The Christmas of 1995 will be one which I shall never forget. With all the normal hustle and bustle of pre-Christmas organizing we sometimes ignore what's going on inside our own bodies. Then comes the stark reality that something is wrong. That is what happened to me and I hope it never happens to you. In these next few pages I shall tell you about an eye disease that I developed and how I came to grips with it. I spent the better part of a month scouring the Internet for information relating to AMD and CNV in hopes of understanding what my future held. I have included numerous links to other AMD and Ophthalmology pages and I'll show you some really gross pictures of my own eyes if you like. If you know what to watch out for, you might be able to receive treatment before its too late. Knowledge is a powerful drug that soothes the soul but only if taken in the right quantity and used with the right attitude.

Executive Summary
The Senate Appropriations Committee, in its report accompanying the fiscal year 1997 Appropriations Bill, has requested a report from the National Eye Institute (NEI). The Committee has urged the NEI to "accelerate and encourage research in age-related macular degeneration (AMD)," and has asked NEI to provide a status report on research and prospects for the future. AMD is a common eye disease of the macula, a tiny area in the retina that helps produce sharp, central vision required for "straight ahead" activities such as reading, sewing, and driving. A person with AMD loses this clear, central vision; in some cases, vision loss is rapid and dramatic. AMD is a leading cause of severe visual impairment and blindness in the United States. It is estimated that AMD already causes visual impairment in approximately 1.7 million of the 34 million Americans over age 65.

Discovery of the gene responsible for a recessive, early-onset maculopathy has focused attention on a biomolecular pathway in which subtle metabolic flaws become toxic to irreplaceable retinal cells. The pathway's identification points toward specific ways of intervening. Among patients with age-related macular degeneration, carriers of single mutations in the same gene are proving to be remarkably common.
Dr. Lewis is Professor of Ophthalmology, Pediatrics, Medicine, and Molecular and Human Genetics and Dr. Lupski is Cullen Professor of Molecular and Human Genetics, Professor of Pediatrics, and Director, Medical Scientist Training Program, Baylor College of Medicine, Houston. In the United States alone, age-related macular degeneration (AMD) affects at least 11 million people. Of the four major causes of vision loss in persons over age 50, it accounts for more cases than the other threecataract, glaucoma, and diabetic retinopathycombined. Apart from laser photocoagulation of the subretinal neovascularization characteristic of the disease's exudative, or wet, form, there is no known intervention. As long as the pathogenesis remains mysterious, a specific treatment seems unlikely to emerge. Our own efforts to trace the pathogenesis have been modeled on an approach now achieving success in complex human disorders ranging from breast and colon cancer to neurodegenerative illnesses such as Alzheimer's disease. Each disorder is scattered throughout the population, often with age-related occurrence. Each has, however, a clinical analogue with a similar or even indistinguishable phenotype except for an earlier onset and occurrence only in certain families, within which it exhibits an inheritance pattern suggestive of a single-gene disease. In such families, modern genetic research techniques may identify a disease-related gene. Identification of the protein that the gene encodes may then give access to upstream and downstream molecules delineating the biologic pathways in which the protein is involvedpathways whose malfunction produces the inherited disease and that may hold at least part of the explanation for the more common, apparently uninherited cases as well.

Overview Age-related macular degeneration is a chronic eye disease that occurs when tissue in the macula, the part of your retina that's responsible for central vision, deteriorates. The retina is the layer of tissue on the inside back wall of your eyeball. Degeneration of the macula results in blurred central vision or a blind spot in the center of your visual field. The first sign of macular degeneration may be a need for more light when you do close-up work. Fine newsprint may become harder to read and street signs more difficult to recognize. Eventually you may notice that when you're looking at an object, what should be a smooth, straight line appears distorted or crooked. Gray or blank spots may mask the center of your visual field. The condition may progress rapidly, leading to severe vision loss in one or both eyes.

What is macular degeneration? People with age-related macular degeneration lose their central vision because of damage to the macula, an area at the back of the eye that allows us to see fine details. Although central vision blurs or darkens, side vision is not affected. Eventually, activities like reading and driving become impossible. Macular degeneration is the leading cause of vision loss in seniors but usually does not result in total blindness. Most people with macular degeneration retain enough useful vision to live independently. What are dry stage and wet stage macular degeneration? In dry stage macular degeneration, the most common form of the disease, macular tissues thin and slowly lose function. Wet macular degeneration is caused by abnormal blood vessels that grow behind the macula. These blood vessels frequently leak, causing scar tissue to form. The wet stage of the disease is less common but usually more damaging than the dry stage. Left untreated, wet macular degeneration may rapidly worsen. What is macular degeneration?
Macular degeneration is a disease that affects central vision and leads to loss of vision. Although there are forms of macular degeneration that strike young people, the condition occurs most commonly in people who are over 60 years of age. This disorder is thus called age-related macular degeneration (AMD). Age-related macular degeneration was formerly called "senile macular degeneration." This term was misinterpreted by many senior citizens as an unflattering reference and it has been discarded. "Senile" has been replaced by "age-related." Does age-related macular degeneration cause blindness?
As a general rule, no. Because only the center of a person's vision is usually affected, blindness rarely occurs from the disease. However, injury to the macula in the center of the retina can destroy the ability to see straight ahead clearly. Therefore, AMD can sometimes make it difficult to read, drive, or perform other daily activities that require fine central vision. What is the macula?

Age-related macular degeneration is the leading cause of severe vision loss among the elderly. In this condition, central vision is lost, but peripheral vision almost always remains intact. Affected persons rarely require canes or guide dogs. The diagnosis of age-related macular degeneration is based on symptoms and ophthalmoscopic findings, and the disease can be classified into atrophic and exudative forms. The two currently proven treatments are laser photocoagulation and photodynamic therapy, but these measures are effective in only a small fraction of eyes with the exudative form of macular degeneration. Vision rehabilitation can help patients maximize their remaining vision and adapt so that they can perform activities of daily living. Families need encouragement in providing support and helping patients adjust to being partially sighted. (Am Fam Physician 2000;61:3035-42.) E lderly persons are concerned about losing independence and mobility. Studies have shown that loss of central visual acuity leads to a reduction of daily activities and mobility in the elderly.

DESCRIPTION: Hereditary and untreatable group of diseases which affect the macular area of the retina. When the disease affects the choriocappilaris, as in central areolar choroidal sclerosis, there is gradual loss of central vision in middle life. Genetically, this type of macular degeneration is autosomal dominant or recessive. Other types of macular degeneration affect the pigment epithelium. Stargardt-Behr disease is transmitted genetically as an autosomal recessive factor; it results in gradual deterioration of the pigment epithelium, which begins at different ages and progresses at different rates; rate and degree of visual loss varies from family to family. In Best's vitelliform macular degeneration (an autosomal dominant genetic pattern), changes are confined to the macula. Initially, it resembles a poached egg "sunny side up;" later, the "yolk" appears scrambled, and central vision is seriously decreased. (see also Scotomas TREATMENT: Untreatable.
