Clinical:
Most lymphangiomas are present at birth and are usually detected within the first 2 years of life. They usually occur in the head, neck, and axilla. Approximately 10% of cervical lymphangiomas extend into the mediastinum. Fewer than 1% of lymphangiomas are limited only to the mediastinum. Primary mediastinal lymphangioma is rare usually presents as a solitary mediastinal mass. The lesion is most frequently seen in men. Because the lesion grows slowly and has a soft consistency, patients are usually asymptomatic. The majority of lesions are cystic and are located in the superior mediastinum adjacent to the right lateral wall of the trachea. Because of their insinuating nature, complete surgical resection may be difficult [2]. Depending on the lesions location differential considerations include a bronchogenic cyst, a foregut duplication cyst, pericardial cyst, thoracic duct cyst, thymic cyst, or cystic teratoma. Histologically lymphangiomas are divided into three groups depending on the size of the lymphatic channels. Capillary (simple) lymphangiomas are composed of small lymphatics; cavernous lymphangiomas of larger lymphantics; and cystic lymphangiomas of large, macroscopic lymphatic spaces. The tumors are usually thin-walled, multilocular, lined by endothelial cells, and contain clear yellow fluid.
X-ray:
On CT the lesion classically appears as a well circumscribed, low density lesion molding to the mediastinal contours and enveloping the great vessels. The most common finding, however, is a rounded, low density mass adjacent to the right lateral wall of the trachea. Increased density or inhomogeneity may be seen secondary to internal hemorrhage. The lesions generally do not enhance after IV contrast administration. Calcification is very rare and the tumor characteristically does not displace the mediastinal structures. On MR imaging, the lesions usually has heterogeneous signla on T1 images, and high signal on T2 images [2].

Email Address: Viewable for logged on visitors ( Log on Age: 14 Years Gender: Female Region-Organ: Abdomen-Spleen Most likely etiology: congenital History: The first presentation of the then 9 year-old girl was based upon unspecific lower abdominal pain. Chance findings in the routine sonographic examination. Initially, the findings were just watched, with follow-ups every quarter year. As the findings were found to increase in size at the age of 13, ultrasound aspiration was done and a short-term drainage was performed. An instillation of a locally acting substance was performed. In the follow-up examination, the structures decreased in size. After 6 months, however, a renewed increase in size was noted and a renewed aspiration was performed. Since the pathological findings remained, a surgical treatment was performed.

Findings: This CT of the abdomen demonstrates a very large multiseptated cystic mass involving the majority of the abdomen. Diagnosis: Mesenteric lymphangioma. Discussion: The most common type of mesenteric or omental cystic structure is the lymphangioma, followed by a nonpancreatic pseudocyst, duplication cyst, mesothelial cyst, and enteric cyst. A lymphangioma is a congenital malformation of lymphatics that occasionally occurs in the mesentery but may also occur in the neck, mediastinum, and axilla. Approximately 65% of these patients present at birth with approximately 90% presenting by 2 years of age. The usual symptoms include abdominal distention and small bowel obstruction. Lymphangiomas are large, thin-walled cystic structures that are usually multiloculated. The fluid may be serous chylous, or occasionally hemorrhagic. Sonographically, lymphangiomas are generally multiseptated anechoic masses which may contain internal echoes if hemorrhagic. CT demonstrates a multiseptated mass of fluid (or rarely fat) attenuation. This mark is produced by a benign growth of the large blood vessels in the skin. It is not present at birth, but will appear as a small spot that grows larger, redder and more raised above the surface. It may increase for the first six months but then begins to decrease in size. It will be very small by the time the child is 5 years old and the majority will disappear completely.
Lymphangioma.
This is a benign proliferation of lymph vessels. Unlike its cousin, the hemangioma, these tumors are rare. However, they are important to recognize because they may mimic many vascular tumors including angiosarcomas and Kaposi's sarcoma. These tumors may occur anywhere but usually present in the head and neck in children and infants. Visceral organs such as the lungs and gastrointestinal tract may also be involved. In the soft tissue, these tumors present as a soft fluctuant mass that varies in size.
