New drugs for migraines studied
From Toria Tolley CNN (CNN) Kiva Schindler is one of the 28 million Americans who suffer from migraine headaches. As often as five times a week, she gets a pulsating feeling accompanied by pain, nausea and light and noise sensitivity. When a migraine hits, her work and home life come to a halt. "I usually have to get someone, my mother or my sister-in-law or somebody, to come and actually get my children ... because they are truly not something that you can just go on with your day to day routine," she said. RESOURCES National Headache Foundation As a woman, she is far from alone. According to the National Headache Foundation, nearly one in five women suffer from migraine headaches, compared with about one in 20 men. Why the difference? Dr. Marshall Nash, a headache specialist and researcher, says there are several biological and environmental reasons why some women get more headaches than men. He points to "a genetic predisposition to have shifts in the chemical called serotonin," in the brain, which plays a role in pain and depression. Serotonin levels also change when estrogen levels drop. Schindler has another possible explanation.

CHICAGO, Illinois (Reuters) Researchers have identified brain lesions in migraine victims, a finding that could indicate the severe headaches are a progressive brain-damaging disease in some cases, new research suggests. The research, which has possible implications for treatment, involved 295 Dutch adults aged 30 to 60, some of whom had migraines with visual disturbances and some migraines without eye problems. They were compared to 140 similar people who were migraine-free. Using magnetic resonance images, the researchers found that for patients with both migraines and visual problems the risk of cerebral infarction tissue which has died due to lack of oxygen when a blood clot blocks an artery was 13 times higher than the group which had no migraines at all. The problem increased with the frequency of migraine attacks.

Drug Treatments for Migraine
Bandolier has been stimulated by a number of papers on different treatments for migraine to try and put together a league table of relative effectiveness. In an ideal world this would be created from all the available evidence from systematic reviews. The problem is that the systematic reviews either have not yet been done, or anyway Bandolier couldn't find any.
In these circumstances the choice is either to forget it, or to press on with what evidence is available. Bandolier , you will not be surprised to learn, chose the latter course. So this section comes with the health warning that it is not complete - it is a "Saturday night special" review. Perhaps migraine treatment, which is now a big industry, is one of those topics about which people should write to ask for a Health Technology Assessment.

What is a migraine and how does it differ from tension headaches?
Tension headaches are usually described as a continuous pressure pain or tightness of varying severity over the entire head, whereas migraine headaches are a severe, throbbing pain over one or both temples, or behind one eye or ear, and are often accompanied by nausea and vomiting. A migraine often starts on waking up in the morning, but can occur later in the day and can last hours to one or two days. In individuals with a form of migraine called classic migraine, visual symptoms described as bluriness, dazzling zigzag lines, blind spots or sensitivity to light occur just before and sometimes during the headache. While fatigue and stress can sometimes bring on both tension and migraine headaches, bright lights, noise and alcohol are specific factors that can trigger a migraine.

The main types of migraine in office practice
Most migraines seen in physicians' offices are migraine without aura (formerly called "common migraine") and migraine with aura (formerly called "classic migraine" by some). Migraine aura without headache is also quite common, and is seen often by ophthalmologists. Neurologists and headache specialists often treat status migrainosus, characterized by a headache phase of over 72 hours. The other migraine types are listed in the left side bar and are fully described in the Headache Classification Committee's classification (1988).

Migraine Headaches
What most people think are migraine headaches are really tension headaches. "Real" migraine headaches are usually associated with nausea or vomiting and tend to be preceded by seeing flashes of light, zigzags, blind spots, or stars. Doctors call these preceding visual symptoms "auras," which are not nearly as much fun or as practical as what New Age people refer to as auras (and the nausea is no fun at all). Migraines are often triggered by psychological stress, but distinct from tension headaches, migraines tend to begin after a stressed person is finally able to relax; then that "relaxing" weekend or vacation becomes relaxation hell. During a migraine headache, blood vessels initially become overly constricted and then abnormally widened. You usually experience this pain on one side of the head, which can make you feel lopsided. Timber!
