Tabes dorsalis is a slow degeneration of the nerve cells and nerve fibers that carry sensory information to the brain. The degenerating nerves are in the dorsal columns of the spinal cord (the portion closest to the back of the body) and carry information that help maintain a person’s sense of position. Tabes dorsalis is the result of an untreated syphilis infection. Symptoms may not appear for some decades after the initial infection and include weakness, diminished reflexes, unsteady gait, progressive degeneration of the joints, loss of coordination, episodes of intense pain and disturbed sensation, personality changes, dementia, deafness, visual impairment, and impaired response to light. The disease is more frequent in males than in females. Onset is commonly during mid-life. The incidence of tabes dorsalis is rising, in part due to co-associated HIV infection.

What is ataxia?
There are many types of ataxia, the most common is Friedreich's ataxia. Ataxia is a symptom, not a specific disease or diagnosis. Ataxia means clumsiness, or loss of coordination. Ataxia may affect the fingers and hands, the arms or legs, the body, speech or eye movements. This loss of coordination may be caused by a number of different medical or neurologic conditions; for this reason, it is important that a person with ataxia seek medical attention to determine the underlying cause of the symptom and to get the appropriate treatment.

The essential symptoms of the diseasestansping gait, and swaying with the eyes shut, the occurrence of blindness and of small fixed pupilswere recognized by Romberg (1851), but it was the clinical genius of Duchenne and his masterly description of the symptoms which led to its acceptance as a definite disease (1858), and he named it locomotor ataxia after its most striking symptom, In 1869 Argyll Robertson discovered that the eye-pupil is inactive to light but acts upon accommodation in the great majorit) of cases. Ataxia is a symptom, not a specific disease or diagnosis. Ataxia means clumsiness, or loss of coordination. Ataxia may affect the fingers and hands, the arms or legs, the body, speech or eye movements. This loss of coordination may be caused by a number of different medical or neurologic conditions; for this reason, it is important that a person with ataxia seek medical attention to determine the underlying cause of the symptom and to get the appropriate treatment.
What causes ataxia?
Most often, ataxia is caused by loss of function in the part of the brain which serves as the "coordination center", which is the cerebellum. The cerebellum is located toward the back and lower part of the head. The right side of the cerebellum controls coordination on the right side of the body, and the left side controls coordination on the left. The central part of the cerebellum is involved in coordinating the very complex movements of gait, or walking. Other parts of the cerebellum help to coordinate eye movements, speech and swallowing.
Ataxia may also be caused by dysfunction of the pathways leading into and out of the cerebellum. Information comes into the cerebellum from the spinal cord and other parts of the brain, and signals from the cerebellum go out to the spinal cord and to the brain. Although the cerebellum does not directly control strength ("motor function") or feeling ("sensory function"), the motor and sensory pathways must work properly to provide the correct input into the cerebellum. Thus, a person with impaired strength or sensation may notice clumsiness or poor coordination, and the doctor may say that person has ataxia.

    Acute Locomotor Ataxia
    While locomotor ataxia usually develops slowly from gradually increasing causes in the cell structure of the spinal cord, there are cases which develop more or less abruptly with but few, if any, premonitory evidences. There is a variety due to cellular change in the structure of the brain, which produces disturbances of speech and psychical symptoms in addition to the ataxic phenomena. The commoner form is marked by the presence of various anesthesias and paresthesias without psychical complications. In either case paralysis may be absent or present. I have had under observation for seven or eight years a case which developed in the course of two or three days and at first was rapidly progressive in character. There might have been a psychic element involved in the cause of this case, as there was an operation performed on the rectum without an anesthetic and entirely unexpected and unannounced to the patient. The ataxic symptoms were quite fully developed within twelve hours after the operation and the patient was mentally depressed to an extreme degree. This case as stated progressed rapidly at first but the progress was finally retarded by treatment and the case assumed a chronic form. There are few cases that received a greater variety of treatment than this had, but I am bound to credit the action of the Roberts-Hawley lymph compound with stopping the progress of the disease and adding five or six years to the life of the patient. This preserved intact, also, the condition of the mind of the patient, enabling him to attend to the details of his business.

    Locomotor Ataxia.
    Synonyms Definition Syphilis bears a very important relation to tabes and is undoubtedly the most important of all predisposing causes. There is a history of syphilis in from seventy-five to ninety per cent of all tabetic cases. Thus Erb reports that eighty-nine per cent of three hundred cases in private practice, had a history of syphilis, while Mobius believes that tabes never exists without syphilis.
    Taylor Among the more common exciting causes may be mentioned sexual excesses, great physical exertion, and exposure to wet and cold. It is more common in cities than in the country. Pathology In some cases, trophic changes are seen in the bones, especially their articular surfaces. Symptoms Pains They last only for a second or two and may be associated with a burning sensation. Occasionally herpes develops at the seat of pain. These attacks occur irregularly, and are apt to follow excesses, cold, and damp weather. In a few cases pain is absent. Paresthesia The girdle pain, or sense of constriction about some part of the trunk, is not uncommon.
