What is malignant mesothelioma?
Malignant mesothelioma, a rare form of cancer, is a disease in which cancer (malignant) cells are found in the sac lining the chest (the pleura) or abdomen (the peritoneum). Most people with malignant mesothelioma have worked on jobs where they breathed asbestos. A doctor should be seen if a person has shortness of breath, pain in the chest, or pain or swelling in the abdomen. If there are symptoms, the doctor may order an x-ray of the chest or abdomen. The doctor may look inside the chest cavity with a special instrument called a thoracoscope. A cut will be made through the chest wall and the thoracoscope will be put into the chest between two ribs. This test, called thoracoscopy, is usually done in the hospital. Before the test, the patient will be given a local anesthetic (a drug that causes a loss of feeling for a short period of time). Some pressure may be felt, but usually there is no pain. The doctor may also look inside the abdomen (peritoneoscopy) with a special tool called a peritoneoscope. The peritoneoscope is put into an opening made in the abdomen. This test is also usually done in the hospital. Before the test is done, a local anesthetic will be given.

Asbestos exposure can occur in many ways including, but not limited to, service on ships in the Navy, work aboard military or commercial ships, work as an insulator, or work in residential or commercial construction. In addition, asbestos exposure can occur from contact with individuals working in these trades. Finding good doctors and cancer treatment centers is very important for persons with mesothelioma or any other type of asbestos cancer. Over the past 20 years, we have helped people diagnosed with mesothelioma all over the nation. Prognosis in this disease is difficult to assess consistently because there is great variability in the time before diagnosis and the rate of disease progression. Various surgical procedures may be possible in selected patients, providing long-term survival without cure. In large retrospective series of pleural mesothelioma patients, important prognostic factors were found to be stage, age, performance status, and histology. [ ] For patients treated with aggressive surgical approaches, factors associated with improved long-term survival include epithelial histology, negative lymph nodes, and negative surgical margins. [ ] For those patients treated with aggressive surgical approaches, nodal status is an important prognostic factor. [ ] Median survival for malignant local pleural disease has been reported as 16 months and extensive disease as 5 months. In some instances the tumor grows through the diaphragm making the site of origin difficult to assess. Cautious interpretation of treatment results in this disease is imperative because of the selection differences among series. Effusions, both pleural and peritoneal, represent major symptomatic problems for at least two thirds of the patients. A history of asbestos exposure is reported in about 70% to 80% of all cases of mesothelioma. [

What is Mesothelioma? Mesothelioma from the (Greek meso+ thelioma, tumor of middle lining tissue) is an uncommon cancer, originating from the cells which form the membrane lining the abdominal cavity (peritoneal membrane or peritoneum) which houses the intestines, or the chest (pleural membrane or pleura) cavity housing the heart and lungs, in which the cells making up those tissues begin to grow out of control.
Mesotheliomas most often are seen in older patients, more often men that have a history of occupational exposure to asbestos, although other causes such as radiation and certain viruses have occasionally been implicated. In a proportion of cases, no asbestos exposure can be identified.
Mesotheliomas involving the lung and pleura characteristically present as progressive shortness of breath due to the thickening of the lining membrane of the lung with gradual contraction of the breathing space; often, fluid accumulates in the lung spaces as well, further interfering with breathing, Mesotheliomas involving the abdominal cavity present with digestive symptoms, and abdominal swelling due to thickening of the lining membranes of the gut, and accumulation of large amounts of fluid in the abdomen.

Onconase plus Doxorubicin vs. Doxorubicin Title: Multicenter randomized comparative phase III trial of onconase plus doxorubicin vs. doxorubicin in patients with malignant mesothelioma (part II) Eligibility Requirements:
Histologically verified diagnosis of malignant mesothelioma
Pathology slides must be available for independent review
Patients aged 21 years or older
Chemotherapy naive or 1 prior chemo (excluding doxorubicin) Treatment Plan:
Randomized to either Onconase plus doxorubicin or doxorubicin alone
Group I: Onconase will be given as a bolus intravenous infusion over 30 minutes weekly followed by a bolus intravenous infusion of doxorubicin every 3 weeks
Group II: Doxorubicin will be given as a bolus intravenous infusion every 3 weeks Trimodal Therapy Title: Phase II trial of combined resection, intraperitoneal chemotherapy, and whole abdominal radiation in peritoneal mesothelioma Eligibility Requirements:

Mesothelioma, a rare form of cancer of the membranes lining the chest or abdominal cavities, has recently been front and center in the European media. Numerous news reports have alerted the public to forecasts of increases in the number of mesothelioma cases in the coming years. Most of the reports have linked these increased rates to 'asbestos exposure', without differentiating between fibre of product types. In general, only cursory attention has been paid to the large body of scientific evidence that has been accumulating since the 1960s regarding the nature of mesothelioma and its risk factors.

    The discovery that exposure to certain types of asbestos is linked to pleural mesothelioma is a result of the pioneering work of Dr. Christopher Wagner, who documented the high incidence of the disease amongst people working at or living near crocidolite (blue) asbestos mines as well as in household members of workers at these mines. Later research by Newhouse and Thompson (1965) also found elevated mesothelioma risks amongst workers (and their household members) at a manufacturing plant using crocidolite. Generally, once diagnosed, cases of mesothelioma are rapidly fatal, but the very long latency of the disease means that symptoms may only begin to appear 20, 30 or even more than 50 years after initial exposures.

    What is Mesothelioma?
    Mesothelioma is a rare form of cancer. It affects the lining of the cavities around the lungs, stomach and heart. Most cases of mesothelioma are related to asbestos exposure. Some of the occupations that exposed workers to asbestos directly or indirectly are those involving carpentry, demolition, factory work, insulation, shipbuilding, installation of brake linings in vehicles, and many others. Despite efforts to eliminate asbestos from our environment, many public and private buildings still contain asbestos today.
    The early symptoms of mesothelioma are generally non-specific, and may lead to a delay in diagnosis. The first symptoms of mesothelioma are often shortness of breath, chest pain, fever, cough, nausea, pain in abdomen, or anemia. All of these symptoms are also caused by less serious illnesses, which makes it difficult to recognize the disease in its early stages. If you have worked with or been exposed to asbestos you should get screened, even if you do not feel sick now.

    Mesotheliomas are uncommon tumours arising from the mesothelial surface (pleura, peritoneum, and rarely from sites such as the pericardium and tunica vaginalis testis). Diffuse mesotheliomas are invariably malignant. At the time of presentation, pleural mesothelioma usually is often confined to the thorax, however, disease typically contaminates the entire pleural cavity and pleural effusion is common. Later, there is extension to the chest wall, mediastinum and encasement of the lung with a thick rind of tumour. Regional lymph nodes may become involved. Extension through the diaphragm may cause peritoneal involvement and ascites. Hematogenous metastases are uncommon but may be seen particularly in patients surviving longer than average. Patients may present with chest pain, dyspnea and cough. The referring clinical problem may be pleural effusion of unknown etiology. Advancing disease causes respiratory insufficiency, chest wall pain, cardiovascular complications from invasion of the heart and general constitutional deterioration. (Revised May 2001)

    The dense white encircling tumor mass is arising from the visceral pleura and is a mesothelioma. These are big bulky tumors that can fill the chest cavity.The risk factor for mesothelioma is asbestos exposure. However, mesothelioma is rare even in persons with asbestos exposure. Asbestosis more commonly predisposes to bronchogenic carcinomas, increasing the risk by a factor of five. Smoking increases the risk for lung cancer by a factor of ten. Thus, smokers with a history of asbestos exposure have a risk 50 fold greater likelihood of for developing lung cancer.
