Vegan parents on trial for baby's severe malnutrition

Story Tools (Court TV) Silva Swinton and her husband, Joseph, followed a strict vegan diet in their Queens home. They swore off meat and dairy products and existed on a regiment of vegetables, grains, seeds and nuts. When Silva gave birth to a daughter, Ice, in July 2000, they put the newborn on the same plan. Instead of breast milk or baby formula, Ice Swinton got herbal tea, flax seed oil, fruit juices and a homemade soy drink. The result, authorities say, was a health catastrophe that rises to the level of a crime. At 15 months, the girl weighed only 10 pounds and had no teeth. She could not sit up or talk and had a swollen abdomen. Doctors diagnosed her with severe malnutrition and rickets, and the Queens district attorney said it was one of the worst cases of neglect he'd seen. Her parents are now on trial in Queens Supreme Court on first-degree assault charges that could land them each in jail for 25 years. The Swintons, both 32, maintain they are loving parents who doted on Ice and thought the diet was best for her and her little brother, Ini, who was born after the couple's arrest. Both children are in foster care. In his opening statement last Tuesday, Silva Swinton's attorney, Christopher Shella, told the jury of nine women and three men, "At the end of the trial, you'll find that the choices they made were not criminal, but human."

Just eight World Food Program biscuits a day will save this malnourished child By Jamie FlorCruz CNN's Beijing Bureau Chief (CNN) More disturbing images of hungry North Korean children filtered out of Pyongyang this week at the end of a visit by a team of World Food Programme officials. In a pediatric hospital in Nampo City, southwest of the capital Pyongyang, emaciated children, ill with diarrhea or malnourished, lie listless in beds with their mothers, some fastened with drips to give them basic sustenance. "The drips were very crude, almost like water bottles with plastic tubing coming out," says Catherine Bertini, WFP executive director. "We saw other children who had marks on their faces from various stages of malnutrition. These children were in very bad shape." It's a near replay of chilling pictures from past years of dying children, their stomachs bloated, arms hanging like twigs and eyes staring vacant.

Gastroenterology is a specialty involved in preventing, diagnosing, and treating diseases of the luminal GI tract (esophagus, stomach, small intestine, colon and rectum) liver, gallbladder and pancreas.
The Gastroenterology division at UMass Memorial Medical Center is dedicated to excellence in patient care, research and medical education. Our staff is committed to providing the highest quality of care to individuals as well as to the community. We strive to advance patient care through excellence in clinical, basic and translational research, and to educate future health care professionals. The high standards set by the division and the scope of services available ensure that patients will receive the best care possible for a wide variety of gastrointestinal and liver diseases.
As a tertiary referral center, we offer expertise in all areas of clinical GI, hepatology and adult nutrition. Our dedicated clinical staff is a caring team of specialized doctors, nurses, technicians, and other ancillary personnel who are committed to improving the health and well being of patients with a variety of gastrointestinal and liver disorders. In addition to addressing specific medical needs, we promote health through preventive care, cancer screening, and by providing education, allowing the patient to better understand their disease process.

Donald P. Kotler, MD
Donald P. Kotler received his BA in 1969 from Rutgers College and his MD from Albert Einstein College of Medicine in 1973. After completing his training in Internal Medicine and Gastroenterology at Bronx Municipal Hospital Center and the University of Pennsylvania, respectively, he joined the Division of Gastroenterology in the Department of Medicine at St. Luke's-Roosevelt Hospital Center in 1979. His laboratory facilities include a clinical area for inpatient or outaptient human studies, a clinical investigative area for measuring body composition and exercise capabilities, and basic science laboratories for cell culture, for routine laboratory analyses, histology, immunoassay, and moleular techniques. Cooperation with the Body Composition Unit, Weight Control Unit, Obesity Research Center, and the Microbiology Laboratory provides increased sophistication for many of the measures. Dr. Kotler has focused on four general areas of research and his laboratory has performed exclusively human studies for about the past 10 years. He has studied the nutritional consequences of HIV infection for the past 15 years, and provided the original description of the cachectic process. Studies include body composition measurements, adverse consequences of malnutrition in HIV infection, pathogenic mechanisms underlying the development of malnutrition, optimization of clinical evaluation, and nutritional support. Nutrition consequences of other chronic disease processes also are being studied in the laboratory, and involve two general areas. Specific investigations involving nutritional aspects of chronic hepatitis C infection, end stage renal disease, and congestive heart failure are ongoing, or in development. The effect of nutrition upon immune function also is being examined in the laboratory.

  • Introduction
    Growth assessment is the single measurement that best defines the health and nutritional status of children, because disturbances in health and nutrition, regardless of their etiology, invariably affect child growth. Health and nutrition problems during childhood are the result of a wide range of factors, most of which particularly in underprivileged populations relate to unsatisfactory food intake or severe and repeated infections, or a combination of the two. These conditions, in turn, are closely linked to the general standard of living and whether a population is able to meet its basic needs such as food, housing, and health care. Growth assessment thus serves as a means for evaluating the health and nutritional status of children, just as it also provides an indirect measurement of the quality of life of an entire population. Of the various anthropometric indices that can be used to assess child growth status, the following provide a comprehensive description: height-for-age portrays performance in terms of linear growth, and essentially measures long-term growth faltering; weight-for-height reflects body proportion, or the harmony of growth, and is particularly sensitive to acute growth disturbances; and weight-for-age represents a convenient synthesis of both linear growth and body proportion

    The Nutrition Screening Initiative (NSI) is a broad, multi-disciplinary effort led by partner organizations the American Academy of Family Physicians and American Dietetic Association and a diverse coalition of more than 25 national health, aging, and medical organizations. Founded in 1989, the goal of NSI is to promote the integration of nutrition screening and intervention into health care for older adults. NSI considers all aspects of health care for older adults as an opportunity to enhance cost-effective nutrition care. To this end, NSI formed alliances with professionals in nutrition, dentistry, pharmacology, mental health, managed care, home care, and community social services. NSI uses a multi-faceted strategy that includes research, professional education, consumer outreach, and policy.     The disease The cause Scope of the problem Interventions ... References Malnutrition is a major health problem, especially in developing countries. Water supply, sanitation and hygiene, given their direct impact on infectious disease, especially diarrhoea, are important for preventing malnutrition. Both malnutrition and inadequate water supply and sanitation are linked to poverty. The impact of repeated or persistent diarrhoea on nutrition-related poverty and the effect of malnutrition on susceptibility to infectious diarrhoea are reinforcing elements of the same vicious circle, especially amongst children in developing countries.
    The disease
    People are malnourished if they are unable to utilize fully the food they eat, for example due to diarrhoea or other illnesses (secondary malnutrition), if they consume too many calories (overnutrition), or if their diet does not provide adequate calories and protein for growth and maintenance (undernutrition or protein-energy malnutrition). Malnutrition in all its forms increases the risk of disease and early death. Protein-energy malnutrition, for example, plays a major role in half of all under-five deaths each year in developing countries (WHO 2000). Severe forms of malnutrition include marasmus (chronic wasting of fat, muscle and other tissues); cretinism and irreversible brain damage due to iodine deficiency; and blindness and increased risk of infection and death from vitamin A deficiency.
