Summary: Nonalcoholic Steatohepatitis ( NASH) is Leptin an Etiological factor (Phase 1)
This study comprises of one day of testing that involves baseline blood work, imaging studies that help to determine body fat distribution and a test that lasts three hours to determine how effective insulin is working in the cells. We will perform these studies on 50 patients with known fat accumulation and scarring. We will use this information to determine if we can develop new therapies to reverse or slow down fat accumulation and scarring in the liver. Entry Criteria:
  • non-pregnant
  • non-diabetic
  • all ethnicities and genders
  • presence of nonalcoholic steatohepatitis (NASH) on liver biopsy
  • absence of chronic liver or kidney or heart failure
Exclusions:
  • Presence of advanced liver disease (as evidenced by abnormal synthetic function, abnormal PT or albumin)
  • Pregnancy or lactation
  • Inability to give informed consent
  • Active infection (may be transient)

Abstract:
Directed probabilistic graphs, such as Bayesian networks, are useful tools for coherent representation of and reasoning with uncertain knowledge. They are based on the sound foundations of probability theory and they readily combine available statistics with expert judgment. When extended with decision options and measures of desirability of outcomes (utilities), they support decision making. This paper describes our work in progress on a probabilistic causal model for diagnosis of liver disorders that we plan to apply in both clinical practice and medical training. The model, and especially its numerical parameters, is based on patient records at the Gastroenterological Clinic of the Institute of Food and Feeding in Warsaw, collected over the period of several years. We present the model and report initial results of our diagnostic performance tests. Keywords: Causal models, Bayesian networks, medical diagnosis.

Fatty Liver
Fatty liver is just what its name suggests: the build-up of fat in the liver cells. Although this is not a normal condition, fat in the liver usually causes no damage by itself. However, on some occasions it can be a sign that other more harmful conditions are at work. Fatty liver may be associated with or may lead to inflammation of the liver. This can cause scarring and hardening of the liver. When scarring becomes extensive, it is called cirrhosis, and this is a very serious condition. A physical examination that reveals an enlarged liver without any other symptoms suggests fatty liver. The diagnosis may be confirmed by performing a liver biopsy, in which a long hollow needle is used to obtain a small tissue sample for examination under a microscope. The mere presence of excessive fat in the liver is not a serious problem. Treatment aims at eliminating the cause or treating the underlying disorder. Repeated liver injury from toxic substances such as alcohol may eventually progress from fatty liver to cirrhosis. CAUSES: Obesity, Diabetes Mellitus, high blood triglycerides, heavy use of alcohol, Vitamin A toxicity, drugs, and possibly other unknown causes.

What are autoimmune liver disorders?
An autoimmune disorder is any reaction or attack of a person's immune system against its own organs and tissues. In the liver, the immune system can destroy liver cells and damage bile ducts. Chronic active hepatitis can be caused by an autoimmune disorder.
What are metabolic liver disorders?
Two main metabolic disorders affect the liver:
  • hemochromatosis (Also called iron overload disease.) - characterized by the absorption of too much iron from food. Instead of secreting the excess iron, the iron is stored throughout the body, including the liver and pancreas. The excess iron can damage these organs. Hemochromatosis is a hereditary disease that can lead to liver disease, liver failure, liver cancer, heart disease, and diabetes. Wilson's disease - characterized by the retention of too much copper in the liver. Instead of releasing the copper into the bile, the liver retains the copper. Eventually, the damaged liver releases copper into the bloodstream. This hereditary disease can cause damage to the kidneys, brain, and eyes, and can lead to severe brain damage, liver failure, and death.

Dear JAnet,
Amanda is correct that the gall bladder is on the right side and does not usually cause left abdominal pain. Moreover, you had an ultrasound stusy that I assume showed that the gall bladder was normal. The issue for your doctors is to define the cause for the elevated liver tests. A 30 lb weight loss is quite substantial. If you were overwieght previously, then this weight loss is good. If however, you were previously average weight, then this is another problem requiring evaluation. I would suggest that you discuss with your physicians the possibility of focusing on the liver problem. Anelevated anti mitochondrial antibody (if appropriate titer) can indicate the future development of primary biliary cirrhosis. This information is presented for educational purposes only. A.1 BUPA Liver Disorders
The BUPA Liver Disorders dataset was created by BUPA Medical Research Ltd. and it was donated to the Irvine collection by Richard Forsyth. Table 15 shows the test costs for the BUPA Liver Disorders dataset. The tests in group A are blood tests that are thought to be sensitive to liver disorders that might arise from excessive alcohol consumption. These tests share the common cost of $2.10 for collecting blood. The target concept was defined using the sixth column: Class was defined as "drinks < 3" and class 1 was defined as "drinks 3". Table 16 shows the general form of the classification cost matrix that was used in the experiments in Section 4 . For most of the experiments, the classification error cost equals the positive error cost equals the negative error cost. The exception is in Section 4.2.3 , for the experiments with complex classification cost matrices. The terms "positive error cost" and "negative error cost" are explained in Section 4.2.3

Diagnostic Procedures for Digestive and Liver Problems
To reach a diagnosis for digestive and liver problems, a thorough and accurate medical history must be taken by the physician, noting the symptoms your child has experienced and any other pertinent information. A physical examination is also done to help assess the problem more completely. Some patients need to undergo a more extensive diagnostic evaluation, which may include laboratory tests, imaging tests, or endoscopic procedures. These tests may include any, or all, of the following: Laboratory tests:
  • albumin level
A sample of blood is obtained from your child's vein. Below-normal levels of albumin, a protein made by the liver, found in the bloodstream are associated with many chronic liver disorders.
  • bilirubin level
A sample of blood is taken from your child's vein. Bilirubin is produced by the liver and is excreted in the bile. Elevated levels of bilirubin may indicate an obstruction of bile flow or a defect in the processing of bile by the liver.
  • complete blood count (CBC)
A sample of blood is taken from your child's finger or vein, and the different types of cells in the bloodstream are examined. White blood cells multiply when infection is present. Red blood cells will be present in smaller amounts than normal if blood has been lost, if the diet has been inadequate, or with certain diseases.
