Mesothelioma
Mesothelioma is a rare form of cancer that attacks cells, called mesothelial cells that make up membrane lining in the chest and abdominal cavity. The tissue called mesothelium, which is formed by the mesothelial cells, help protects the organs by producing a lubricating fluid that allows the organs to move without irritating nerves. Almost always mesothelioma is caused by exposure to asbestos but some researchers believe the virus SV40 can also be a factor. While mesothelioma affects the lungs, it is not a lung cancer. Smokers are at risk for lung cancer, which in effect could lead to contracting other asbestos-related diseases. Lung cancer is more common and arises in the lungas inner tissues opposed to mesothelioma, which develops in the tissue that covers the lung. Mesothelioma is most commonly located in the pleural and peritoneal regions of asbestos exposed workers. Pleural mesothelioma is cancer of the pleura, the membrane that lines the lungs and chest cavity. Peritoneal mesothelioma is cancer of the peritoneum, the lining of the abdomen. Pleural mesothelioma can usually be diagnosed earlier than peritoneal mesothelioma. Both pleural and peritoneal cancer involves serosal membranes, which are delicate membranes composed of connective tissue. Mesothelioma normally begins in the lungs and spread to the abdominal lining, which worsens the condition. The invasive nature of the tumor on heart and lungs are usually the ending factor in the patient.

Presentation
A 67 year old male presented with cough and dyspnea. A chest radiograph (not showm) revealed moderately large left pleural effusion. A chest MRI (not shown) was also performed that showed contrast enhancement of the left pleura. FDG PET imaging and thoracoscopy were then performed.
Imaging Technique
Coronal FDG PET: Attenuation-corrected imaging 45 minutes after intravenous injection of 17 mCi FDG.
Imaging Findings
The series of coronal PET images , from anterior (top left) to posterior (bottom right), show asymmetric abnormal hypermetabolism surrounding the left hemithorax highly suspicious for malignancy.
Differential Diagnosis
The differential diagnosis for pleural disease is as follows:

  • Diagnosis
    The patient underwent thoracoscopy with pleural biopsy followed by left extrapleural pneumonectomy. Both the biopsy and the surgical specimen demonstrated extensive malignant mesothelioma involving both parietal and visceral pleura.

    Asbestos Disease Related Links Tumors of the mesothelium can be either benign (non-cancerous) or malignant (cancerous). Since malignant mesothelioma is much more common than benign mesothelioma it is often simply called mesothelioma. Characteristics of a malignant tumor are that it has the ability to spread to other parts of the body, making it much more dangerous.
    Malignant mesothelioma
    is divided into three types: epithelioid, sarcomatoid, and mixed/biphasic. 50% to 70% of mesothelioma cases are epithelioid, which also has the best prognosis (outlook for survival). 7% to 20% are of the sarcomatoid type, and 20% to 35% are the mixed/biphasic type. Epithelioid affects the membranous tissue covering internal organs and other internal surfaces of the body. Sarcomatoid arises from supportive tissue, such as bone, muscles, cartilage, or fat. Biphasic mesothelioma has features of both epithelioid and sarcomatoid. The treatment options are the same for each type.
    Approximately three-fourths of mesothelioma cases start in the chest cavity and are known as pleural mesothelioma . Another 10% to 20% begin in the abdomen and are called peritoneal mesothelioma . Those starting in the cavity around the heart, or pericardial mesothelioma, are very rare. The covering layer of the testicles is actually an outpouching of peritoneum into the scrotum. Mesothelioma that affects this covering of the testicles is quite uncommon.

    PREDICTED DEATHS FROM MESOTHELIOMA
    The Health and Safety Executive (HSE) today published updated statistics predicting the future numbers of people likely to die in Britain as a result of mesothelioma, a form of cancer caused by asbestos. These statistics, put together using the latest modelling techniques, suggest the annual number of deaths will peak at a lower level than previously predicted (published in 1995), and will peak sooner. The number of mesothelioma deaths in Great Britain (males and females of all ages) is now predicted to peak somewhere between 1,950 and 2,450 annually. The peak is expected between the years 2011 and 2015. Previously published projections were restricted to men below age 90. Making the same restriction to the current model suggests a peak of between 1,650 and 2,100 deaths, between the years 2011 and 2015. This updates a projection made in 1995 which suggested an annual peak of between 2,700 and 3,300 deaths around the year 2020.

    Mesothelioma is almost always fatal, but steps can be taken to prolong life. So far, treatment options include: Surgery - Depending on how far the cancer has progressed, it may be necessary to remove part of the lining of the chest or abdomen and some of the tissue around either. In more advanced cases, a doctor may have to remove one of the patient's lungs, part of the diaphragm, or the pericardium. Draining fluid from the lungs, abdomen or pericardium is another surgical procedure. This treatment is used to ease a patient's pain or discomfort, and sometimes to prevent fluid from building up again, but it is not a cure for the disease. Chemotherapy - This is the use of drugs for treating cancer. The drugs can be ingested in pill form or they can be injected by needle into a vein or muscle. Chemotherapy therapy is systemic therapy, which means that the drug enters the bloodstream and circulates throughout the body (through the whole system) to reach and destroy the cancer cells.

Healthy mesothelium is composed of a flattened monolayer of mesothelial cells that line the pleural, pericardial, and peritoneal cavities. These cells also cover the tunica albuginea of the testes. Mesothelial cells are characterized by the presence of microvilli and desmosomes. They also have the potential to phagocytose cells and particulate matter. When mesothelial cells are perturbed following inflammation or irritation, they may increase in size (hypertrophy) and number (hyperplasia). Following fluid accumulation within the body cavities, mesothelial cells may exfoliate and implant on serosal surfaces. Mesotheliomas are malignant neoplasms. Their exfoliated cells also can seed the body cavities. Although multiple tumors often are present, distant metastases are rare. Non-neoplastic mesothelial cells affected by inflammation, irritation, or malignancy may have a similar cytologic appearance to mesothelioma cells. Thus, cytologic distinction of neoplastic and reactive mesothelial cells can be challenging to impossible.

    Monday, January 26, 2004 Honeywell Terminates Bendix Discussions MORRIS TOWNSHIP, N.J. - Honeywell shares fell more than 4 percent Tuesday on news the company has terminated discussions regarding the sale of its Bendix Friction Materials business. Honeywell said in a statement Monday that it was unable to agree to terms with Federal-Mogul Corp. after several months of negotiations. Last January, the two companies entered into a letter of intent under which Federal-Mogul would acquire Bendix and Honeywell would receive a permanent injunction shielding it from all current and future asbestos-related personal injury claims related to Bendix, according to Honeywell's 10-Q filing released in November. Bendix manufactured automotive brake pads that contained asbestos in an encapsulated form. Through the end of last September, Honeywell had resolved approximately 62,500 asbestos claims, largely from professional brake mechanics. There were approximately 71,000 claims pending last November, according to the company's 10-Q filing.
