Madelung's Disease
Lanois-Bensaude Syndrome
MSL, Multiple Symmetrical Lipomatosis, also known as Benign Symmetric Lipomatosis or Madelung's Disease and Lanois-Bensaude Syndrome is a metabolic condition characterized by the growth of fatty masses around the face, back of the head, neck, upper arms, abdomen, back and upper leg in a very specific pattern or distribution. Unlike the usual lipoma, these benign, fatty masses are not enclosed within a membranous capsule with very distinct boundaries. It is because of this characteristic, as well as the absolute symmetry in their occurrence that the condition is often dismissed as simple obesity. There are, however, other systemic manifestations which are part of this syndrome, but they tend to be treated as discrete medical problems. This often leads to simultaneous medical investigations and possible treatment by a wide variety of specialists. Quite often, lab tests fall within normal ranges. This leads to the frustrating and demoralizing situation of feeling ill and weak, with no identifiable cause. In addition there is a steady increase in weight, despite all attempts at dieting, which often leads to the judgment of "a non compliant patient." This, and the frequent reference to psychological explanations leads to an escalating sense of fear and stress which only worsens the condition.

Familial Multiple Lipomas
This is a condition in which groups of fat cells in the subcutaneous tissues begin to grow spontaneously and produce multiple fatty lumps underneath the skin. These are benign fatty tumors or lipomas. This condition is quite common and is frequently genetic in origin, due to a mutation of the mitochondrial DNA. The presentation of this disease is variable. Most patients have multiple small fatty subcutaneous nodules that are painless. The number of nodules may vary from very few to many. They are mostly located on the arms, forearms, legs and trunk but they can occur in the neck, face and buttocks. In some patients, if neglected, these fatty lumps can grow to a large size and can make the patient appear grotesque. While these lumps are usually painless, in some patients they are painful and if large, they are subcategorized into an entity called . This disease was first described by Dr. F. X. Dercum in 1888, when he found a patient with large painful subcutaneous tumors. The correct medical term is called Adiposis Dolorosa. In this aspect of familial multiple lipomatosis, the fatty lumps are painful. The degree of pain varies from being mild to severe. The cause of the pain is unknown.

L ipomas are slow-growing, nearly always benign, adipose tumors that are most often found in the subcutaneous tissues. Most lipomas are asymptomatic, can be diagnosed with clinical examination (Table 1) and do not require treatment. These tumors may also be found in deeper tissues such as the intermuscular septa, the abdominal organs, the oral cavity, the internal auditory canal, the cerebellopontine angle and the thorax. Lipomas have been identified in all age groups but usually first appear between 40 and 60 years of age. Congenital lipomas have been observed in children. Some lipomas are believed to have developed following blunt trauma.

Determining the underlying causes of and differences between these syndromes will likely reveal insights into treatment possibilities for HIV fat redistribution.
Two separate research groups at the Lipodystrophy Workshop indicated that HIV fat redistribution and associated metabolic changes resemble two other medical syndromes. The first is a rare, congenital (genetic, sometimes present at birth, also called familial), while the other is a syndrome associated with early heart attacks. In his oral presentation, Abhimanu Garg, MD, from the University of Texas Southwestern Medical Center at Dallas, presented information about different types of non-HIV liposdystrophy syndromes that are established in the medical literature. Almost all of these syndromes are rare and patients with them are often only diagnosed by a medical university-based endocrinologist ("gland specialist") like Dr. Garg. These various syndromes are either familial or acquired at a time after birth. Many of them have some overlapping features with what has been observed in HIV-related fat redistribution (HFR).

Abstract
Multiple symmetrical lipomatosis (Madelung's disease) is a rare disease of undetermined cause characterized by symmetrical deposits of non-encapsulated fat on the suboccipital area, cervical area, shoulders and trunk. The patients are usually middle-aged male alcoholics. The treatment is palliative surgical removal of excess fat from the neck and paracervical regions or any other lesion sites. Oriental report about multiple symmetrical lipomatosis (MSL) is very rare. Here a case of multiple symmetrical lipomatosis in a 43year-old man is described. - Discussion: pathophysiology: - it is caused by an absence or underdevelopment of ulnar portion of growth plate of the radius, so that it fails to contribute to linear growth of the corresponding border of the radial diaphysis; - exact cause of this localized growth disturbance has not been determined, but hemiatrophy of the epiphysis of the distal radius, abnormal muscle insertions, and dysgenesis of vasuclar supply to epiphysis have been suggested; - involved radial and dorsal portions of the growth plate continue to grow; - faster growing, newly formed bone bends toward area of slower growth, causing the articular surface of distal radius to slant in palmar and ulnar direction; - associated conditions: - Madelung's dyschondrosteosis: - a type of mesomelic dwarfism should be suspected if condition is bilateral or if there is positive family history, short stature, or mesomelia; - diff dx: multiple exostoses Ollier's disease multiple epiphyseal dysplasia - Clinical Manifestations: - characterized by insidious onset of pain in one wrist, then in other, and increasing prominence of the dorsal ulnar head and bowing of distal radius; - pain from radioulnar subluxation or radiolunate impingement usually becomes less severe at maturity; - pain and mild deformity can be treated with distal ulnar resection; - in presence of normal distal ulna - thus making wrist appear subluxed; - it is bilateral in two thirds of patients; - deformity progresses until growth plate of the distal radius closes; - wrist motion, particularly extension and supination, is limited; - pain usually subsides at maturity, but when the deformity becomes stabilized, the incongruity of the joint surfaces in the wrist may lead to the recurrence of painful symptoms; - although Madelung's deformity is considered a congenital deformity, it does not become manifest until late childhood or early adolescence;
