  • What is macular degeneration? Macular degeneration is the physical disturbance of the center of the retina called the macula. The macula, which is about the size of the capital letter "O" in this sentence, is the part of the retina which is capable of our most acute and detailed vision. We use the macula for reading, driving, recognizing faces, watching television, and fine work. Macular degeneration is the leading cause of legal blindness in people over age 55. (Legal blindness means that a person can see 20/200 or less with eyeglasses.) Even with a loss of central vision, however, color vision and peripheral vision may remain clear. Vision loss usually occurs gradually and typically affects both eyes at different rates.

    Macular Degeneration
    Overview Age-related macular degeneration (ARMD) is a degenerative condition of the macula (the central retina It is the most common cause of vision loss in the United States in those 50 or older, and its prevalence increases with age. AMD is caused by hardening of the arteries that nourish the retina. This deprives the sensitive retinal tissue of oxygen and nutrients that it needs to function and thrive. As a result, the central vision deteriorates. This example demonstrates what a patient with advanced macular degeneration sees. Macular degeneration varies widely in severity. In the worst cases, it causes a complete loss of central vision, making reading or driving impossible. For others, it may only cause slight distortion. Fortunately, macular degeneration does not cause total blindness since it does not affect the peripheral vision. What is the difference between wet and dry macular degeneration?

    By Linda Kaspari Cigarette smoking reduces levels of plasma antioxidant, a substance in the blood stream, which protects retinal cells. Smoking causes the protective layer between the retina and blood vessels to erode, resulting in poor circulation, irritation and scarring. Smoking is damaging to the blood supply. Smoking exposes the body to free radicals which causes cellular damage and can cause poor circulation of nutrients to the retina and lens. Carotenoid levels and vitamin C blood concentrations are substantially reduced. Smoking oxidizes cholesterol, causing fat to build up in the arteries. This leads to heart attack, stroke, and probably macular degeneration.
    Cataracts
    Researchers think smoking contributes to the incidence of some types of cataracts. "What makes us suspect this is the consistency of studies all over the world," says Robert Sperduto, M.D., chief, epidemiology branch of the National Eye Institute. "If there's one thing people can do to improve their general health, including the health of their eyes, it is to stop smoking," says Dr. Sperduto. Carbon monoxide from cigarette smoke creates a hypoxic effect, it is clear that hypoxia will degrade night vision. (Hypoxia is a state of oxygen deficiency in the body which is sufficient to cause an impairment of function. The inadequate oxygen transport or the inability of the tissues to use oxygen causes hypoxia.)

    Researching the Cause of Macular Degeneration
    The causes of age-related macular degeneration (AMD) are still not understood, but researchers at the Medical College of Wisconsin continue to seek answers through research at the cellular level. Age-related macular degeneration is the leading cause of vision loss for individuals over age 60, affecting 13 million Americans. Macular degeneration affects light-sensitive photoreceptor cells in the macula, the center of the retina at the back of the eye. While it doesn't cause total blindness, the disease destroys central vision so reading, watching TV and driving are impossible. There is no cure, and treatments are very limited. Like most age-related diseases, AMD is likely to have many causes, including both genetic and "toll of years" components. A gene or several genes may predispose the retina to cellular changes resulting in macular degeneration. But whether or not a person with a genetic predisposition gets the disease also depends upon how the aging process itself affects cells.
