Current Therapy for Mesothelioma

Malignant pleural mesotheliomas are locally aggressive, invasive, and almost universally fatal. Background: Diffuse malignant pleural mesotheliomas (DMPMs) are highly lethal tumors that are becoming more common. Standard management approaches have provided limited effectiveness.
Methods: The literature on management has been revised, and the authors present their data on outcomes for 120 patients treated with an aggressive trimodality approach.
Results: An aggressive trimodality approach including extrapleural pneumonectomy followed by chemoradiation produces low mortality and acceptable morbidity. The five-year survival rate in patients with epithelial histology and negative nodes approaches 40%.
Conclusions: Nodal status and histologic subtype are major predictors for survival in patients with early DMPM. A uniformly accepted staging system would allow comparison of treatment approaches from various institutions. More effective management interventions are required.
Introduction
Mesotheliomas of the pleural cavity are relatively rare tumors. Generally, two types of pleural tumors can be referred to as mesotheliomas. The less common is the solitary (or localized) fibrous tumor of the pleura, previously known as "benign mesothelioma." This slow-growing, commonly benign, well-circumscribed tumor is pedunculated on a pleural-based pedicle and often is cured by resection. The tumor appears to originate from submesothelial rather than mesothelial or epithelial cells.

What is the mesothelium?
The mesothelium is a membrane that covers and protects most of the internal organs of the body. It is composed of two layers of cells: One layer immediately surrounds the organ; the other forms a sac around it. The mesothelium produces a lubricating fluid that is released between these layers, allowing moving organs (such as the beating heart and the expanding and contracting lungs) to glide easily against adjacent structures. The mesothelium has different names, depending on its location in the body. The peritoneum is the mesothelial tissue that covers most of the organs in the abdominal cavity. The pleura is the membrane that surrounds the lungs and lines the wall of the chest cavity. The pericardium covers and protects the heart. The mesothelial tissue surrounding the male internal reproductive organs is called the tunica vaginalis testis. The tunica serosa uteri covers the internal reproductive organs in women.

Background Mesotheliomas form a spectrum of tumors that usually arise in the pleura or in the abdominal cavity. The malignant tumors arising within the pleura are strongly associated with prior asbestos exposure in up to 90% of cases. The time period from initial exposure to development of the cancer may range from 25-45 years and the lifetime risk of developing mesothelioma following heavy exposure ranges from 7-10%. It diffusely spreads through the pleural space, directly invading other thoracic structures and associated with pleural effusion. The underlying lung is usually encased within the tumor. The abdominal peritoneal variant is also related to asbestos exposure and 50% of these patients may have pulmonary fibrosis. Although 50% of these cases may be limited to the abdominal cavity, intestinal obstruction is common frequently leading to death. The classic histopathology of the mesothelioma is a biphasic tumor with both an epithelioid and sarcomatoid or spindle cell component. From this description, it can be readily appreciated how the diagnosis may be very difficult to confirm. The table below lists the major histologic variants of the tumor. The task of the pathologist is to separate the mesothelioma from its many mimics including adenocarcinoma from the lung (in pleural cases) and soft tissue sarcomas such as malignant fibrous histiocytoma (in peritoneal cases). Immunoperoxidase studies and electron microscopy may aid in this distinction.

This page provides an overview of treatment and clinical research in the Thoracic Oncology Program at Johns Hopkins which is focused on mesothelioma. Recently added clinical trials in mesothelioma will soon be open to enrollment. Please click on the highlighted links for more detailed information. Mesothelioma is one form of cancer found in the chest which has been shown to have a direct link to previous exposure to asbestos. Patients with this diagnosis have very poor chances of cure. Chemotherapy, radiation, or surgical treatment each used alone have not improved survival. A combination of surgery followed by chemotherapy and radiation has shown only minimal improvement to this outcome. Chemotherapy given pre-operatively has been shown to increase the chances for survival in patients who have a diagnosis of lung cancer and esophageal cancer. Hopkins researchers in the Thoracic Oncology Program feel that this approach will yield similar results for cases of mesothelioma. Dr. Yang, Dr. Ettinger, and Dr. Gabrielson are working to offer better treatment to patients with this diagnosis by specifically targeting the genetic material in mesothelioma cells. It is composed of a fairly unique type of cell, also found in the lining of the abdomen. When this tissue-mesothelium-undergoes malignant change, it is natural to designate it as "mesothelioma." One of the pecularities is the capacity to produce two different kinds of cells- "epithelial" and "fibrous." The neoplasms that are produced can be predominately one or the other cell type or "mixed" (biphasic). When it is predominately fibrous, it is categorized as "fibrous mesothelioma" but, even here, some epithelial cells are often seen, although they may have to be diligently sought. "Malignant fibrous mesothelioma" thus designates a particular appearance of the cells that constitute the tumor. John Battelle found a story about the bidding war over the keyword Mesothelioma for targeted search engine advertising. It seems the lawyers are raking in so much money on their Mesothelioma cases that they have bid the keyword up to $100 in some cases. According to Mesothelioma Web , one could have been exposed to asbestos for a period of only a few months and then develop the rare cancer 30 to 40 years later. So, many people who were exposed in the mid-1900s are just now seeing symptoms. And in the world of always-on internet connections that we live in today, it is only natural to see an overwhelming amount of advertising for anything that can be so profitable. The Thoracic Oncology Program at the University of Maryland Medical Center is one of the pioneering institutions nationally using a finely tuned multidisciplinary approach to the treatment of malignant mesothelioma and other asbestos-related diseases. With our multidisciplinary care program, patients have access to all of the team members in one visit. Treatment with medical, surgical and radiation oncologists is coordinated through one specialized nurse. Communication is maximized and patient stress and inconvenience is minimized. This "one-stop shopping" approach has been found to be the best way to tackle the complex care for this difficult disease. Mesothelioma Malignant mesothelioma, an uncommon form of cancer, is a disease in which cancer (malignant) cells are found in the sac lining the lung and chest cavity (the pleura), abdomen (the peritoneum) and the heart (the pericardium). A layer of specialized cells called mesothelial cells lines these cavities, forming tissue called mesothelium. Tumors of the meothelium can be benign (noncancerous) or malignant (cancerous). Most mesothelial tumors are cancerous; therefore, malignant mesothelioma is often simply called mesothelioma.
