VOL 105 / NO 6 / MAY 15, 1999 / POSTGRADUATE MEDICINE Lymph nodes are part of the body's special circulatory system for moving and filtering an important fluid called lymph. This clear fluid comes from the blood and carries disease-fighting cells throughout the body. Lymph also contains proteins and salts removed from the tissues. When the lymph nodes are removed, injured, or blocked, fluid builds up in the tissues. This buildupand the swelling it causesis called lymphedema. Lymphedema can be hereditary or caused by infection, but most often it occurs after surgery or radiation therapy for cancer. About 20% of women who have had lymph nodes removed because of breast cancer have symptoms of lymphedema. More than 400,000 American women are living with this problem. Hereditary lymphedema can affect one or all limbs and sometimes other parts of the body. It may be present at birth, or it might appear later in life. The cause is often unknown, but abnormalities of the lymph system seem to be responsible for some cases. Acquired lymphedema occurs after surgery, radiation therapy, infection, or injury. Surgical operations that require removal of lymph nodes greatly increase the risk for lymphedema. Swelling can occur immediately or weeks, months, or even many years after such surgery.

LYMPHEDEMA Lymphedema is a long-term swelling condition that can occur in the adjacent hand, arm, and/or the entire quadrant (chest, side and back) as a result of breast cancer surgery and radiotherapy. The lymphatic system drains excess protein and water from the blood and natural and foreign substances from the tissues. This lymph fluid is filtered by the lymph glands and then returned to the blood system. When the lymphatic system has been damaged or blocked, lymph fluid builds up and causes mild to severe swelling in the hand or arm. This condition reduces oxygen in the tissues, slows healing and creates an ideal environment for infection and serious risk to your general health. Lymphedema can occur at the time of surgery and treatment, or months or years later. It is important that women diagnosed with breast cancer are informed about the causes, symptoms, prevention and treatment of lymphedema.

Have you noticed women with an oversized arm? Or perhaps you have noticed someone wearing what looks like an Ace bandage from their hand to their shoulder. That person is most likely suffering from lymphedema, a severely underaddressed issue relevant to women's health. It is an "orphan disease" with no specialists. And the Ace bandage is not what it appears. An Ace bandage is very elastic, while the lymphedema bandaging has very little stretch in it, but acts like a cast. When the muscle pushes, the bandage holds the skin in place, so that the tissue is squeezed and wrung out, like a sponge. Lymphedema is a chronic swelling, due to inadequate lymph drainage. You are at risk for developing lymphedema if you have had lymph nodes removed and/or radiation treatment following breast cancer.Jeanne Petrek, MD, director of the Evelyn Lauder Breast Center at Memorial Sloan-Kettering Cancer Center in NYC headed a study released in September 21, 2001 found that almost half of women who had surgery for breast cancer and removal of lymph nodes devlop the condition within their lifetime. Lymphedema is not just an issue for women who have had breast cancer surgery, it is also seen in all kinds of pelvic tumors in both men and women. The number of men who suffer with lymphedema in the legs is high. It is also becoming apparent that plastic surgery patients, who have had liposuction, are at risk from the disturbance in the lymph tissue following the intrusion of the cannula.

Swollen Legs (Lymphedema) Swelling of the legs may be caused by many diseases including heart failure, kidney disease, venous disease and disease of the lymphatics. This discussion will cover lymphedema which is an accumulation of fluid and protein that causes swelling in the arms and legs. This can occur when lymphatic vessels are impaired. Lymphedema can develop from idiopathic or unknown causes a situation known as primary lymphedema. This may start at birth (congenital lymphedema), in the teens (lymphedema praecox) or late in life (lymphedema tarda). Breast, abdominal or groin surgery may result in secondary lymphedema because of disruption of the lymph vessels or removal of lymph nodes from the surgical procedure. Radiation therapy, limb trauma and repeated or chronic infection may also affect the lymphatics.
If a patient notices persistent swelling of a limb and the skin becomes hard and less elastic, the cause may be lymphedema. Patients who have lymphedema are at a high risk for infections.
Depending on the severity of the lymphedema, a treatment plan will be developed based on decongestive therapy methods which consist of manual lymphatic drainage, bandaging, proper skin care, sequential gradient pumps and compression garments. Surgery is rarely used.
