I am on a mailing list with other folks with secondary and primary lymphedema. We have reports from some people that low carb diets make them feel better with their disease. As you probbly know,affected limbs go up and down,and with massage and wrapping we try to minimize the swelling as much as we can. Can you think of any reason our unscientific but widely held view on carbs might have some science behind it? Dean H.

Mr. H., This is a particularly vexing question as is the problem of lymphedema. I have had a few patients with both primary and secondary disease and, like your question, it is a difficult problem. I have not, however, been able to find any research that proves a low carbohydrate diet is helpful. It may be that because some lymphedema is a problem with the difference in "oncotic" pressure between the veins and tissues that some have come to this conclusion. Because the oncotic pressure is controlled mostly by proteins one might consider that eating a diet with less carbohydrates (thus relatively more proteins) would help. Unfortunately the body doesn't really process the proteins or carbohydrates in this way.

Introduction to Lymphedema
The lymphatic system is a network of lymph nodes and lymphatic vessels. Its primary function is to transport and filter lymph fluid throughout the body. This fluid contains mostly water and protein and is a vital component in maintaining the body's fluid levels. Lymphedema is a medical condition that may occur when regional lymph nodes are compromised due to surgery, radiation or injury, or if there is a congenital deformity within the lymphatic system. This results in an abnormal accumulation of fluid in the surrounding tissue and eventually in the entire limb. Lymphedema can be manifested in two ways:

How is Lymphedema Treated?
Treatment consists of a short- and long-term technique called Combined Decongestive Therapy or CDT. CDT is recognized as the most effective and least invasive approach to the treatment of lymphedma. It consists of the following steps:

What happened to change the quality of my edema The normal swelling of RSD had turned into lymphedema. This article will try to explain the difference between lymphedema and the edema of RSD, as well as look at the problems cause by lymphedema, treatments and some of the things that all persons living with RSD can do to prevent lymphedema.
THE LYMPH SYSTEM The lymphatic system is similar to the blood system because it has vessels, in fact the lymph system has as many vessels as the circulatory system.. Every tissue has many tiny lymph vessels which gradually join together into bigger ones Lymph vessels contain lymph fluid, which is clear and cannot be seen (unless a suitable dye is injected). Lymph fluid is drained away from each body part by lymphatic vessels.
Swelling, (Edema)
When tissue is injured or inflamed excess fluid enters the tissues from damaged blood vessels within these injured tissues. If the veins cannot remove all of this fluid, the part swells (edema). However this swelling is usually only temporary, because the tissues heal and the blood vessels no longer leak excessively. Swelling is one of the symptoms of RSD. Early in the course of the disease this inflammatory process causes edema. The swelling in RSD may exist far longer than it would take normal tissue to heal because RSD: 1) prevents healing; 2)causes constant inflammation; 3) may cause dilation of the arteries which will cause more fluid to leak, and 4)may cause the veins to contract, which also prevent the normal removal of non-protein fluid from tissues. It is pretty easy to see that the edema of RSD may last for long periods of time.

Lymphedema is swelling of a body part (face, neck, trunk, genitals, arms, legs, etc.) caused by an abnormal accumulation of lymph fluid. Lymphedema is caused by an imbalance between the normal amount of lymph fluid and reduced capacity that the vessels are able to transport. This results in an accumulation of protein-rich fluid. Primary vs. secondary lymphedema
Primary lymphedema is due to congenital malformations of the lymphatic system. It can be present at birth or develop later in life. It is more common in females and usually appears in the legs. Secondary lymphedema is caused by an inadequate lymphatic drainage system, generally due to constriction or obliteration resulting from surgery, radiation, trauma or infections. Secondary lymphedema may appear 10 to 20 years or more after surgery or radiation. The risk of developing lymphedema is always present. Symptoms of lymphedema

Lymphedema is a swelling of the arm or leg and/or other body part caused by an abnormal build-up of protein and excess water in the tissue space. Not all swelling is lymphedema. Your physician can help determine this. What causes lymphedema?
Lymphedema is categorized as primary or secondary, as well as having an acute or chronic form.
  • Primary (Hereditary) Lymphedema is usually due to an abnormality of the lymphatic vessels. It can be present at birth (Congenital Lymphedema) or occur in adolescence (Lymphedema Praecox) or in adulthood (Lymphedema Tarda) after age 35. The disorder is slowly progressive, tends to affect mainly females, and the legs are the area of the body more often affected. Secondary (Acquired) Lymphedema iis generally caused by obstruction (blockage) or interruption of the lymphatic system. Obstructive causes include infection or cancer. The usual causes of interruption are surgical removal of lymph nodes and vessels, scarring due to radiation therapy, or traumatic injury to the lymphatic system. Common cancers associated with blockage from tumor or lymph node removal include breast, skin (melanoma), gynecologic (ovarian/cervical/uterine/vulvar), lymphoma, prostate, testicular, bladder and colon cancer.

Whether you have a lumpectomy or mastectomy, your surgeon will probably remove some of the lymph nodes under your arm to see if the cancer has spread outside the breast. Both the surgery and radiation therapy can change the normal drainage pattern which can result in a swelling of the arm called lymphedema. The problem can develop right after surgery or months to years later. With proper education and care, lymphedema can be avoided, or, if it develops, kept well under control.

    My child's feet suddenly swelled. Does this mean she has lymphedema? Many things, including abdominal masses, venous problems, and rheumatologic disorders, can cause immediate or rapid swelling of the feet. An appropriate medical exam and evaluation is necessary before a physician can rule out competing causes and begin considering a diagnosis of lymphedema.
    How is congenital lymphedema diagnosed in a child? First, our medical staff takes your child's medical history and performs a physical exam. Depending on the results, the staff may schedule your child for a lymphoscintigraphy, an imaging test that allows physicians to view your child's lymphatic vessels. During a lymphoscintigraphy, a radiologist will inject a small amount of a radioactive tracer in your child's foot or hand, then use a special camera to view your child's lymphatic vessels. The radiologist can then make an accurate diagnosis, classifying your child's lymphedema by type - hyperplasia, hypoplasia, or absence of lymphatics. In some cases, the medical staff may order additional X-rays and imaging tests for further clarification.
    My child has a rheumatologic disorder that periodically causes her limbs to swell. Yet she also experiences limb swelling that does not seem related to her rheumatologic flare-ups. Could she have congenital lymphedema?

    Stages of Lymphedema
    STAGES OF LYMPHEDEMA
    Stage 1 (mild) - Upon waking in the morning the limb or affected area is almost a normal size.The tissue is still in a "pitting stage" ( when pressed by a finger the area indents and holds the indentation).
    Stage 2 (moderate) - The tissue is "non-pitting" (when pressed by a finger the tissue bounces back without leaving an indentation). The tissue has a spongy consistency.
    Stage 3 (severe) - The tissue at this stage is hard (fibrotic) and will be unresponsive to the touch. The swelling is irreversible and the limb is very large and swollen. Infections are possible at any stage of lymphedema but occurrence becomes greater as stages progress. A swollen limb, left untreated, becomes hard (fibrotic) and full of lymph fluid which is high in protein and a perfect medium for bacteria and infections.
    CAUSES OF LYMPHEDEMA
    Secondary and Primary
    Surgical procedures and/or radiation therapy in the treatment of Breast, Gynecological, Head , Neck, Prostate or Testicular, Bladder, Colon Cancers or Melanomas.( Due to treatment, there is removal of, or damage to the lymph nodes or vessels so that lymph fluid is no longer able to be transported properly in the body).
