Mitral Valve Prolapse
Mitral valve prolapse (MVP) is the most common condition of the heart valves. Some studies show it affects 6% of all women. Incidence
MVP is extremely interesting since it is so commonly diagnosed in young women and yet, the incidence decreases markedly in elder women. This drop off in incidence is not seen in the male population (refer to graph below). The occurrence of MVP in men is consistent in both young and old populations. Previous to the Framingham study, the incidence of MVP was thought to be much higher in women, however, with this new data, it is now believed that MVP affects equal numbers of men and women. This intrigues researchers searching for the cause(s) of mitral valve disease and attempting to explain why the condition manifests itself so differently in men and women. The cause of MVP is usually unknown but occassionally, MVP is associated with other heart conditions (e.g. atrial septal defect, coronary artery disease, diseases of the heart muscle) but most likely, these diseases would have been present with or without MVP. The outcome from MVP differs for men and women too. Recent studies have indicated that, for men, the incidence of mitral prolapse requiring surgical intervention is higher than for women and increases with age.

The diagnosis and treatment of mitral valve disease is an integral part of the delivery of heart care services. The University of Michigan Medical Center is a leader in the diagnosis and treatment of mitral valve disease. Mitral valve disease and its complications, mitral prolapse syndrome, mitral murmurs, congestive heart failure, shortness of breath, embolism, atrial fibrillation, etc. affect tens of thousands of patients each year. This interdisciplinary program was founded to provide these patients with services that their primary care physicians and cardiologists can not provide, including:
  • provision of identified patients with a multi-disciplinary approach to the diagnosis and treatment of mitral valve disease evaluation of the etiology of mitral valve disease generation of research literature regarding mitral disease provision of patients and referring physicians with mitral valve disease education in the areas of effective diagnostic methods and treatment options available identification and targeting of adult patients at risk for and with mitral valve disease
  • Comprehensive evaluation of patients with mitral valve disease is a frustrating problem in the practice of cardiology, internal medicine and family practice, as many physicians are not familiar with the scope of this disease. The establishment of this program is in keeping with current trends in medical care, which are focusing on timely, effective interdisciplinary approaches to the treatment of systemic diseases.

    INTRODUCTION
    Mitral valve repair is an open heart procedure performed by cardiothoracic surgeons to treat stenosis (narrowing) or regurgitation (leakage) of the mitral valve . The mitral valve is the "inflow valve" for the left side of the heart. Blood flows from the lungs, where it picks up oxygen, and into the heart through the mitral valve. When it opens, the mitral valve allows blood to flow into the heart's main pumping chamber called the left ventricle. It then closes to keep blood from leaking back into the lungs when the ventricle contracts (squeezes) to push blood out to the body. It has two flaps, or leaflets.
    The mitral valve is highlighted on this illustration of a heart. The mitral valve is the "inflow valve" for the left side of the heart.     *Oddzia³ Internistyczno-Kardiologiczny Szpitala w ¦wiebodzicach A case of 41-year-old patient with moderate combined mitral valve disease and severe left ventricular insufficiency is presented. Despite of only moderate mitral valve stenosis the patient suffered from recurrent pulmonary oedema. Angiography showed normal coronary arteries, and the patient underwent mitral valve replacement with subsequent significant clinical improvement. The issues concerning referral for surgery and decision-making in valvular disease are discussed.

    Sweedish Study: Chronic valvular disease in Cavalier King Charles Spaniels
    by Jens Haggstrom Currently the most serious problem facing Cavlier breeders today is Mitral Valve Disease. It is estimated that this may affect as much as 50% of the dogs in the breed in North America at this point. MVD leads in many cases to premature death or compromised quality of live and occurs in Cavaliers at a rate unknown in any other breed in the world. Dr. Mike O'Grady, cardiologist affiliated with the Ontario Veterinary College, University of Guelph, Ontario, Canada has been collecting data since 1993. You should quiz the breeder about the steps they are taking to avoid MVD in their lines. Remember that this is a relatively new problem to be recognized in the breed and no firm or consistent policies have been set as data is still being collected. It appears that color doppler ultrasound exams are the best for picking up evidence of MVD, but auscultation exams are also often done. In addition, the evidence appears to be mouinting that puppies bred from parents who show clear of MVD at older ages (3-5 years) are more likely to be clear themselves or to develop the less virulent form of MVD. Request to SEE the documentation for the dam. You should also be able to see copies for the stud dog even if it doesn't live with the breeder.

    A LETTER TO THE EDITOR OF ANTIC
    I wanted to share some information that we have on mitral valve disease as Max struggled with it, having been diagnosed with a murmur when he was five. (Ed. Note: See Max's obituary We recently took him to Colorado State University to Consult with Dr. Christopher Orton, a veterinary cardiologist. He is the only veterinary cardiologist performing open-heart surgery on dogs to repair/replace (with tissue valves) their mitral/tricuspid valves. Max was to have been his smallest patient, but had deteriorated too far to sustain him to his surgery date. CSU was also waiting for a heart-lung machine to accommodate his size, as most of the other dogs were larger. Dr. Orton did an outstanding job of adjusting Max's medications, thereby giving him an extra few months of good life. One thing he noted which I feel is important to convey to others battling this horrible disease is that the dog's blood pressure needs to be monitored and kept at 100-120 as high blood pressure can exacerbate the disease. Dr. Orton felt strongly that this was important in the management of this disease

    What is the mitral valve?
    The heart has four chambers. The upper chambers are called atria (singular: atrium), and the lower chambers are called ventricles. The heart is also divided into right and left sides.
    Blood flows from the body into the right atrium. It is stored there briefly then pumped into the right ventricle. The right ventricle pumps blood into the lungs where it receives oxygen. It flows from the lungs into the left atrium where it is held a few seconds before going into the left ventricle. The left ventricle is surrounded by the largest and strongest of the heart muscles. This large muscle is necessary to pump blood to all parts of the body.
