SAVING THE VOICE IN LARYNX CANCER Saving the voice in larynx cancer Reported by Susan Aldridge, PhD, medical journalist Results of a clinical trial suggest that simultaneous treatment with chemotherapy and radiation preserves the voice of patients with advanced larynx cancer without affecting survival.
Researchers at the Johns Hopkins Kimmel Cancer Center say that experience with combined treatment has reduced the need for complete removal of the larynx from 100 percent to about 15 percent. Removing the larynx leaves patients unable to speak with their natural voice and reliant upon speaking aids, even though it may cure the cancer. Other previously-studied treatment options included radiation therapy alone or several cycles of chemotherapy followed by radiation. Previous studies showed that the survival rate of patients treated with chemotherapy followed by radiation was just as good as for those receiving surgery. This new study of 547 patients shows that giving chemotherapy and radiation together instead of sequentially is more effective in preserving the voice box.
Eighty eight per cent of patients receiving chemotherapy and radiation together still had their voice box after two years compared to 75 percent receiving chemotherapy followed by radiation and 70 percent receiving radiation alone. For each of these three treatment options, overall survival was similar at about 75 percent after two years.

Lubin, Ph.D. Primary lung cancer accounts for about 15 percent of all cancer cases (19 percent in males and 11 percent in females) in the United States; however, because of its high death rate, it accounts for 29 percent of all cancer deathsÑ35 percent in males and 21 percent in females (Boring et al., 1994). The overall 5-year relative survival rate is only 13 percent (Ries et al., 1994). Lung cancer remains the leading cause of cancer death in most countries. Maoris of New Zealand experienced the highest incidence rate in the world among males, with a world standardized rate of 119.1 per 100,000 (Parkin et al., 1992). U.S. black men were near the highest, with an incidence rate of 90.0 per 100,000. Low rates were noted for Israel, India, and Latin American countries. The worldwide incidence of lung cancer is substantially lower in femalesa difference generally attributed to lighter tobacco consumption by women, although other factors may play a role. For example, high levels of indoor and outdoor air pollution have been postulated as an important contributor to the high rates of lung cancer among females in China, whose smoking prevalence is relatively low (Blot and Fraumeni, 1992). In the United States, lung cancer incidence has risen more sharply in females (4.6 percent annual change) than in males (0.5 percent annual change) in recent years, reflecting the growing popularity of cigarette smoking among females over the past several decades (Ries et al., 1994). Currently, more women die each year from lung cancer than breast cancer (Boring et al., 1994).

As cancer can spread to other organs in the body, in the very late stages there may be symptoms from the lungs or bone. Diagnostic Tests If larynx cancer is suspected, the doctor will examine for swellings or lumps in the neck. An angled mirror that faces downwards is placed against the back of the throat (the palate) to examine the voice box and surrounding organs. Or a flexible tube, two millimetres in diameter, is passed into one of the nostrils to the back of the throat to look for lumps or ulcers in the voice box and surrounding organs. Small pieces of tissue will be removed (biopsy) during this examination to confirm if there is cancer. This examination is performed under anaesthesia. If there is a neck lump, small pieces of tissue may also be removed using a needle and syringe. These procedures, although uncomfortable, usually last about 10 to 15 minutes.

Cancer of the Larynx The most frequently occurring cancer of the head and neck is cancer of the larynx. It is more common in men, although the number of women developing cancer of the larynx is increasing. Cancer of the larynx usually occurs between the ages of 50 and 75 years (check the Canadian Cancer Society web site for the most up to date statistics ). As with other forms of cancer, the cause of larynx cancer is not yet clear, but smoking and heavy use of alcohol are felt to be major contributing factors. Research continues on many fronts to find the key to why cells in various organs and tissues of the body begin a pattern of uncontrolled growth (i.e. become cancerous). Early detection and treatment are extremely important for the current forms of treatment to be most effective. Where is the larynx located? The larynx, or voice box, is a tube that connects the back of the throat to the wind-pipe. A flap of cartilage (epiglottis) at the top of the larynx prevents food and liquids from entering the larynx and wind-pipe. Sound is produced when air from the lungs passes through the vocal cords (glottis) and causes them to vibrate. The vocal cords are located just below the epiglottis at the top of the larynx.
