What is the prognosis?
The prognosis for most individuals with Miller Fisher syndrome is good. In most cases, recovery begins within 2 to 4 weeks of the onset of symptoms, and may be almost complete within 6 months. Orthoptists are often confronted with patients with diffuse ophthalmoplegia of apparent supranuclear origin who have normal neuroimaging studies. Disorders like myasthenia gravis, vasculopathic, inflammatory and traumatic cranial nerve palsies are frequently encountered. A rare disorder for which the orthoptist should be familiar is called the Miller Fisher syndrome. In 1956 C. Miller Fisher described three patients with diffuse ophthalmoplegia, ataxia and loss of tendon reflexes. He theorized that these patients had an unusual variant of the Guillian Barre syndrome. In order to fully appreciate the Miller Fisher syndrome, a review of Guillian Barre syndrome is in order. © 2001 The Board of Regents of the University of Wisconsin System

Thanks for the kind words and advice on my Mom. FYI, today we finally got a diagnosis of Miller Fisher syndrome. NOW all the pieces fit!!! I've posted another article today (12/9/98) describing what we've learned. Best of luck to all out here searching for answers!

Institute of Neurology, University of Vienna, Austria. andrea.neisser@univie.ac.at Seven patients with Miller Fisher syndrome (MFS), six in the acute phase and one in the recovery phase, were investigated for serum antibodies against gangliosides and purified lipopolysaccharides (LPS) from different strains of Campylobacter jejuni, including the MFS- associated serotypes O:2 and O:23. Immunoglobulin G antibodies against gangliosides GT1a and GQ1b were found in five of six patients in the acute phase of disease. Thank you for providing this forum.
I was recently hospitalized and diagnosed with Miller Fisher syndrome. It began one day with distorted vision and by the next morning I could not control my motor functions and my vision was double. The neurologist ordered every tetst imaginable, blood, CT, tensilon, cranial and spinal MRIs, lumbar puncture, EMG etc.
My dilemmas is, nothing unusual has shown up in the diagnostic work. All of the tests were withing the normal range. There was a very small brain lesion on the MRI but I was told, not in an area likely to cause the type of motor problems I was having. They initially began treating me with steroids for MS, but based on the presence of ophthalmoplegia and lack of reflexes, changed the diagnosis to Miller Fisher. I was given 5 days of IVIg and released. I am still weak - but can walk - although the double vision persists.
During a followup appointment last week, my neuro told me that a blood antibody test for Miller Fisher had just come back "negative" but they are standing by the diagnosis based on my symptoms and the response to IVIg.
