Meige Syndrome Secondary to Basal Ganglia Injury: A Potential Cause of Acute Respiratory Distress
C. Adam Kirton and Richard J. Riopelle Abstract: Background: Meige syndrome is a movement disorder that includes blepharospasm and oromandibular dystonias. Its etiology may be idiopathic (primary) or it may arise secondary to focal brain injury. Acute respiratory distress as a feature of such dystonias occurs infrequently. A review of the literature on Meige syndrome and the relationship between dystonias and respiratory compromise is presented. Methods: A 60-year-old woman suffered a cerebral anoxic event secondary to manual strangulation. She developed progressive blepharospasm combined with oromandibular and cervical dystonias. Neuroimaging demonstrated bilateral damage localized to the globus pallidus. Years later, she presented to the emergency department in intermittent respiratory distress associated with facial and cervical muscle spasms. Results: Increasing frequency and severity of the disorder was noted over years. The acute onset of respiratory involvement required intubation and eventual tracheotomy. A partial therapeutic benefit of tetrabenazine was demonstrated.

The purpose of this study was to determine whether there was a difference in the long-term outcome between elderly patients with anxious depression and those with non-anxious depression. Eighty-three patients with non-bipolar, non-psychotic major depression who had responded to treatment of the index episode were maintained on full-dose antidepressant medication and followed on a monthly basis over a period of 2 years. Anxiety status at index assessment was not related to outcome as defined by rates of relapse and recurrence or time to these events. However, the few patients who remained anxious at the point of remission of the index episode had a significantly shorter time to relapse/recurrence. These findings suggest that, once they achieve remission and are given adequate prophylactic treatment, most older patients with anxious depression have a similar long-term outcome to patients without anxiety.

Types of Movement Disorders
Ataxias a group of degenerative disorders affecting the brain, brainstem or spinal cord that erode muscular coordination, causing weakness, atrophy and other problems depending on the tissues involved. The disorders are often inherited. There is no treatment. Dystonia prolonged muscle contractions that can cause twisting and rhythmic, jerky movements or abnormal posture. General dystonia involves the entire body. Focal dystonias involve only one body location, most commonly the neck (spasmodic torticollis), eyelids (blepharospasm), lower face (Meige syndrome) or hand (writer's cramp or limb dystonia). Essential Tremor an uncontrolled shaking or trembling, usually of one or both hands or arms, that worsens when basic movements are attempted. Although emotionally distressing, the condition is usually benign and not a sign of more serious disease. Huntington's Disease a progressive, degenerative disease caused by the deterioration of certain nerve cells in the brain. The condition is hereditary. Symptoms include a wide, prancing gait; hesitant speech; involuntary, jerky movements in arms, neck, trunk and face; personality changes and intellectual deterioration. Onset is usually between ages 35 and 50.
