In Healthy People 2000: National Health Promotion and Disease Prevention Objectives , nutrition screening was emphasized as a necessary, routine component of primary care because so few physicians or other health professionals ask patients about nutrition. A report of the U.S. Senate Committee on Education and Labor stated that "85% of the older population have one or more chronic conditions that have been documented to benefit from nutrition interventions." In 1993, in a national survey commissioned by the Nutrition Screening Initiative (NSI), 750 geriatric doctors, nurses, and administrators of hospitals, nursing homes, and home care agencies reported that one in four of their elderly patients suffer from malnutrition as do one half of their elderly hospital patients, and two in five nursing home residents. Malnourished older Americans get more infections and diseases, their injuries take longer to heal, surgery on them is riskier, and their hospital stays are longer and more expensive.

Its report "Wasting Away - The Crisis of Malnutrition in India" blames poverty and low status of women for some of its most shocking statistics - half of all children under four are malnourished, it says, and 60% of women are anaemic. Mothers and sisters often forego food in poor families to give husbands and sons more than their share, the report says. One of the authors of the report, Mira Chatterjee, says malnutrition on such scale means money invested in education is not used effectively as hungry children cannot study. Economic cost The report estimates that malnourished workers mean an annual loss of $10 bn in lost productivity. The green revolution helped Indian food production It praises India's effort to feed its people through agricultural innovation and poverty alleviation schemes, but says population growth, high prices and unemployment often offset well intentioned government programmes.

It was approved by the Patient Care Committee on July 15, 1996, and by the AGA Governing Board on July 27, 1996. H uman immunodeficiency virus (HIV) infection is a devastating illness with major public health implications throughout the world. Gastrointestinal and hepatobiliary complications are common in patients with acquired immunodeficiency syndrome (AIDS) and affect most patients at some point during the course of the disease. Gastrointestinal complications are important not only because of their frequency but because they result in significant morbidity, mortality, and use of health care resources. Management of these patients provides a challenge to the practicing physician for several reasons, including the atypical presentations, the broad spectrum of potential causes, and the multisystem nature of these complications. In addition, because these complications often occur in patients with advanced disease, ethical considerations must be taken into account. The following guidelines provide recommendations for the management of HIV-infected patients with malnutrition and cachexia, diarrhea, and hepatobiliary diseases. These guidelines are based on a systematic, comprehensive review of the medical literature, which is provided in an accompanying report.

Speaking here at the annual gathering of the UN's Economic and Social Council, A. N. Ngongi, Deputy Executive Director of the World Food Programme, emphasized that the pandemic has been seriously aggravated by a lack of access to nourishing food that plagues a large percentage of Africa's people. According to the UN Food and Agricultural Organization, Ngongi told Reuters Health, food production in the nine countries most affected by HIV/AIDS has dropped by 15% in the past 10 years. Support for populations affected by the pandemic has tended to be focused on urban areas, he continued, with the result that rural areas have been especially hard hit, to the point where many rural communities have been thoroughly destabilized, with a concomitant drop in food output. Ngongi said that in hospitals he visited recently in Uganda and Kenya, some 60% of the beds were occupied by people infected with HIV who were suffering not from opportunistic infections but from severe malnutrition. Experts estimate, he noted, that 35% to 40% of Africa's people suffer from chronic malnutrition, and the problem is increasing rapidly.

Approximately 2.8 million preschoolers are at risk of xerophthalmia (the clinical eye lesions in VAD leading to rupture of the cornea and blindness) each year, and it is estimated that between 250,000 and 500,000 preschool children go blind from vitamin A deficiency annually. Approximately 250 million people are at risk of deficiency of vitamin A worldwide, the prevention of which would reduce childhood mortality rates by up to 30 percent. Iron deficiency anemia (IDA) increases the risk of prematurity and low birth weight, and is a major contributor to maternal mortality. Severe anemia may account for one in five maternal deaths. IDA affects an estimated 42 percent of all women, 58 percent of pregnant women, and a third of all young children globally. IDA is a primary cause of cognitive deficit in infants and young children. In adults, physical work capacity is reduced with even moderate anemia. Iodine deficiency disorders (IDD) impair growth, development, and intellectual capacity, and contribute to reproductive problems in women. IDD affects an estimated 655 million people worldwide, and 915 million more people live in areas that put them at risk for IDD. Iodine deficiency is the greatest single preventable cause of intellectual impairment in the world today. It is a significant health problem in 118 countries.
