions of the mitral valve. Integrated intraoperative and postoperative echocardiography is used to monitor early and late valve function, and determine the physiology and function of implanted or repaired valves.

The prolapse usually occurs in mid or late systole and may be accompanied by various degrees of mitral regurgitation. The primary pathology is myxomatous degeneration of the mitral valve; there is variable degeneration of the leaflets, chordae tendinae and annulus. Flail leaflet with severe regurgitation is a complication of chordal rupture. The posterior leaflet is more frequently involved. Other valves, particularly the tricuspid valve, may be affected. MVP most frequently occurs as an isolated entity but has also been encountered as a component of several syndromes including Marfans syndrome Ehlers Danlos syndrome osteogenesis imperfecta , and pseudoxanthoma elasticum . MVP may also occur from ischaemic papillary muscle dysfunction. The major clinical feature of the mitral valve prolapse syndrome is mid to late systolic clicks identified with auscultation of the mitral valve. Echocardiography is usually employed for diagnosis. M-mode echocardiography shows posterior movement of the posterior or both leaflets of the mitral valve in systole. Two-dimensional echocardiography displays one or both leaflets prolapsing behind the mitral annulus and into the left atrium in systole. Other features are thickened leaflets and enlarged mitral annulus.

Introduction. Heart murmurs are abnormal heart sounds occurring due to abnormalities in one or more of the hearts four valves (1). The most common type of heart murmur is a mitral valve prolapse (MVP). Poor closure of the mitral valve and/or subsequent regurgitation of blood from the left ventricle to the left atrium causes a clicking or shuffling sound heard mid- to late-systole. MVPs are reported to be the most common valvular heart disease in industrialized countries, affecting about 3% of adults (2,3,4). MVP occurs in 6-10% of young women and 4% of young men. The prevalence may exceed 10% in women ages 14-30. Following is a brief orientation to characteristics of MVP including methods of detection, associated disorders, and treatment. Abnormal Heart Sounds (Heart Murmurs). Depending upon when they occur during the normal cardiac cycle, heart murmurs can be classified into three types: systolic, diastolic, and continuous. Brukner and Khan identify eight systolic, two diastolic, and two continuous types of murmurs (5). Fabius identifies six systolic and two diastolic types of murmurs (6). Lillegard and Rucker identify five systolic, two diastolic, and one continuous type of murmur (7). Although sources differ regarding the number of diagnosable murmurs, the classification of when the murmur occurs is critical to an appropriate diagnosis. (Systolic murmurs are typically benign and diastolic murmurs are always pathological.) MVP is classified as a systolic murmur.

More Disorders The American Heart Association this year formally announced it recognized a connection between panic attacks and Mitral Valve Prolapse (MVP), although it did not understand of the connection. But researchers into this problem understand the connection is actually a dysfunction in the Autonomic Nervous System (ANS). The ANS controls the involuntary system of the body such as heart beat, blood pressure, body temperature, intestinal functions, sweating, etc. The system is made of two parts: the sympathetic (the "accelerator") and the parasympathetic (the "brakes"). When these two are out of balance or goes awry, it is described as dysautonomia. There are many types of dysautonomia, but it is "Mitral Valve Prolapse Syndrome/Dysautonomia" that we discuss here. The name seems to indicate maybe the problem is with MVP when actually MVP is just one of the myriad of symptoms of MVP SYNDROME/DYSAUTONOMIA.

Mitral Valve Prolapse
What is it? Mitral valve prolapse, or MVP, happens when the doors of the mitral valve in your heart do not close smoothly or evenly. MVP may also be called click-murmur syndrome or floppy-valve syndrome. The heart has 4 rooms or chambers in it. The upper chambers are called atria and the lower chambers are called ventricles. When the heart "beats," the atria push blood into the ventricles. The ventricles then push blood out of the heart. There are valves between each of these rooms. These valves are like doors that direct the blood flow through the heart. The mitral valve is between the left atrium and the left ventricle. It is made up of two smaller leaflets or doors. These leaflets come from each side of the valve and meet in the middle. The job of the leaflets is to keep blood from moving back into the left atrium. The valve opens to let blood flow into the ventricle. It closes to keep blood in the ventricle until the ventricle pushes it out. With mitral valve prolapse, part of one or both leaflets falls backward into the left atrium. This may let a small amount of blood leak backward through the valve. Your caregiver may hear a clicking sound of the floppy leaflets. Some of the blood from the ventricle may come back into the atrium. This causes a leaking or "whooshing" sound and is called a murmur.

ou are not crazy. MVPS is real! In the United States alone, over ten million people-many of them women and young adults-are know to suffer from the heart condition known as Mitral Valve Prolapse Syndrome (MVPS). Countless others with chest pains, fatigue, dizziness, depression, migraines, or anxiety attacks are totally unaware that these symptoms may be MVPS related or even that MVPS is treatable. Now, Lyn Frederickson, founder of the first medical facility in the United States solely devoted to the treatment of MVPS, gives us the first complete guide to understanding and coping with this lifestyle-threatening problem. Written for MVPS patients, their family members, and even their physicians, Confronting Mitral Valve Prolapse Syndrome offers:
  • Important information on the nature of MVPS and how it can affect your career, marriage, and self-esteem
  • Guidance on getting a proper diagnosis and finding the right doctor
  • An MVPS exercise program developed especially for MVPS sufferers-with 30 pages of photos
  • Diet and nutrition plans with menus and recipes
  • A thorough discussion of medications-both prescription and over-the-counter
  • A chapter on the twenty most frequently asked questions on MVPS...and much more.

MITRAL VALVE PROLAPSE Gabe Mirkin, M.D. One in ten Americans suffers from mitral valve prolapse and the vast majority have no symptoms and will never know that they have it. Valves are located in your heart to keep blood from backing up. With aging, some of these valves can stretch and to close completely, so they allow a small amount of blood to leak backwards. This is usually harmless, but can be associated with an irregular heart beat or chest pain caused by a stretching of the muscles that hold the valves in place. Patients with mitral valve prolapse usually do not seek out medical help unless a doctor hears a murmur or click in the heart (85 percent), the patient suffers from chest pain (31percent) or palpitations (40 percent), suddenly passes out (40 percent), feels excessively tired (22 percent) or is short of breath (10.5 percent). Mitral valve prolapse is hereditary for one patient in five. When germs get into the bloodstream, they can stick to the rough edges of prolapsed mitral valves and cause an infection, so doctors recommend that people who have this condition take antibiotics whenever germs can enter their bloodstreams, such as during dental or other surgical procedures or when they have a cold. The vast majority of people with mitral valve prolapse do not need any treatment, but those who are bothered by irregular heart beats or chest pain are often given beta blockers, such as propranolol to control symptoms. 1) W Rokicki, J Krzystolikladzinska, B Goc. Good Book I would recommend this book to most. Very short, easy reading, and packed with information. After being diagnosed with MVP this really reassured me. I FEEL ALOT BETTER THANKS TO THIS BOOK! I was in a state of depression after being diagnosed with MVP, now all the missing pieces are falling in place. This book should be required reading for all MVP sufferers. Thank you Dr. Hoffman!!!!! A good primer on a poorly understood syndrome Short and to the point. This little book manages to put a series of symptoms in context for those of us who have gotten to the point that we're a little embarassed to complain to our doctor about yet ANOTHER symptom. Always risking being told that we're somatizing some other malaise, the explanations of the inter-relatedness of symptoms, and the suggestions as to how to render them managable not only have given me actual options to improve my quality of life, but have taken my anxiety level down many notches! I'm not crazy after all! And there's something I can do to help myself! The Mitral Valve Prolapse Syndrome/Dysautonomia Survival Guide Published in Paperback by New Harbinger Pubns (December, 2002)
