    Megaloblastic Anemia is a rare blood disorder characterized by the presence of large, abnormal, immature red blood cells (megaloblasts). Decreased numbers and immaturity of white blood cells (leukocytes) and blood platelets (thrombocytes) may also occur. This disorder may be caused by a deficiency or defective absorption of vitamin B12 or folic acid. Certain antitumor or immunosuppressive drugs may also cause Megaloblastic Anemia.

Click here to view next page of this article Vitamin B12 deficiency anemia Since body stores of vitamin B12 are adequate for up to five years, deficiency is generally the result of failure to absorb it. Pernicious anemia, Crohn's disease and other intestinal disorders are the most frequent causes of vitamin B12 deficiency. Symptoms are attributable primarily to anemia, although glossitis, jaundice, and splenomegaly may be present. Vitamin B12 deficiency may cause decreased vibratory and positional sense, ataxia, paresthesias, confusion. Treatment of vitamin B 12 deficiency anemia . Intramuscular, oral or intranasal preparations are available for B 12 replacement. In patients with severe vitamin B12 deficiency, daily IM injections of 1,000 mcg of cyanocobalamin. Folate deficiency anemia Folate deficiency is characterized by megaloblastic anemia and low serum folate levels. Most patients with folate deficiency have inadequate intake. Lactate dehydrogenase (LDH) and indirect bilirubin typically are elevated, reflecting ineffective erythropoiesis and premature destruction of RBCs. RBC folic acid and serum vitamin B levels should be measured. RBC folate is a more accurate indicator of body folate stores than is serum folate, particularly if measured after folate therapy has been anemia, vitamin B12, anemea, megaloblastic

Figure 1. A 36-year-old woman who had been a vegan for many years presented with a one-week history of decreased visual acuity. She had had megaloblastic anemia six years earlier (hemoglobin, 7.5 g per deciliter) that was found to be due to folic acid deficiency. PubMed Citation Abstract We studied a six-month-old infant with severe megaloblastic anemia, coma and hyperpigmentation of the extremities. He was found to have methylmalonic aciduria (79 mumol per milligram of creatinine) and homocystinuria (0.85 mumol per milligram of creatinine). Additional biochemical abnormalities included cystathioninuria, glycinuria, methylcitric aciduria, 3-hydroxypropionic aciduria and formic aciduria. The concentration of vitamin B12 in the serum was 20 pg per milliliter. This severe nutritional deficiency was a consequence of inadequate intake, for the infant was exclusively breast-fed by a strictly vegetarian mother who manifested methylmalonic aciduria. Our observations emphasize the importance of educating strict vegetarians about the deficiency of vitamin B12 in their diets and the importance of vitamin B12 supplementation.

Complete Blood Count
One test for determination of anemia is the complete blood count (CBC), a test that provides the physician with information on the number, size and appearance of red blood cells. The iron component, hemoglobin, and hematocrit are also measured in a CBC. The hematocrit is the percentage by volume of RBCs in the blood and can indicate possible anemia. From the blood smear portion of a CBC, RBC morphology can be evaluated for cell color, size, and shape.
Vitamin B12
Vitamin B12 deficiencies may result in neurologic symptoms such as impaired touch perception and decreased reflexes. More serious conditions due to low vitamin B12 levels may include psychological symptoms such as memory loss, disorientation, and depression. These conditions may be seen either with or without macrocytic changes in the blood cells.
Testing for high vitamin B12 levels is less common, but high levels can occur in cases of chronic renal failure, either acute or chronic myelogenous leukemia, and in some liver diseases.

WHAT ARE THE SYMPTOMS OF ANEMIA?
Common Symptoms
Symptoms of anemia vary depending on the severity of the condition. Anemia may occur without symptoms and be detected only during a medical examination that includes a blood test. When they occur, symptoms may include the following:
  • Weakness and fatigue are the most common symptoms of even mild anemia. (Even iron deficiency without anemia can reduce working capacity in some people.)
  • Shortness of breath on exertion.
  • Rapid heartbeat.
  • Lightheadedness or dizziness.
  • Headache.
  • Ringing in the ears (tinnitus).
  • Irritability and other mood disturbances.
  • Pale skin (it should be noted, however, that healthy-looking skin color does not rule out anemia if a patient has risk factors and other symptoms of anemia) .
  • Restless leg syndrome and other sleep disturbances.
  • Mental confusion.
  • Loss of sexual drive.
Unusual Symptoms
Pica. One odd symptom, and in some cases a cause of iron deficiency is pica . This is the habit of eating unusual substances, such as ice (called pagophagia ), clay, cardboard, foods that crunch, or raw starch. For example, in one study, half of people whose pica took the form of pagophagia (eating at least one tray of ice every day for two months) or eating foods that crunch (such as raw potatoes, carrots, or celery) were iron deficient. The pica often stops, particularly in children, when iron supplements are given. Pica is difficult to detect because patients are often ashamed to admit to such cravings.

HOW IS ANEMIA DIAGNOSED?
Medical History and Physical Examination
The first step in any diagnosis is a physical examination to determine if the patient has symptoms of anemia and any complications. [ See What are the Symptoms of Anemia?] Because anemia may be the first symptom of a serious illness, determining its cause is very important. This may be difficult, particularly in the elderly, malnourished, or people with chronic diseases, whose anemia may be caused by one or more of a number of factors. A detailed medical, personal, and dietary history should report the following:
  • Any family or personal history of anemia.
  • A history of gallbladder disease, jaundice, or enlarged spleen.
  • Heavy menstrual bleeding in women.
  • Any occurrence of blood in the stool or other signs of internal bleeding. (Even if the patient has not observed any bleeding, nonvisible blood may present so a rectal exam and stool test are essential.)
  • Any dietary history, particularly in people who are elderly, poor, or both.
The physician should examine the patient carefully, especially checking for swollen lymph nodes, an enlarged spleen, and pale skin and nail color.
