Objective:
The aim of our study was to demostrate that there is a correlation of intervertebral disk herniation enhacement with the course of the desease process, in patients with acute-onset of radiculopaty secondary to disk herniation.
Methods: Pattients suffering from acute sciatica and lumbar disk herniation diagnosed by Computed Tomography were included in the study, and those with another morfologic causes of sciatica were excluded. We performed a prospective study: patients underwent physical examination and Magnetic Resonanace imaging (all using GD) every four months; and enhacement and type of enhacement, size, location and type of disk herniation were measured in correlation with clinical manifestations measured.
Results: Magnetic Resonanace study demostrate enhacement of the disk herniation in 75 % of 100 patients. Disk herniation size was: 2 cm in 4 % of the cases. 37 % of disk herniations were extruded and 63 % were subligamentous disk herniations. Homogeneous enhacement was noted in 39 % of the disk herniations, heterogeneous enhacement in33 % and peripheral enhacement in 28 %. We present the results of the follow-up along one year in fifty patients.
Conclusion: The purpose of this prospective study was to define imaging criteria like type and pattern of enhacement on Magnetic Resonance correlated with clinical outcomes, with prognostic patterns might allow earlier patient selection for treatment. Disk herniation enhacement on GD-Magnetic studies seems to be a favourite prognostic factor, in correlation with clinical and radiological findings.

Purpose:
To assess the feasibility and efficiency of foraminal or intracanalar steroid injections in cases of sciatica due to disk herniation.
Materials and methods: Between January 1999 and January 2001, 80 patients presenting with typical sciatica, resisting usual medical treatment (four weeks), underwent a prospective protocol of pain treatment. A percutaneous approach was performed using CT guidance under local anesthesia. In foraminal lateral herniation (25 %), the 22 G needle tip was positioned through the soft tissues. In intracanalar herniations (75 %), the needle tip was advanced directly into contact with the posterior part of the root, through the ligamentum flavum (homo-lateral or contra-lateral approach in case of a narrowing lumbar tendency). In all cases the steroid injection (Dexamethasone acetate 20 mg) was rigorously epidural.

The cervical vertebrae are the seven vertebrae that form the upper part of your spine, between the skull and the chest. The thoracic vertebrae are the 12 bones between your neck and your lower back.  Thoracic vertebrae have cup-shaped surfaces called facets, in which the ribs rest and connect to the spine.  These ‘joints’ help the ribs to move up and down during breathing. The lumbar vertebrae are the five largest and strongest of all vertebrae.  They are found in your lower back between the chest and hips.  The strong muscles of the back are attached to the lumbar vertebrae. Your sacrum and coccyx are the bones found at the base of your spine.  The triangular sacrum—made up of five vertebrae fused together—supports the spine and connects it to the pelvis.  Your coccyx, or tailbone, is formed from four fused vertebrae and has little function. The vertebral foramen is the hollow part of the vertebrae where the spinal chord (nerve tissues) attaches to your brain and sends signals all over your body.

  • What Is It? The intervertebral disks are thin, oblong structures in the spine that serve as cushions between the vertebrae (backbones). Each disk is made of a soft gel core surrounded by a tough, fibrous outer shell. This structural pattern allows the disk to be firm enough to maintain the space between the vertebrae, but soft enough to compress when the spine flexes during bending, leaning and turning sideways.
