Cystic hygromas (lymphangiomas)
are multiloculated cystic structures that are benign in nature. They form as the result of budding lymphatics and, thus, may occur anywhere in the body, although they are most frequently encountered in the neck (75%) and axilla (20%).
Often the tongue, base of tongue, supraglottic larynx and mediastinum are involved, causing severe respiratory obstruction. . Surgery is the treatment of choice. http://www.emedicine.com/ped/topic536.htm

Quick Review for Patients
Top of This Page Introduction Note: click on underlined words for more detail or photos. The lymphangioma is a benign hamartomatous hyperplasia of lymphatic vessels, with three-fourths of all cases occurring in the head and neck region. While occasional adult-onset cases occur, this tumor is thought to be a developmental malformation of vessels which have poor communication with the normal lymph system. Very large cystic spaces may be seen in lesions proliferating in loose connective tissues and fascial spaces. Diagnosed cases are typically superficial but may extend deeply into underlying connective tissues. Rarely, multiple lesions are seen in infancy and childhood in lymphangiomatosis , the lymphatic counterpart to angiomatosis of blood vessels and a potentially life-threatening disease when visceral involvement occurs.

Other characters Lymphangioma, congenital malformation of the lymphatic system, caused by failure of drainage from primordial lymph sacs into veins, sequestration of lymphatic tissue with failure to unite with the central lymphatic channels, or abnormal budding of lymph vessels with loss of continuity with lymphatic primordia. Four main types are described: Cystic lymphangiomas ( cystic hygroma ) are unilocular or multilocular masses located mainly in the neck (posterior triangle being the most common), mediastinum and axilla. The lesions are hypoechoic on ultrasound unless there has been haemorrhage into them. MRI demonstrates hypointensity or hyperintensity on T1-weighted images depending on the protein content of the fluid and hyperintensity on T2-weighted images. Occasionally, there may be fluid fluid level due to haemorrhage. Cavernous lymphangiomas have mildly dilated lymphatic spaces with intermediate size cyst formation and a tendency to penetrate contiguous structures. These are more often found in the tongue, floor of the mouth and salivary glands. Magnetic resonance imaging demonstrates similar change as for cystic hygroma except that there is a fibrous stromal component that is of low signal intensity on T1- and T2-weighted images. Capillary or simple lymphangiomas are the least common and are found in the epidermis and dermis of proximal limbs.

Other characters Lymphangioma, retroperitoneal, a developmental malformation and not a true neoplasm, though it presents as a retroperitoneal mass. Lymphangiectasia is the dilatation of abnormal lymphatic channels, which have failed to establish normal communication with the rest of the lymphatic system. The dilated lymphatic channels conglomerate and form a unilocular or multilocular cystic mass, known as a lymphangioma. The diagnosis of retroperitoneal lymphangioma is usually made in infancy. It occurs with equal frequency in males and females. Common clinical findings include pain, abdominal distension, fever, fatigue, weight loss and haematuria. A retroperitoneal lymphangioma can be imaged by ultrasound, CT or MRI. It may be elongated in contour and characteristically spans more than one retroperitoneal compartment. Uncomplicated fluid is seen within the mass or locules, which are separated by thin septa. The fluid appears complicated in the presence of haemorrhage or infection. Rarely, calcification and chyle are present.

    http://www.laparoscopy.com/pictures/lap_lymp.html
    surgeon: Mark Pleatman, MD
    Small bowel lymphangioma
The patient was an 8-year old boy suspected of having appendicitis. The tumor of the bowel was exteriorized and resected. The pathology showed acute inflammation in the dilated lymphatic channels.
Lymphangioma of the mesentery is a rare benign cystic tumor of the lymphatics of the bowel.
It arises from developmental defects in the mesenteric where fluid is being collected in small closed spaces.
Most of the times this tumors are multiloculated, have an endothelial lining, and contain a not very clear fluid similar to chyle.
As many others benign tumors of the mesentery it is found peripherally, near the intestine. Malignant tumors such as lymphoangiosarcoma, malignant teratoma, liposarcoma etc., are found near the root of the mesentery.
This type of “benign condition” can have local invasion with mechanical compression of adjacent viscera. Therefore, surgical excision of the tumor including the segment of the bowel involved is the only treatment for this kind of condition.

Background This is a benign proliferation of lymph vessels. Unlike its cousin, the hemangioma, these tumors are rare. However, they are important to recognize because they may mimic many vascular tumors including angiosarcomas and Kaposi's sarcoma. These tumors may occur anywhere but usually present in the head and neck in children and infants. Visceral organs such as the lungs and gastrointestinal tract may also be involved. In the soft tissue, these tumors present as a soft fluctuant mass that varies in size. SYNONYMS Lymphangioma INCIDENCE Rare AGE RANGE-MEDIAN Birth 50-60%
